Dysphagia worsens mortality and quality of life for persons diagnosed with amyotrophic lateral sclerosis (ALS), yet our understanding of its incidence and timing remains limited. this website In this study we sought to estimate dysphagia incidence and dysphagia-free survival over time.
Using data from the Pooled Resource Open-Access ALS Clinical Trials Database, we compared characteristics of persons with and without dysphagia upon study entry. To account for competing mortality risk, we used Kaplan-Meier curves to estimate the cumulative incidence of dysphagia and the median number of days until the development of dysphagia or death in those without dysphagia at study entry.
Patients with dysphagia upon study entry were more likely to have bulbar onset and had faster rates of functional decline and shorter diagnostic delays. The cumulative incidence of new-onset dysphagia was 44% at 1 year and 64% at 2 years after trial enrollment for those with spinal onset, and 85% and 92% for those with bulbar onset. The median duration of dysphagia-free survival after trial enrollment was 11.5 months for those with spinal onset and 3.2months for those with bulbar onset.
Our findings underscore the high risk for dysphagia development and support the need for early dysphagia referral and evaluation to minimize the risk of serious dysphagia-related complications.

To describe the clinical effect of dietary alteration as a sole change to therapy in dogs with steroid-resistant protein-losing enteropathy.
Prospective study. Eligible enrolled dogs received dietary alteration as sole change to their therapeutic plan. Canine Chronic Enteropathy Clinical Activity Index and serum albumin were monitored for the 3-month study period. Long-term follow-up data were also available for some of the study participants.
Fifteen dogs were eligible for enrollment over the study period. Twelve were enrolled, 10 remained in the study at 30 days, nine completed the 3-month study period. Following dietary alteration, eight of 10 dogs achieved complete remission, one dog achieved partial remission and one dog had no response. Seven of eight dogs achieving complete remission have remained in remission up to 4 years following study. In dogs with complete remission, median Canine Chronic Enteropathy Clinical Activity Index score was 11.5 and 4, and median serum albumin concentration was 15 g/L and 26 g/L at 0 and 14-28 days, respectively.
Dogs with protein-losing enteropathy with previous lack of response to a combination of dietary therapies, glucocorticoids and immunosuppressive medications can achieve remission following a dietary change. Improvement is likely to be seen within 14 to 30 days. A change in dietary approach may be an alternative to further immunosuppression or anti-inflammatory strategies in some dogs with difficult to treat protein-losing enteropathy.

Measurement of ruminal pH throughout a 148-day feeding period in cattle fed commercial diets and to relate this to feed intake, growth rate and feed conversion ratio. Factors contributing to variation in rumen pH, including meal frequency, duration and weight, and, total daily intake, were also evaluated.
Forty-eight cattle were randomly allocated to two pens and 12 randomly selected from each pen had rumen pH monitoring boli inserted. Ruminal pH was measured every 10 min and feed intake was measured continually. The cattle were fed a commercial feedlot diet for 148 days and weighed into and out of the feedlot to measure growth rate and to calculate feed conversion ratio. Cattle from both pens were registered to collect individual feed intake data using the GrowSafe® feed management system.
Mean ruminal pH decreased with days on feed. Mean daily dry matter intake was the major contributor to greater average daily gain and lower ruminal pH. Lower mean ruminal pH was associated with greater average daily gain and lower feed conversion ratio, where it remained above the threshold of 5.6. There was no association between ruminal pH and average daily gain or feed conversion ratio for mean ruminal pH below 5.6.
Ruminal acidosis can occur at any time during the feeding period, and the risk could be greater as days on feed increase. Feedlot production outcomes are not improved by ruminal pH depression below the threshold of 5.6.

Recruiting/retaining healthcare professionals within rural and underserved communities in the United States remains a challenge. The West Virginia University Institute for Community and Rural Health implemented a service scholarship program in 2011 to address a lack of dental and primary care.
Dental and medical students commit to practice 12-months full time following graduation in a Health Profession Shortage Area (HPSA) or Rural Urban Commuting Area (RUCA) in West Virginia for each $25,000 received.
Since inception of the program, 20 of 23 (87 percent) dental recipients had completed their service commitment, 17 (85 percent) are still practicing in WV, and 3 (13 percent) are fulfilling their commitment. Four of 20 medical recipients have repaid (2) or fulfilled (2) program commitments and 16 are completing commitments or deferred during training. Retention rate is 82 percent for both dental and medical professionals following program completion.
Service scholarships effectively engage students to provide care in rural/underserved areas and remain there after program completion.
