We describe the Quality Enhancement Research Initiative (QUERI) Impact Framework and its application to QUERI investments.

The QUERI Impact Framework adapts and expands on metrics from the National Academy of Medicine, incorporating lessons learned from QUERI initiatives. The cross-cutting impact metrics reflect QUERI's strategic methodology across five domains of impact (Alignment, Commitment, Tailoring, Informing the field, Observing healthcare changes and generating New questions/projects or ACTION). Key impact metrics, including the number of implementation facilities, number of staff trained, and number of patients served, were derived dystem, allowing both research and operations leadership to understand the value of implementation and quality improvement investments to inform program and policy decisions.
The QUERI Impact Framework aligns multiple stakeholders at different levels of a health system around common metrics, which cross implementation science and quality improvement boundaries. The Framework supports a comprehensive assessment of the short-term and distal impacts of implementation efforts in a health system, allowing both research and operations leadership to understand the value of implementation and quality improvement investments to inform program and policy decisions.
Skilled nursing facilities (SNFs) are high-risk settings for SARS-CoV-2 transmission. Infection rates among employees are infrequently described.

To describe SARS-CoV-2 rates among SNF employees and residents during a non-outbreak time period, we measured cross-sectional SARS-CoV-2 prevalence across multiple sites in the Seattle area.

SARS-CoV-2 testing was performed for SNF employees and residents using quantitative real-time reverse transcription polymerase chain reaction. A subset of employees completed a sociodemographic and symptom questionnaire.

Between March 29 and May 13, 2020, we tested 1583 employees and 1208 residents at 16 SNFs for SARS-CoV-2.

Eleven of the 16 SNFs had one or more resident or employee test positive. Overall, 46 (2.9%) employees had positive or inconclusive testing for SARS-CoV-2, and among those who completed surveys, most were asymptomatic and involved in direct patient care. The majority of employees tested were female (934, 73%), and most employees were Asian (392, 30%), Black (360, 28%), or white (360, 28%). Among the 1208 residents tested, 110 (9.1%) had positive or inconclusive results. There was no association between the presence of positive residents and positive employees within a SNF (p = 0.62, McNemar's test).

Patients are treated with antiplatelet drugs, statins and limb revascularization, but a significant number of patients are not candidate for revascularization. Literature shows that in such cases, gene therapy could be a perfect therapeutic option. The aim of our study was to evaluate efficacy of double vascular endothelial growth factor/hepatocyte growth factor (VEGF/HGF) gene therapy in patients with CLI complicated by DM. Based on the results of this study, we suggest that the therapy with pIRES/VEGF165/HGF bicistronic plasmid administration is a safe and effective method of treatment of patients with both CLI and DM. Acknowledging the proven RAΑSi benefit in patients with several diseases such as hypertension, heart failure, coronary disease, and diabetic kidney disease in the non-COVID-19 era, it is a reasonable strategy in this period of uncertainty to use these agents judiciously with careful consideration and to avoid the use of RAASi in select patients whenever possible, until definitive evidence becomes available.
