Published by BMJ.OBJECTIVE Management of thoracic aortic aneurysms (TAAs) comprises regular diameter follow-up until the indication criterion for prophylactic surgery is reached. However, this approach is unable to predict the majority of acute type A aortic dissections (ATAADs). The current study aims to evaluate the diagnostic accuracy of ascending aortic diameter, length and volume for occurrence of ATAAD. METHODS This two-centre observational cohort study retrospectively screened 477 consecutive patients who presented with ATAAD between 2009 and 2018. Of those, 25 (5.2%) underwent CT angiography (CTA) within 2 years before dissection onset. Aortic diameter, length and volume of these patients ('pre-ATAAD') were compared with those of TAA controls (n=75). Receiver operating curve analysis was performed to evaluate the predictive accuracy of the three different measurements. RESULTS 96% of patients with pre-ATAAD did not meet the surgical diameter threshold of 55 mm before dissection onset. Maximal aortic diameters (45 (40-49) mm vs 46 (44-49) mm, p=0.075) and volume (126 (95-157) cm3 vs 124 (102-136) cm3, p=0.909) were comparable between patients with pre-ATAAD and TAA controls. Conversely, ascending aortic length (84±9 mm vs 90±16 mm, p=0.031) was significantly larger in patients with pre-ATAAD. All three parameters had an area under the curve of >0.800. At the 55 mm cut-off point, the maximal diameter yielded a positive predictive value (PPV) of 20%. While maintaining same specificity levels, measurements of aortic volume and length showed superior diagnostic accuracy (PPV 55% and 70%, respectively). CONCLUSION Measurements of aortic volume and length have superior diagnostic accuracy compared with the maximal diameter and could improve the timely identification of patients at risk for ATAAD. © Author(s) (or their employer(s)) 2020. PM requires cue identification (prospective component) and remembering what should happen in response to the cue (retrospective component). We assessed neuroanatomical basis and functional relevance of PM. METHODS 84 older participants (53-94 years old, 58% male) with or without Mild Cognitive Impairment (MCI) performed PM tests, Activities of Daily Living (ADL) scale and had a structural MRI of the brain to estimate for cortical thickness and hippocampal subfield volumes. A General Linear Model cluster analysis was carried out using FreeSurfer to determine which cortical regions were correlated with PM scores. Linsitinib RESULTS Both components of PM are impaired in MCI (p less then .001). The retrospective component of PM correlates strongly with ADL (p = .005). Prospective component performance correlates positively with cortical thickness of bilateral frontal-temporal-parietal cortex and volume of CA1 of hippocampus. In contrast, the retrospective component performance correlates positively with cortical thickness of a right-lateralised fronto-temporal-parietal network and volumes of subiculum and CA3 hippocampal subfields. DISCUSSION Our neuroimaging findings complement and extend previous research into structural correlates of PM. Here, we show that there are distinct, yet, overlapping brain regions correlating with the two components of PM. PM performance provides a window into real-life functional abilities in people at risk of Alzheimer's disease and could be utilised as a marker of clinically relevant disease. OBJECTIVES To investigate whether dosiomics can benefit to IMRT treated patient's locoregional recurrences (LR) prediction through a comparative study on prediction performance inspection between radiomics methods and that integrating dosiomics in head and neck cancer cases. MATERIALS AND METHODS A cohort of 237 patients with head and neck cancer from four different institutions was obtained from The Cancer Imaging Archive and utilized to train and validate the radiomics-only prognostic model and integrate the dosiomics prognostic model. For radiomics, the radiomics features were initially extracted from images, including CTs and PETs, and selected on the basis of their concordance index (CI) values, then condensed via principle component analysis. Lastly, multivariate Cox proportional hazards regression models were constructed with class-imbalance adjustment as the LR prediction models by inputting those condensed features. For dosiomics integration model establishment, the initial features were similar, but with additional 3-dimensional dose distribution from radiation treatment plans. The CI and the Kaplan-Meier curves with log-rank analysis were used to assess and compare these models. RESULTS Observed from the independent validation dataset, the CI of the model for dosiomics integration (0.66) was significantly different from that for radiomics (0.59) (Wilcoxon test, p=5.9×10-31). The integrated model successfully classified the patients into high- and low-risk groups (log-rank test, p=2.5×10-02), whereas the radiomics model was not able to provide such classification (log-rank test, p=0.37). CONCLUSION Dosiomics can benefit in predicting the LR in IMRT-treated patients and should not be neglected for related investigations. OBJECTIVES Characterisation of tumor-infiltrating lymphocytes (TILS) population for cancer prognostication has enabled deeper understanding of tumor immune interactions in cancer immunology. We aim to examine the significance of both the density and functional status of NK cells in a cohort of Epstein Barr Virus (EBV) associated Nasopharyngeal Cancer (NPC) patients. METHODS NK TILS of 50 NPC samples were quantified on immunohistochemistry and the density of NK TILS was correlated with clinical outcomes. Next, NK cells and a panel of cytokines of 10 newly diagnosed NPC patients were characterized in both NPC tissue and peripheral circulation. Exhausted NK cells were identified using co-expression of PD-1 and/or Tim-3. Comparison of percentage of NK cells in NPC and healthy controls was performed using student t-test for two groups; and a p value of less than 0.05 values was considered significant.
