However, conventional MSC culture on planar (2D) plastic surfaces has been shown to induce MSC senescence and decrease cell functionality over long-term proliferation, and usually, it has a high labor requirement, a high usage of reagents, and therefore, a high cost. In this Review, we describe current MSC-based therapeutic strategies and outline the important factors that need to be considered when developing next-generation cell expansion platforms. To retain the functional value of expanded MSCs, ex vivo culture systems should ideally recapitulate the components of the native stem cell microenvironment, which include soluble cues, resident cells, and the extracellular matrix substrate. We review the interplay between these stem cell niche components and their biological roles in governing MSC phenotype and functionality. We discuss current biomimetic strategies of incorporating biochemical and biophysical cues in MSC culture platforms to grow clinically relevant cell numbers while preserving cell potency and stemness. This Review summarizes the current state of MSC expansion technologies and the challenges that still need to be overcome for MSC clinical applications to be feasible and sustainable.
This article summarizes key elements of the 2020 Harold A. Stein Lecture presented at the Eye and Contact Lens Special Symposium "Healing the Ocular Surface What's New for Neurotrophic Keratitis." The major point that I made in this talk is that we need to think about neurotrophic keratopathy as ophthalmology's diabetic foot problem; as a chronic condition that is, often best managed by prevention. Neurotrophic keratopathy is not primarily an inflammatory condition, as suggested by the term ‟neurotrophic keratitis." The epitheliopathy in this condition is a consequence of the dysregulated epithelial cell migration, differentiation, and homeostasis that occurs with denervation of the ocular surface. Although this epitheliopathy may be complicated by poor lid closure, or rarely, aqueous tear deficiency, most of these eyes are not dry and do not require exogenous lubrication for comfort, prophylaxis, or healing. The use of topical agents in these eyes inevitably leads to unrecognized iatrogenic injury, with po homeostasis that occurs with denervation of the ocular surface. Although this epitheliopathy may be complicated by poor lid closure, or rarely, aqueous tear deficiency, most of these eyes are not dry and do not require exogenous lubrication for comfort, prophylaxis, or healing. We present a case of a 30-year-old man with proximal humerus osteosarcoma and periarticular soft-tissue involvement. Severe humeral and glenoid bone loss was observed, with nonfunctional deltoid after limb-sparing resection and a failed attempt of stabilization. The shoulder was reconstructed using a reverse allograft-prosthesis composite, with deltoid reconstruction by pectoralis major transfer.

To the best of our knowledge, this is the first time that this reconstruction technique has been reported in an oncological patient. Favorable clinical and radiological results were achieved at the 24-month follow-up.

We report a case of a 56-year-old man who sustained a ruptured long head of biceps tendon (LHBT) that presented with symptoms typical of a pectoralis major rupture. Magnetic resonance imaging revealed an isolated LHBT tear. Intraoperatively, the biceps tendon had an unusually thick distal segment entrapped under the insertion of the pectoralis major. Surgery led to an excellent outcome.

An abdominal aortic aneurysm is rarely seen in children. We report the case of a 15-year-old girl who presented with lower back pain in a subfebrile setting with moderate biological inflammatory syndrome. Imaging assessment, including a spinal magnetic resonance imaging, had shown no evidence of spondylodiscitis or other spinal or renal pathology. Ten days later, she presented in hemorrhagic shock and was found to have a ruptured infected aneurysm of the abdominal aorta.

This case reminds us to include an aneurysm in the differential diagnosis of lower back pain, even in children.
