Vitamin D is a fat-soluble vitamin that is found in some animal food products and is also synthesized in our bodies via exposure to the sun. It is frequently used as both prescription and over the counter formulation. The toxicity of vitamin D (hypervitaminosis D) is rare and is usually caused by taking heavy doses of vitamin D due to misuse of over the counter supplements or erroneous prescriptions. Toxicity of vitamin D leads to hypercalcemia and imbalance in the regulation of bone metabolism, and the resultant hypercalcemia leads to clinical manifestations of symptoms of toxicity.Bladder trauma is an uncommon injury that can be caused by a direct blow to a distended bladder, high energy injury which disrupts the pelvis, penetrating, and iatrogenic injuries. Bladder traumas are divided into broad categories of extraperitoneal (EP), intraperitoneal (IP), or combined injuries which guide the management plan. There is limited data regarding morbidity and mortality associated with isolated bladder injuries as patients often present with concurrent injuries. Injuries to the bladder occur in up to 10% of abdominal trauma and may be associated with significant morbidity and mortality (10% to 22%).Itraconazole can treat many different fungal infections. Many of these fungal infections are rare; however, they can be detrimental to the immunocompromised. Itraconazole treats blastomycosis, histoplasmosis, and aspergillosis and has FDA approval as a treatment for these infections. However, itraconazole has also shown efficacy in treating paracoccidioidomycosis, coccidioidomycosis, and candidiasis, but it does not have approval from the FDA for these conditions. In addition to treating infections, itraconazole can be used as prophylaxis in patients at risk of these systemic fungal infections. Some patient populations that frequently use itraconazole prophylaxis are patients with HIV, those receiving chemotherapy, or people who have had an organ transplant. Itraconazole offers excellent prophylaxis for these immunocompromised patients due to its broad-spectrum coverage, safety profile, and minimal fungal resistance.Axillary lymphadenectomy, or axillary dissection, is a procedure where a surgeon dissects out the lymph nodes within the axilla en bloc. This is done most commonly for cancer workups and treatment. This procedure used to be done widely, but it is done much more selectively in recent years with advances in early detection and treatment, as well as numerous studies showing no increased benefit to this procedure in certain circumstances. While the focus of this article will be on breast cancer, it is important to note that sentinel lymph node biopsy and axillary lymphadenectomy can be performed for lung cancer and melanoma as well.Healthcare systems must continuously evolve, often allocating significant resources to create new clinical spaces that support changes in healthcare delivery. Opening these new spaces to the demands of medical practice is wrought with a diverse spectrum of challenges ranging from simple space configurations to complex multimodal, multidisciplinary processes. Training interprofessional teams using established hospital processes while incorporating new workflows in new clinical environments is extraordinarily complicated. Despite extensive planning for these spaces, weaknesses, inefficiencies, and even safety issues will likely remain - and potentially only reveal themselves after the first patients arrive. In-situ simulation training has been shown to identify latent safety threats. and provide a solution to develop, test, and refine new processes, workflows, and spaces before full operation. Healthcare simulation continues to rapidly evolve its methods to achieve valuable healthcare goals, including efficiency, quality, and safety. Moving from basic skill acquisition to more complex realms – knowledge translation, complex decision-making under stress, interprofessional team performance, workflow, and failure-point analysis – healthcare simulation is being applied to virtually every facet of care delivery. The use of fully immersive, high-fidelity, in-situ simulation to evaluate new clinical spaces reflects a natural progression and impactful application for these techniques and has shown to increase organizational learning. Creating a simulation program to evaluate a new clinical space involves key steps and deliberate application to ensure successful implementation.Sphincter of Oddi dysfunction (SOD) is a rare clinical entity, often forgotten in the typical differential. When identified, its treatment has the potential to improve a patient's quality of life significantly. A diagnosis of exclusion, the typical patient will present with recurrent biliary colic type symptoms, generally after undergoing cholecystectomy, often in concert with transaminitis, pancreatitis, or both. More severe cases can progress to clinically apparent obstructive jaundice and chronic pancreatitis, although rare. While several of these signs/symptoms are also present in more morbid disease processes, once ruled out, attention should be turned to Sphincter of Oddi dysfunction as a possible cause for the patient's complaints.Betaxolol is a lipophilic beta-adrenergic blocker that is selective to the beta-1 adrenergic receptor, with a limited beta-2 blocking effect. This factor is an important distinction compared to topical nonselective beta-blockers. Administration of the drug can be either via topical or systemic routes. The FDA primarily indicates topical betaxolol for use in patients with ocular hypertension and chronic open-angle glaucoma. In the systemic form, the FDA approves its use to treat essential hypertension, post-myocardial infarction, and chronic stable angina. Similar to the other beta-1 selective beta-blockers, betaxolol is associated with a decreased risk of stroke, congestive heart failure, coronary artery disease. It also demonstrates decreased mortality and morbidity post-myocardial infarction. The non-FDA uses for betaxolol include tremor reduction, migraine prophylaxis, certain cardiac arrhythmias, and anxiety disorders. Recently, one study showed the use of topical betaxolol in the treatment of relapsing paronychia, also known as a skin infection around a fingernail.
