This case describes a novel pathogenic APC variant. CASE REPORT We report the unexpected identification of a rare, pathogenic germline APC variant, p.S2627Gfs*12 in an 80-year-old man with a diagnosis of renal cell carcinoma, without any family history of APC-associated polyposis or personal history of colorectal cancer. After the identification of the APC variant, a review of the patient's medical records showed a personal history of 15 adenomatous polyps over a decade ago, with no follow-up genetic testing at the time. CONCLUSIONS This novel APC variant has not been characterized to date. The presence of the APC-p.S2627Gfs*12 variant in this patient led to the recommendation of additional cascade genetic testing and surveillance measures for any family members who tested positive for this variant. This report highlights the broad spectrum of the APC-associated polyposis features, and a mild phenotype associated with the pathogenic APC p.S2627Gfs*12 variant.BACKGROUND Autologous stem cell transplantation (ASCT) has become a standard procedure in multiple myeloma (MM) patients. selleck Cryopreservation (CRYO) of stem cells may be associated with adverse reactions of dimethyl sulfoxide. Previous studies showed that stem cell storage at 4°C (non-cryopreserved [NC] method) may have some advantages. This analysis focused on comparing the transplant-related outcomes of the 2 preservation methods. MATERIAL AND METHODS This was a cohort study of consecutive MM patients who underwent ASCT at Chiang Mai University from 2014 to 2019. Primary outcomes were time to neutrophil and platelet engraftment. Key secondary outcomes were the incidence of infusion reactions, duration of hospitalization, cost, and survival. RESULTS A total of 42 MM patients underwent ASCT. Of these, 26 patients and 16 patients underwent NC and CRYO stem cell collections, respectively. There was no difference in time to neutrophil engraftment (median 12 vs. 10.5 days, P=0.203) or platelet engraftment (median 14 vs. 12 days, P=0.809) between groups. The incidence of infusion reactions and duration of hospitalization were similar in both groups. The average cost of ASCT was 10% lower in the NC group. There was no difference in progression-free survival (median 16 vs. 22 months, P=0.701) or overall survival between NC and CRYO groups. CONCLUSIONS ASCT in MM using the NC preservation method is effective and safe compared to the CRYO method in both short-term and survival outcomes.Surgical site infection (SSI) occurs at the incisional site of a surgical procedure and usually involves the skin. The use of antibacterial courses to manage SSIs is still very challenging in clinical settings. When not used appropriately, antibacterial agents can lead to increased rates of adverse events. However, various antibacterial agents that can destroy the growth of bacteria are now available. This article aims to discuss the role of preoperative intranasal decolonization with topical povidone-iodine antiseptic in the incidence of SSI based on a review of the literature. Topical bactericidal agents can be administered intranasally before surgery to eliminate potentially harmful bacteria, including antibiotic-resistant strains of bacteria. Therefore, a few studies have recommended the use of intranasal povidone-iodine solution in the clinical setting; however, it also appears to be a promising antiseptic regimen for preoperative decontamination in patients planned to undergo surgery. Povidone-iodine is a commonly used medical antiseptic agent that is used by surgeons to promote wound healing and prevent postoperative bacterial infections. Chlorhexidine gluconate is both an antiseptic and a disinfectant, which is used to clean the skin and surgical instruments. Our review of the literature on studies on the effectiveness of intranasal povidone-iodine in the reduction of intranasal bacterial colonization and the prevention of SSI identified only 5 controlled clinical studies. One study, however, showed increased effectiveness in preventing SSI when topical intranasal povidone-iodine was combined with the use of chlorhexidine gluconate washcloths. Further large-scale controlled clinical studies are needed before proper guidelines can be made.Parasitological examination of freshwater fishes of the Phongolo River in north-eastern KwaZulu-Natal, South Africa resulted in the discovery and morphological and molecular characterisation of a new species of Wenyonia Woodland, 1923 (Cestoda Caryophyllidea). The new species from the plain squeaker, Synodontis zambezensis Peters (Siluriformes Mochokidae), is morphologically most similar to Wenyonia acuminata Woodland, 1923, a species reported from three species of Synodontis in north-eastern, western and central Africa (Sudan, Nigeria, Democratic Republic of the Congo). Both these species are markedly different from congeners by having a nematoform body and a digitiform scolex. Wenyonia gracilis sp. n. differs from W. acuminata in its general body size, length and width of main body regions (testicular and uterine regions), a posterior extension of the testes into the uterine region, numerous postovarian vitelline follicles filling the entire medulla, eggs c. 1/3 larger in size, and a scolex with an apical introvert but devoid of longitudinal furrows and a well-defined base. Wenyonia gracilis is the seventh species in the genus and the first autochthonous caryophyllidean ever reported and described from southern Africa (south of the Zambezi River).
