Previous work from the diagnostic error literature has provided indirect evidence that faulty clinical reasoning may be the most frequent cause of error when attaching a diagnostic label. The precise mechanisms underlying diagnostic error are unclear and continue to be subject to considerable theory informed debate in the clinical reasoning literature.
We take a theoretical approach to merging these two worlds of literature by first
using distributed cognition as a social cognitive lens (macro theory) to develop a view of the process and outcome of clinical reasoning occurring in the
- defined as the integrated clinical workplace - the natural habitat of clinicians working within teams. We then
using the novel combination of cognitive load theory and distributed cognition to provide additional theoretical insights into the potential mechanisms of error.
Through the lenses of distributed cognition and cognitive load theory, we can begin to prospectively investigate how cognitive overload is re distributed cognition to provide additional theoretical insights into the potential mechanisms of error. Implications Through the lenses of distributed cognition and cognitive load theory, we can begin to prospectively investigate how cognitive overload is represented and shared within interprofessional teams over time and space and how this influences clinical reasoning performance and leads to error. We believe that this work will help teams manage cognitive load and prevent error.Whilst nausea affects around 80% of pregnant women, hyperemesis gravidarum, an extreme form of the same, affects only 0.5% to 3%, but may lead to severe nutritional deficiency. Wernicke's encephalopathy is an acute neuropsychiatric disorder which occurs due to thiamine deficiency and needs emergency treatment to prevent neurological morbidity and mortality. Wernicke's encephalopathy is characterised by a clinical triad of oculomotor abnormalities, cerebellar dysfunction and altered mental state. SB273005 research buy Korsakoff's psychosis is a chronic condition and consequence of Wernicke's encephalopathy, resulting from its delayed treatment. Wernicke's encephalopathy is a well-known complication of chronic alcohol abuse. Not many are aware of its association with hyperemesis gravidarum. Although it is a rare complication, if not diagnosed and treated promptly, it may result in permanent and irreversible neurological sequelae. The objective of our retrospective observational study was to analyse the clinical profile and outcome (short and long term) in a rare yet preventable complication of pregnancy.Neurofibromatoses are inherited tumour-suppressive disorders that are characterised by multiple neoplastic and non-neoplastic tumours. Neurofibromatosis type 1 is a common disorder with multiple neurofibromas with widespread complications. We here report a seven-year old boy presenting with first episode of seizure and multiple café-au-lait macules but neuroimaging revealed corpus callosal changes without any focal areas of signal intensities.Morningness-Eveningness Stability Scale improved (MESSi) is an assessment tool that evaluates distinctness of daily changes as well as the person's propensity for morningness and eveningness. The aim of this study is to evaluate psychometric properties of the Turkish version of MESSi and associations of chronotypes and diurnal variations with personality, affect, sleep quality as well as validity of the scale in Turkish adolescents. The sample of this study consisted of 445 students between ages of 10-19. Participants filled in MESSi, Morningness/Eveningness Scale for Children, Composite Scale of Morningness, Personality Traits Scale (BIG-5), Positive and Negative Affect Scale (PANAS), Pittsburgh Sleep Quality Index (PSQI). Reliability analysis of the scale showed quite reliable internal consistency values. Confirmatory factor analysis (CFA) was used to test construct validity of three-factor model of MESSi. An acceptable model fit was demonstrated in CFA. There was a moderate and significant relationship between MA and DI subscale scores of MESSi. A moderate but negative correlation was found between MA and EV subscale scores of MESSi. MA and DI subscale scores of MESSi were found to be weakly correlated with conscientiousness, emotional stability, openness to experience and subscale scores of BIG-5, respectively. There was a positive correlation of positive affect subscale scores of PANAS with MA and DI subscale scores of MESSi. MA subscale scores of MESSi were found to be negatively correlated with scores of subjective sleep quality, latency, duration, disturbances, habitual sleep efficiency, daytime dysfunction, and global scores of PSQI. This study shows that Turkish version of MESSi is valid and reliable for use in adolescents. Three-factor MESSi provides an advantage over other chronotype scales in adolescence since it includes DI subscale as well as MA and EV subscales. Psychometric results were supported by other chronotype scales and clinical features such as sleep, affect, and personality.Purpose To examine two screening tools for phonological production, the Evaluation Sommaire de la Phonologie chez l'enfant d'âge préscolaire (ESPP) and the Test de Phonologie du Français Canadien-Dépistage (TPFC-D), developed according to differing theoretical perspectives. The TPFC-D, designed according to nonlinear phonology, includes more words and contains a greater variety of segments across word structure as compared to the ESPP, which was guided by a linear phonological framework. The greater response rate to test items, time of administration, and phonological complexity were expected on the TPFC-D.Method Each screening tool was administered to 14 4-year-old French-speaking children living in Central Canada. Paired samples t-tests compared children's responses on the two tasks with regards to (a) response rate and time of administration, (b) an overall percentage of consonants correct (PCC) and percentage of vowel correct (PVC), and (c) complexity of productions (i.e. PCC and PVC in relation to word structure, Word Shape Match, Whole Word Match, Phonological Mean Length of Utterance (pMLU) and Proportion of Whole-Word Proximity).
