Adjuvant steroids have been used for the treatment of encephalitis, although there is limited data regarding its benefit. We described the use and impact of adjunctive steroids on adverse clinical outcomes (ACO) in adults with encephalitis.

Retrospective observational study of 230 adults with encephalitis at two tertiary care hospital systems in Houston, Texas, between August 2008 and September 2017. An ACO was assessed at the time of death or discharge and defined as a Glasgow Outcome Scale 1-4. A propensity score analysis was performed.

Out of 230 adult encephalitis patients enrolled, 121 (52.6%) received steroids. Adjunctive steroids were given more frequently to those who had focal neurological deficits (P = 0.01), required mechanical ventilation (MV) (P = 0.01), had intensive care unit admission (P < 0.001), had white matter abnormalities (P = 0.01) or cerebral edema on magnetic resonance imaging of the brain (P = 0.003). check details An ACO was seen in 135 (58.7%) of patients. The use of adjunctive steroids did not impact ACOs (P = 0.52) on univariate analyses or after propensity score matching. Predictors for an ACO in logistic regression analyses included a Glasgow Coma Score (GCS) < 8, fever, MV, and cerebral edema.

Adjunctive steroids are used more frequently in sicker patients and are not associated with improved clinical outcomes.

However, there are only few studies discussing long-lasting changes in bioclimatic conditions. Thus, the purpose of this paper is to present the temporal and spatial distribution of hazardous heat and cold stress conditions in different regions of Poland. Its focus is on long-lasting changes in such conditions in the period 1951-2018. To assess changes in hazardous thermal stress conditions, the Universal Thermal Climate Index (UTCI) was used. UTCI values at 12 UTC hour (respectively 1 pm winter time, 2 pm summer time) were calculated daily based on air temperature, relative humidity, total cloud cover and wind speed at 24 stations representing the whole area of Poland. We found that the greatest changes were observed in minimum (1.33 °C/10 years) and average (0.52 °C/10 years) UTCI values as well as in cold stress frequency (- 4.00 days per 10 years). The changes vary seasonally and regionally. The greatest increase in UTCImin and decrease in cold stress days were noted from November to March and had the highest values in north-east and east Poland, and also in the foothills of the Carpathian Mountains. The trends in maximum UTCI are much smaller and not always positive. The spatially averaged trend in UTCImax for Poland as a whole was 0.35 °C/10 years and the increase in heat stress days was 0.80 days/10 years. The highest increases in UTCImax and heat stress days were noted in eastern and south-eastern Poland.Among the ecosystem services provided by salt marshes is the use of their natural vegetation as pastures for livestock production. As a result, the prediction of biomass productivity can be of great interest for the sustainable management of these environments. Evapotranspiration is one of the variables most used to estimate the yield of green biomass in pastures and crops, which to date has not been examined for natural environments such as salt marshes. We studied the aboveground biomass and species cover variability for two categories (erect and sward plants) in three plots affected by low, moderate, and high cattle grazing. Erect biomass was associated only with Spartina densiflora while for sward plants it gathered a diverse set of prostrate and stoloniferous species with high seasonal turnover. The evapotranspiration was estimated with a coupled surface resistance-Penman-Monteith model developed for these environments. The biomass of the plant categories shows different growth response according to livestock impact. S. densiflora has a slow-growing response after cattle consumption, even with high evapotranspiration. On the other hand, sward plants respond with biomass overproduction to livestock consumption, and a significantly positive relationship to evapotranspiration rate.Vertebral fractures are independent risk factors for vertebral and nonvertebral fractures. Since vertebral fractures are often missed, the relatively new introduction of vertebral fracture assessment (VFA) for imaging of the lateral spine during DXA-measurement of the spine and hips may contribute to detect vertebral fractures. We advocate performing a VFA in all patients with a recent fracture visiting a fracture liaison service (FLS). Fracture liaison services (FLS) are important service models for delivering secondary fracture prevention for older adults presenting with a fragility fracture. While commonly age, clinical risk factors (including fracture site and number of prior fracture) and BMD play a crucial role in determining fracture risk and indications for treatment with antiosteoporosis medications, prevalent vertebral fractures usually remain undetected. However, vertebral fractures are important independent risk factors for future vertebral and nonvertebral fractures. A development of the DXA techering, vertebral fracture events.
Obstetric anal sphincter injury (OASI) is a severe form of perineal trauma that can occur during vaginal birth. Long-term morbidities include anal incontinence and psychosocial disorders. To reduce these injuries within England, Scotland and Wales, the OASI Care Bundle was introduced to 16 maternity units (January 2017-March 2018). The OASI Care Bundle comprises four elements (1) antenatal information, (2) manual perineal protection, (3) medio-lateral episiotomy (when indicated) and 4) recognition and diagnosis of tears. As part of the project evaluation, a qualitative study was conducted to explore women's experiences of the OASI Care Bundle.

Semi-structured interviews were conducted with women (n = 19) who received the OASI Care Bundle as part of their maternity care. This was to explore their experience of each element. A thematic analysis of the interview data was performed.

Three themes were identified (1) memories of touch, whereby women reported that a 'hands-on' approach to perineal protection was a positive experience; (2) midwife as a supportive guide, where women reported that good communication facilitated a calm birth and post-birth diagnosis; (3) education women need more information about perineal trauma.
