The case here described suggests the importance of patient's management in SRCs. A planned combined treatments with both surgery and RT seems to be the best choice to improve local control. RT seems to be promising within this specific histotype. Further studies are needed to confirm if the observed efficacy of combined treatments reflects in a consistent survival benefit for EMC patients.

COVID-19 is a new disease affecting and killing a large number of people across the world every day. One way to improve health care for these patients is to recognize their needs. Nurses, as a large population of health care staff, can be rich sources of information and experience on patients' care needs. Therefore, the aim of this study was to explore nurses' perception about the care needs of patients with COVID-19.
The present qualitative research was performed using the conventional content analysis approach in Iran from March to May 2020. The participants of this study included the nurses caring for patients with COVID-19, recruited by the purpose sampling method. The data was collected through 20 telephone interviews and analyzed based on the method proposed by Lundman and Graneheim.
Qualitative data analysis revealed six main categories including need for psychological consulting, need for quality improvement of services, need for upgrading of information, need for improving of social support, need for spiritual care and need for social welfare.
The data showed that patients with COVID-19 were psychologically, physically, socially, economically, and spiritually affected by the disease. Therefore, they should be comprehensively supported by health care staff and other supportive systems.

Although antioxidant substances have long been recognized as a measure of alleviating oxidative stress and restoring redox balance, the arsenal of effective means of preventing the development of various disorders is still limited. There is an emerging field that utilizes molecular hydrogen (H2 ) as a scavenger of free radicals and reactive oxygen species (ROS). Among the remarkable characteristics of H2 is its ability to counteract harmful effects of hydroxyl radical and peroxynitrite without affecting activity of functionally important ROS, such as hydrogen peroxide and nitric oxide. The beneficial effects of H2 have been documented in numerous clinical studies and studies on animal models and cell cultures. However, the established scavenging activity of H2 can only partially explain its beneficial effects, because the effects are achieved at very low concentrations of H2 . Given the rate of H2 diffusion, such low concentrations may not be sufficient to scavenge continuously generated ROS. H2 can also act as a signaling molecule and induce defense responses. However, the exact targets and mechanism(s) by which H2 exerts these effects are unknown. Here we analyzed both positive and negative effects of the endogenous H2 , identified the redox-sensitive components of the pathways affected by molecular hydrogen, and also discussed a potential role of molecular hydrogen in regulating cellular redox.ABSTRACT While models considering the relationship between emotion and risk differ, many agree that emotions should affect risk in accordance with the adaptive function of the emotion. The function of boredom has been proposed to motivate the pursuit of an alternative experience. Based on this, we predicted that a state of boredom would result in an optimistic perception of risk and increased risk-taking. In Study 1 (n = 164) and Study 2 (n = 200) participants who were made bored (relative to neutral, anger, and fear conditions) reported less worry and concern and estimated fewer deaths for causes of death. Study 3 (n = 149) showed that participants who were made bored (compared to neutral and fear conditions) perceived risk more optimistically, reported being more likely to take risks, and perceived more potential benefits from taking risks. In Study 4 (n = 84) participants who were made bored (relative to neutral) took more risks on the Balloon Analogue Risk Task, a behavioural measure of risk. These findings show that state boredom results in an optimistic perception of risk, increased self-reported risk taking, and increased risk taking. Our results support boredom as an emotion that impacts risk in line with its function.Introduction Positive expiratory pressure (PEP) and oscillating positive expiratory pressure (OscPEP) therapies are often used by people with cystic fibrosis (CF) to facilitate airway clearance. However, suboptimal adherence and poor technique may reduce their effectiveness. this website Objective To develop a device (PEPtrac) to accurately measure and provide preliminary clinical data of adherence and technique characteristics when airway clearance is performed using PEP/OscPEP devices. Methods This study comprised two distinct phases 1) a benchtop validation study; and 2) clinical study. Benchtop study Accuracy of PEPtrac was measured by comparing it to video analysis for five different PEP/OscPEP devices. Clinical study Clinical data were then collected for 18 adults with CF using one of three PEP/OscPEP devices (PariPEP S®, Acapella DH® or Aerobika®) unsupervised. Results There was 100% agreement between PEPtrac and video analysis data. Clinical data revealed significant variability in expiratory duration and pressure properties between the three PEP/OscPEP devices and between participants.
