Short-term, low-dose quetiapine is used to treat postoperative delirium and insomnia. Quetiapine is contraindicated for patients with diabetes in Japan because there have been several case reports of diabetic ketoacidosis (DKA) in patients receiving long-term, high-dose quetiapine. However, because safety of short-term, low-dose quetiapine remains controversial, it is prescribed for patients with diabetes in real-world clinical practice. The present study aimed to compare in-hospital mortality and morbidity between short-term, low-dose quetiapine and risperidone in postoperative patients with diabetes.

We used a national inpatient database in Japan to perform a retrospective cohort study. We identified hospitalized patients with diabetes who underwent scheduled elective surgery and received oral quetiapine 200 mg/d or less or oral risperidone 4 mg/d or less within 7 days of surgery between July 2010 and March 2018. We performed one-to-one propensity score-matched analyses to compare outcomes between patients with quetiapine and risperidone. The primary outcome was in-hospital mortality. The secondary outcome was infectious complications (pneumonia, urinary tract infection, surgical site infection, and sepsis).

Propensity score matching created 665 pairs of patients who received quetiapine or risperidone. The primary outcome was observed in 19 (2.9%) of the quetiapine group and 11 (1.7%) of the risperidone group (relative risk, 1.27; 95% confidence interval, 0.97-1.68; P = .14). The secondary outcome did not differ significantly between the groups.

Observations of videotaped conversations of 130 Mexican-origin mothers and their adolescent (13- to 16-year-old) daughters discussing their disagreements were systematically coded. Analyses of the conversations showed that the most frequently recurring conflicts involved autonomy privileges (appearance, friendships, going out, media use), household responsibilities (chores, sibling caretaking), and family dynamics (sibling tensions, sibling differential treatment, mutual respect in communication). Daughters from traditional immigrant families who had lived longer in Mexico were just as assertive in expressing their viewpoints as daughters from less traditional families, although they were less likely to display negative affect.A 9-year-old boy presented for evaluation of variegated skin pigmentation. In this review, we analyzed the current status of eHealth in transplantation by performing a PubMed search over the last 5 years with a focus on clinical studies for post-transplant care. We retrieved 463 manuscripts, of which 52 clinical reports and eight randomized controlled trials were identified. Most studies were on kidney (n = 19), followed by liver (n = 10), solid organ (n = 7), bone-marrow (n = 6), and lung transplantation (n = 6). Eleven articles included adolescents/children. Investigated eHealth features covered the whole spectrum with mobile applications for patients (n = 24) and video consultations (n = 18) being most frequent. Prominent topics for patient apps were self-management (n = 16), adherence (n = 14), symptom-reporting (11), remote monitoring of vital signs (n = 8), educational (n = 7), and drug reminder (n = 7). In this review, we discuss opportunities and strengths of such new eHealth solutions, the implications for successful implementation into the healthcare process, the human factor, data protection, and finally, the need for better evidence from prospective clinical trials in order to confirm the claims on better patient care, potential efficiency gains and cost savings.Spatial synchrony, defined as the correlated fluctuations in abundance of spatially separated populations, can be caused by regional fluctuations in natural and anthropogenic environmental population drivers. Investigations into the geography of synchrony can provide useful insight to inform conservation planning efforts by revealing regions of common population drivers and metapopulation extinction vulnerability. We examined the geography of spatial synchrony and decadal changes in these patterns for grassland birds in the United States and Canada, which are experiencing widespread and persistent population declines. We used Bayesian hierarchical models and over 50 years of abundance data from the North American Breeding Bird Survey to generate population indices within a 2° latitude by 2° longitude grid. We computed and mapped mean local spatial synchrony for each cell (mean detrended correlation of the index among neighboring cells), along with associated uncertainty, for 19 species in 2, 26-year periods, 1968-1993 and 1994-2019.
