Introduction Despite recent advances in the treatment of adult acute myelogenous leukemia (AML), the overall outcome remains dismal especially in high-risk AML patients, including the elderly and the relapsed/refractory populations. In this setting, various clinical trials have recently explored novel therapeutic agents either used alone or in combination with intensive chemotherapy or low-intensity treatments.Areas covered The current paper reviews the clinical development of glasdegib, a selective inhibitor of the Hedgehog signaling pathway through binding to its target SMO, for the treatment of AML.Expert opinion Glasdegib confirmed its efficacy and showed an acceptable tolerability, especially when used in combination either with '3 + 7' chemotherapy or with low-intensity therapies. In 2018, glasdegib was approved by the Food and Drug Administration (FDA) in combination with low-dose cytarabine for the treatment of newly diagnosed AML in patients older than 75 years or presenting with severe comorbidities.Objective The aim of this study was to evaluate the effect of cochlear implantation (CI) in advanced Ménière's disease (MD).Design The initial search on PubMed, EMBASE, and Cochrane databases yielded 171 articles; no language restriction was applied.Study sample A total of 11 articles met the inclusion criteria and were included in this systematic review. Six articles provided patient-level data on improvement in speech recognition testing after CI.Results The methodological quality of included studies was assessed by examining the study design, level of evidence, method of measurement and adequacy of outcome reporting. A random-effect model was fitted for calculating weighted means. Post-operative improvement in word recognition score (WCS) was 50.8% (95% confidence interval 34.6-67.1%); general improvement of vestibular symptoms after CI was found in 67% of the pooled patients; when reported in the studies, quality of life (QoL) and tinnitus were also generally improved after CI.Conclusions CI in advanced MD is a valid option providing good outcomes in terms of speech performances, regardless of the disease duration, uni- or bilaterality, age at implantation, previous therapeutic procedures and stage of activity of MD.We collected data from all the women with singleton pregnancies complicated by early-onset severe preeclampsia between 2008 and 2018 to identify the factor(s) that contributed to favourable neonatal outcome. Thirty women delivered the neonates with favourable outcome and the remaining 21 women delivered those with unfavourable outcome. Univariate logistic regression analysis revealed that gestational age at diagnosis ≥28 weeks (crude odds ratio [OR], 6.00), protocol-based management (crude OR 5.83), use of magnesium sulphate (crude OR, 6.00), gestational age at delivery ≥32 weeks (crude OR, 31.90), and birthweight ≥1000 g (crude OR, 10.36) were significantly associated with favourable neonatal outcome. Such management policy could contribute to improvement of the neonatal outcomes.Purpose To compare the differences in higher-order aberrations (HOAs) after laser subepithelial keratomileusis (LASEK) between two different laser platforms.Methods One hundred and seven eyes of 107 patients were included in this study. Fifty-six eyes underwent LASEK with the Triple-A profile (an ablation profile of the MEL 90 excimer laser) and 51 eyes underwent LASEK with the aspheric (Aberration Smart Ablation [ASA]) profile. Uncorrected distance visual acuity, corrected distance visual acuity (CDVA), corneal topography, and ocular aberrations were measured before and 6 months postoperatively.Results In the ASA group, the values of horizontal trefoil, vertical coma, horizontal coma, spherical aberrations, and total HOAs increased significantly after surgery (all P less then 0.05). There were no significant differences in the vertical trefoil between the preoperative and 6-month postoperative periods. In the Triple-A group, there were no differences in vertical trefoil and horizontal trefoil values between the preoperative and 6-month postoperative periods. Compared with the preoperative values, vertical coma, horizontal coma, spherical aberrations, and total HOAs were significantly increased at 6 months after surgery (all P less then 0.05). Compared to the Triple-A group, higher horizontal trefoil and horizontal coma were introduced in the ASA group at 6 months postoperatively.Conclusion The Triple-A ablation profile of the MEL 90 excimer laser at a 500-Hz pulse rate was an efficient method to correct myopia, especially for mild-to-moderate myopia, compared with the aspheric ablation model, fewer horizontal trefoil and horizontal coma were induced at 6 months after LASEK.Abbreviations HOAs, Higher-Order Aberrations; LASEK, laser subepithelial keratomileusis; ASA, Aberration Smart Ablation; UDVA, uncorrected distance visual acuity; CDVA, corrected distance visual acuity.The aim of this study was to determine the association of maternal pre-pregnancy body mass index (BMI) and pregnancy outcome among pregnant women in Croatia. Women who gave birth in Croatia in 2017 were categorised into four groups according to pre-pregnancy BMI and analysed regarding maternal characteristics and perinatal outcomes. Among 32,051 pregnant women, 5.3% were underweight, 65.5% had normal BMI, 20.4% were overweight and 8.8% were obese. Overweight and obese women had a higher prevalence of induction of labour and delivery by caesarean section compared to normal-BMI women (p less then .001), while underweight and obese women had a higher prevalence of preterm birth (p less then .001). Gestational diabetes (GD), gestational hypertension and preeclampsia were higher in overweight and obese group. Multivariate logistic regression model showed that older maternal age, higher BMI, GD, gestational hypertension and preeclampsia were significant predictors of having non-spontaneous onset of labour and of not delivering vaginally.
