Although used in flap surgery, there are no reports on its value in procedures using a pedicled thoracodorsal artery perforator (TDAP) flap. The aim of this study was to assess the usefulness of DIRT in preoperative perforator mapping and in monitoring intra- and postoperative flap perfusion of pedicled TDAP flaps.
This prospective study comprised 21 patients (21 flaps) scheduled for reconstructive surgery with a TDAP flap. selleck kinase inhibitor Perforator mapping was done by DIRT, handheld unidirectional Doppler ultrasound, and computer tomography angiography. Intra- and postoperative flap perfusion was assessed by clinical signs and with the use of DIRT and handheld unidirectional Doppler ultrasound.

Perforator mapping with DIRT showed that first-appearing bright hotspots were always associated with arterial Doppler sounds and suitable perforators intraoperatively. Computer tomography angiography presented useful information on the thoracodorsal artery branching pattern but was less beneficial for perforator mapping. Intra- and postoperative flap monitoring with DIRT was more useful than handheld unidirectional Doppler ultrasound and clinical signs to detect early arterial and venous perfusion problems. DIRT demonstrated that TDAP flap perfusion is a dynamic process with an increase in perfusion during the first operative days. Nineteen flaps survived, of which 3 sustained distal necrosis. Two flaps were lost due to inadequate blood perfusion.

The causes, risk of recurrence in subsequent offspring, and pathogenesis of HMH remain unclear, and the condition can involve a number of features. Among them are pronounced facial asymmetry and changes to both hard and soft tissue structures, as well as facial hemiparesis. We describe the long-term surgical management of HMH in a 3-year-old girl who presented to our department with no other systemic manifestation. We describe the treatment options as well as our approach, which included the use of botulinum toxin injections, and our considerations when choosing to employ careful preservation of the facial nerve and facial mimetic muscles (rather than facial paralysis and facial reanimation) in 2 stages using muscle and nerve grafts. We found that sequential debulking procedures undertaken at significant intervals have offered our patient improved aesthetic and functional results in comparison with the use of nonsurgical techniques, comparable to the more complex grafting technique used in facial reanimation surgery. Due, in part, to the rarity of HMH, there is currently no consensus regarding the optimal treatment approach to the condition. Our use of serial debulking rather than the more complex and problematic microsurgical approach of facial reanimation surgery offers a feasible surgical solution with both aesthetic and functional improvement for these patients.Fat grafting has been shown to improve diseased soft issue. Methods This retrospective observational cohort study of patients admitted with COVID-19 pneumonia from April 1 to May 31, 2020, excluded those aged less than 18 years, those with end-stage renal disease, and those with renal transplants. Clinical and laboratory patient data were collected. The Kidney Disease Improving Global Outcomes (KDIGO) criteria were used to define AKI. Results The mean age across the 73 included patients was approximately 54 years; about 60% were men, and nearly 58% were Bahraini nationals.
