Skin metastases of RCC are considered a rare phenomenon of unfavorable outcomes. We present a 75-year-old male patient who developed a rapidly evolving lesion on his left cheek four years after undergoing a right radical nephrectomy for non-metastatic RCC. Immunohistochemistry of the skin lesion was diagnostic for cutaneous metastasis of renal clear cell carcinoma, which eventually led to the detection of internal malignancy recurrence by positron emission tomography. A new facial skin lesion may unmask the underlying recurrence of RCC.
Attendants of in-hospital patients are prone to undue stress resulting in depression, anxiety, melancholy and psychological distress. Hitherto available studies cater to attendants of patients admitted in critical care units and none have ventured to look into their cortisol dynamics. Herein, we have evaluated the magnitude of psychological distress and depression amongst ostensibly healthy attendants of non-critically ill patients and correlated them with cortisol dynamics. Selleckchem SCH58261 Methods Non-critically ill patients admitted to the general medicine ward were chosen by purposive sampling and one attendant was selected from each patient. Those with known risk factors, psychiatric illnesses, chronic drug intake, addictions, and overweight/obesity were excluded. Psychological distress and depression were assessed using the General Health Questionnaire 12-items (GHQ-12) and the Patient Health Questionnaire (PHQ-9), respectively. The mean duration of stay in hospital ambience prior to recruitment was 16.2±1.2 days. Based on the PHQ-9 score, 55% of the participants had depression. The median LNPC/LNSC was higher in participants with depression compared to those with no depression, however, there was no statistically significant difference. There was no significant correlation between GHQ-12/PHQ-9 scores and cortisol dynamics. This literature review tries to shed some light on the role of HIV and HAART in the cardiovascular manifestations seen in HIV-infected individuals.Introduction Hyperthyroidism is one of the primary dysfunctional thyroid diseases that results from abnormally high thyroid hormone secretion. It leads to metabolic derangement that manifests as heightened vital functions and metabolism and hyperdynamic blood circulation. Hyperthyroidism jeopardizes one's quality of life and hampers the patient's job performance. The disease is chronic but diagnosable and treatable and can be kept under control medically. We aimed to estimate the prevalence of suspected hyperthyroidism cases among a population sample in Jeddah city by using Wayne's scoring and delineation of factors associated with the disease. Materials and methods A cross-sectional study design was used. A total of 346 participants were conveniently selected from two shopping malls in Jeddah city during a five-day health awareness campaign. The data were collected by certified physicians, according to Wayne's score inventory. Results The study included 194 women (56.1%) and 152 men (43.9%). The participants' ages ranged from 16 to 56 years. The Wayne's scores showed 80.6% were euthyroid, 18.2% equivocal, and 1.2% have hyperthyroidism. Bronchial asthma was significantly associated with hyperthyroidism in men, while obesity was significantly associated with hyperthyroidism in women. Age, gender, level of education, and family history of chronic diseases, including thyroid disorders, were not significantly associated with hyperthyroidism. Conclusions Wayne's scoring for hyperthyroidism, which is a noninvasive tool, in addition to its detection of hyperthyroidism cases, can predict patients with equivocal results or those that might be subclinical for further medical assessment and follow-up. A Wayne's score can give the results of hyperthyroidism comparable to the results obtained by biochemical investigations. This tool is not in common use, but it is quite useful and should be put into broader usage.Image-guided brachytherapy (IGBT) is commonly used for patients with cervical cancer, but two-dimensional intracavitary brachytherapy (2D-ICBT) is also still utilized for certain patients. We report a patient with cervical cancer who developed vaginal fistulas of the bladder and small bowel after chemoradiotherapy with 2D-ICBT. A 61-year-old woman with stage IIB cervical cancer underwent a combination of external beam radiotherapy (EBRT) at a dose of 50.4 Gy in 28 fractions and 2D-ICBT at a dose of 22 Gy in four fractions. As packs were well inserted around the uterus in all fractions of 2D-ICBT, the doses to the surrounding organs at risk (OAR) could be likely to be kept at low levels. She developed a huge fistula between the vagina and bladder approximately 2.5 years after radiotherapy (RT). She also developed a fistula between the vagina and small bowel approximately seven years after RT and underwent bypass from the small bowel to the transverse colon. The OAR were delineated using computed tomography for EBRT planning, and the cumulative dose of 2D-ICBT plus EBRT was evaluated as the source of toxicity.
