In some countries, including China, primary health care is rarely utilized because of medical personnel shortages at primary medical and health care institutions (PMHCIs). Several studies suggest that the most effective solution is to guide qualified doctors and medical graduates to work in PMHCIs, but the studies and measures have been formulated only from the perspective of the government and PMHCIs; few have considered the subjective willingness of medical personnel. Therefore, it is necessary to explore the measures to develop human resources of PMHCIs from the guiding object. This research was divided into two parts based on implicit theory and a lexical approach. The first part collected the factors affecting their choosing PMHCIs for employment, and the second part used exploratory factor analysis (EFA), confirmatory factor analysis (CFA), and structural equation modeling (SEM) to explore the dimensions and paths of the influencing factors. At last, seven factors were obtained from the EFA, and the SEMrole in guiding medical personnel to PMHCIs for employment. This research provides theoretical support for improving the development of human resources, guiding medical personnel to work in PMHCIs, and promoting the use of primary care services.Sarcopenia can cause various senile diseases and is a major factor associated with the quality of life in old age. To diagnose, assess, and monitor muscle loss in daily life, 10 sarcopenia and 10 normal subjects were selected using lean mass index and grip strength, and their gait signals obtained from inertial sensor-based gait devices were analyzed. Given that the inertial sensor can measure the acceleration and angular velocity, it is highly useful in the kinematic analysis of walking. This study detected spatial-temporal parameters used in clinical practice and descriptive statistical parameters for all seven gait phases for detailed analyses. To increase the accuracy of sarcopenia identification, we used Shapley Additive explanations to select important parameters that facilitated high classification accuracy. Support vector machines (SVM), random forest, and multilayer perceptron are classification methods that require traditional feature extraction, whereas deep learning methods use raw data as input to identify sarcopenia. As a result, the input that used the descriptive statistical parameters for the seven gait phases obtained higher accuracy. The knowledge-based gait parameter detection was more accurate in identifying sarcopenia than automatic feature selection using deep learning. The highest accuracy of 95% was achieved using an SVM model with 20 descriptive statistical parameters. Our results indicate that sarcopenia can be monitored with a wearable device in daily life.This article demonstrates the possibility to use a novel powerful approach based on Raman mapping of analyte solutions drop casted on a disordered array of Ag covered silicon nanowires (Ag/SiNWs), to identify the characteristic spectral signal of the four DNA bases, adenine (A), thymine (T), cytosine (C), and guanine (G), at concentration as low as 10 ng/µL, and to study their specific way of interacting with the nanostructured substrate. The results show a distinctive and amplified interaction of guanine, the base that is most susceptible to oxidation, with the nanostructured surface. In the treatment of KD, the utility of high-dose intravenous immuno-globulin (IVIG) therapy has already been clarified, and it has been established as the first-line treatment method. However, since approximately 10% of patients are refractory to this IVIG therapy and 2.6% of all patients have coronary sequelae, 500 children with KD still remain every year in Japan. In this disease, it is necessary to calm inflammation within 10 days of onset in order to suppress CALs caused by a large amount of persistent inflammatory cytokines. Indeed, the early suppression of inflammation is an effective means of suppressing the onset of CALs. Here, we describe the pathophysiology of Kawasaki disease and plasma exchange (PE), which is a therapeutic method that can calm the hyper-cytokine state of this disease. The treatment result of PE for IVIG-refractory Kawasaki disease is outstanding, and an extremely large effect can be expected if it can be started before the appearance of CALs.
