The study aimed to explore the predictive capacity of the left atrial volume index (LAVI) and the Synergy Between Percutaneous Coronary Intervention with Taxus and Cardiac Surgery (SYNTAX) score II in relation to long-term outcomes after percutaneous coronary intervention (PCI), coronary artery bypass grafting (CABG), or medical therapy (MT) in patients with TVD.
A large, prospective Chinese cohort study, analyzed post hoc, determined the long-term results of PCI, CABG, or solely optimal MT for patients with TVD. During the period from 2004 to 2011, a total of 8943 TVD-affected patients were consecutively admitted to Fuwai Hospital. The research involved 7818 patients, all of whom had baseline LAVI data available for inclusion. The collection of data included baseline, procedural, and follow-up information. A primary endpoint was defined as major adverse cardiac and cerebrovascular events (MACCE), a combination of mortality (any cause), myocardial infarction (MI), and stroke. Secondary measures of endpoint success included fatalities from all causes, fatalities from heart conditions, myocardial infarctions, revascularization procedures, and cerebral vascular incidents. Long-term outcomes were assessed across groups categorized by LAVI quartiles.
During a 66-year median follow-up, a higher LAVI was strongly predictive of increased risks for MACCE (Q3 HR 120 [95% CI 106-137], P = 0.0005; Q4 HR 185 [95% CI 164-209], P <0.0001), all-cause mortality (Q3 HR 141 [95% CI 117-169], P <0.0001; Q4 HR 254 [95% CI 216-300], P <0.0001), and cardiac mortality (Q3 HR 181 [95% CI 139-237], P <0.0001; Q4 HR 347 [95% CI 271-443], P <0.0001). LAVI's effect on the SYNTAX score II was substantial in its improvement of both discrimination and reclassification. It is noteworthy that the LAVI quartile divisions displayed a substantial interaction with the treatment strategies for mitigating MACCE. CABG's association with a lower MACCE risk than MT alone persisted across various LAVI quartile strata. The long-term effectiveness of CABG procedures exceeds that of MT alone, regardless of the LAVI quartile range. For patients with a severely elevated left atrial volume index (LAVI), percutaneous coronary intervention (PCI) is linked to a more elevated risk of cardiac death than coronary artery bypass grafting (CABG).
