Retained rectal foreign bodies are a common concern in patients presenting to the emergency department. Thankfully, the rates of injury from these retained objects are low. Numerous techniques have been described for the extraction of these foreign bodies, including using endoscopic snares, Foley catheters to break suction mechanisms, and various kinds of clamps to grasp the objects. Some foreign bodies may not be amenable to snaring or grasping with forceps. We describe the use of laparoscopic specimen extraction bags to remove such objects.
The patient is placed in the lithotomy position under the appropriate level of anesthesia. The extraction bag is opened out of the pouch, and the metallic ring is lubricated. The ring is then compressed with 1 hand and gently guided into the rectum between the rectal wall and the foreign body with the other hand. The ring is then allowed to expand, and the foreign body is encircled. The bag is then closed around it, and the object is removed.
The specimen-extraction bag works especially well for spherical objects and objects that are hard to grasp with clamps. We were able to extract a billiard ball and a glitter ball with this technique. It has minimal risk of injury to the rectum.

In this study, we investigated the genetic basis in an African American patient with factor V activity 3%. Custom sequence capture and targeted next-generation (NGS) sequencing of the F5 gene were undertaken followed by PCR and Sanger sequencing. Two novel variants were identified. In silico analyses correlated clinically with the patient's factor V activity and hemorrhagic tendency. A review of the literature regarding these genomic alterations is presented. We described two novel mutations causing moderate FVD. The first, Chr1g.169483698C>A with cDNA change (F5)c.6529-1G>T, occurred in a conserved nucleotide at the canonical acceptor splice site of intron 24. The second, Chr1g.169515775C>T with cDNA change (F5)c.1667G>A, was a missense variant of exon 11, affecting a highly conserved amino acid in the A2 domain. Further research into the mechanisms of F5 mutations leading to FVD and residual factor V expression are needed.

Eosinophilic esophagitis (EoE) is a chronic inflammatory disease of the oesophagus whose incidence is on the rise. Despite maximal use of guideline-based therapies including food elimination diets and steroids, many patients remain symptomatic. This review serves to summarize safety and efficacy of monoclonal antibodies in treatment of EoE.

Data regarding the effects of coronavirus disease 2019 (COVID-19) on host-microbiome alteration and subsequent effects on susceptibility and clinical course of COVID-19, especially in atopic patients, are currently limited. Here, we review the studies regarding the microbiome of atopic patients with other respiratory infections and discuss the potential role of probiotics as therapeutic targets for COVID-19 to decrease its susceptibility and severity of COVID-19.
Respiratory tract virus infection affects the gut and airway microbiome structures and host's immune function. Diverse factors in atopic diseases affect the airway and gut microbiome structures, which are expected to negatively influence host health. However, response to respiratory virus infection in atopic hosts depends on the preexisting microbiome and immune responses. This may explain the inconclusiveness of the effects of COVID-19 on the susceptibility, morbidity, and mortality of patients with atopic diseases. Beneficial probiotics may be a therapeutic adjuvant in COVID-19 infection as the beneficial microbiome can decrease the viral load in the early phase of respiratory virus infection and improve the morbidity and mortality.
