Transcatheter mitral valve-in-valve replacement (TMVR) is a feasible alternative in high-risk patients requiring reoperation for failing mitral bioprosthesis. Such patients may present with hemodynamic instability or sudden complications, which may jeopardize the outcomes. We report a successful transapical TMVR in a patient, with severe kyphoscoliosis and on prolonged mechanical ventilation, with prophylactic extracorporeal membrane oxygenator support. This combined procedure may be helpful to reduce the complications of TMVR in critically ill subjects.The transanal and transperineal endoscopic approaches are useful advanced surgical options for removing rectal and anorectal cancers. Intraoperative carbon dioxide (CO2 ) embolisms, however, have been increasingly reported as potentially fatal complications associated with surgery employing these approaches. We report our experience with a CO2 embolism that was detected because of a sudden drop in end-tidal CO2 with decreasing saturation of percutaneous arterial oxygen during total pelvic exenteration using the transperineal endoscopic approach under pneumopelvis/pneumoperitoneum. Transesophageal echocardiography confirmed that it was a CO2 embolus. We reversed the pneumopelvis and pneumoperitoneum, which alleviated the cardiopulmonary problems, and the surgery then proceeded to achieve R0 resection. The patient was discharged without severe complications other than the CO2 embolism.
This study was designed to assess the clinical presentation of patients with H-type hypertension who were of Zhuang nationality in Guangxi, China. The relationship between the C677T polymorphism in the MTHFR gene and H-type hypertension was also assessed.
This was a case-control study in which 185 Zhuang nationality patients with hypertension that had been hospitalized at the Wuming Hospital of Guangxi Medical University between February 2018 and December 2018 were assessed for plasma homocysteine (Hcy) levels. These levels were used to divide patients into H-type (>15μmol/L) and non-H-type (≤15μmol/L) hypertension groups. Patient clinical data were then analyzed, and PCR was used to analyze samples from all patients for the presence of the C677T polymorphism in the MTHFR gene. Differences between these two groups of hypertension patients were then compared using appropriate statistical methods.
We found that relative to patients in the non-H-type hypertension group, patients in the H-type hypertension group exhibited significant differences in sex, age, urea nitrogen levels, creatinine levels, and uric acid levels. There were, however, no significant differences between these two groups with respect to interventricular septum thickness, left ventricular posterior wall thickness, or ejection fraction. We did not detect any association between the MTHFR gene C677T polymorphism and H-type hypertension in Zhuang nationality individuals in Guangxi.

The distinction between MPLC and intrapulmonary metastasis (IM) is of great importance to clinical treatment and prognosis. Here, we report the case of a patient with four lung cancers (tumor 1, named T1, in the right middle lobe seven years earlier; tumor 2, named T2, in the left lower lobe; tumor 3 and tumor 4, named T3 and T4, in the left upper lobe) and two tumors (T1 and T2) which shared the mutation in epidermal growth factor receptor (EGFR) L858R/T790M based on targeted multigene sequencing, which indicate that these two tumors might have originated from a common ancestor. However, based on previously published guidelines, these three tumors (T2T4) were diagnosed as mMPLC.
Pancreatic ductal adenocarcinoma (PDAC) is regarded as incurable, with a limited survival rate after curative resection. The aim of this study was to explore long-term survival and late recurrence of PDAC after surgery.
Medical data of 859 patients who underwent resection between 1995 and 2014 were retrospectively examined. The clinicopathological features of the 5-year recurrence-free survivors and the patients with recurrent disease after 5years were investigated separately.
Among the 768 patients who were finally included in this study, elevated CA 19-9, tumor size, poor differentiation, and positive lymph node metastasis were associated with recurrence. In 89 patients with 5-year RFS, age, tumor size, differentiation, and lymph node metastasis were statistically significant predictive factors. Among these patients, disease relapse occurred in 11 patients; age was the only difference compared to those who remained free of recurrence.
Most prognosticators failed to predict the risk of recurrence in the 5years following surgery for PDAC, and recurrence can occur even at time points up to 100months. Therefore, cure of PDAC cannot be guaranteed by a 5-year recurrence-free interval, and further studies into the inherent nature of PDAC are needed to develop adequate surveillance systems which may lead to improvements in survival.
