Orv Hetil. 2020; 161(18) 727–737.in English, Hungarian Bevezetés és célkitűzés A világjárványt jelentő 2-es típusú diabetest (2DM) jelző géneltéréseket kerestünk – a mindennapos diagnosztika részére hozzáférhető – keringő fehérvérsejteken. Módszer Anyagcseréjüket illetően egészséges, elsőfokú, 2-es típusú cukorbeteg rokonokkal rendelkező személyek (n = 6) izombiopsziás mintáinak mRNS-microarray-eredményét hasonlítottuk ugyancsak anyagcseréjüket illetően egészséges, de negatív családi anamnézisűek (n = 6) mintáival. A microarray-eredmények kvantitatív real-time (valós idejű) PCR-rel történő megerősítése után 8 gén expressziója maradt különböző, ezeket ellenőriztük a keringő fehérvérsejteken nagy esetszámú (n = 58 anyagcseréjüket illetően egészséges, nincs cukorbeteg rokon és n = 58 egészséges, cukorbeteg rokonnal rendelkező önkéntes) vizsgálatban. Orv Hetil. 2020; 161(18) 738–746.Nowadays harmful alcohol consumption represents one of the most important risk factors for the development of several type of chronic and acute diseases in the western countries, contributing to a great number of deaths. Focusing the attention on cancer development and progression, the scientific community has a large consensus in declaring the existence of a harmful association between alcohol consumption and liver, breast, upper aerodigestive tract (mouth, oropharynx, hypopharynx, and esophagus), pancreas and colon cancer appearance. However the precise biological links by which the alcohol could be responsible for cancer initiation and progression are not fully understood yet, even if the International Agency for Research on Cancer (IARC) indicated both ethanol and acetaldehyde as carcinogen for humans. The possible explanation of the effect exerted by ethanol and acetaldehyde could be related to direct genotoxicity, hormonal disturbance, triggering of oxidative stress and inflammation. In this review, we examine the relationship between alcohol dosage and associated diseases, with focus on alcohol-related cancers. Furthermore, to understand the potential molecular mechanisms of these diseases, the results of in vivo experiments on animal models were discussed.INTRODUCTION Kidney transplantation (KT) is the gold standard for treatment of patients with end- stage-renal disease. To expand the donor reserve, it is necessary to use marginal/suboptimal kidneys. METHODS We retrospectively evaluated the short/long-term outcome of 34 KT elderly patients who received allografts with vascular abnormalities (MRA group), in comparison with 34 KT patients who received a kidney with a single renal artery (SRA group) pair-matched by age, length of time on dialysis, comorbidity and donor age. Mavoglurant mouse RESULTS All participants completed the International Physical Activity Questionnaire at KT, and then 4, 8, and 12 weeks after transplantation. Our data indicate that kidney with vascular anatomical variants may be successfully transplanted, since the overall rate of surgical complications was 20.6% in the SRA group and 17.6% in the MRA group and that the 5-year survival rate after KT was 100% in both groups. CONCLUSIONS The data also underline that individualized physical activity programs induced similar excellent results in both groups, improving physical capacities, arterial pressure, lipid metabolism, insulin sensitivity, quality of life and physical and mental status.BACKGROUND Dysphagia is a primary risk factor for pneumonia and affects around 50% of acute stroke patients. Systematic bedside swallowing screening of acute stroke patients is recommended before oral intake. Currently there is lack of comprehensive dysphagia assessment tools with robust good accuracy, clinical utility and cost-effectiveness. An altered hyoid bone movement may represent a major risk factor for aspiration . Ultrasonography quantitatively measures hyoid-larynx approximation, which was found reduced in stroke patients with dysphagia. Although ultrasonography was suggested for assessing stroke patients with dysphagia, there is lack of evidence about the acute phase of stroke. Thus, our aim was to investigate the use of ultrasonography for bedside screening of dysphagia in acute stroke patients. METHODS Nineteen acute stroke patients were included. Each patient performed clinical bedside screening for dysphagia by means of the Gugging Swallow Screen and the Functional Oral Intake Scale. Furthermore, all patients underwent ultrasonography in order to measure the distance between the thyroid cartilage and hyoid bone during swallowing (water bolus of 3 mL). The hyoid-larynx approximation distance [obtained by subtracting (a - b) the shortest distance between the hyoid bone and thyroid cartilage during swallowing (b) from the initial resting distance (a) and degree [(a - b) / a] x 100 were calculated. RESULTS The Functional Oral Intake Scale showed a significant direct association with the hyoid-larynx approximation distance (P=0.011) and degree (P=0.005). Also, the Gugging Swallow Screen showed a significant direct association with the hyoid-larynx approximation distance (P=0.008) and degree (P=0.004). The hyoid-larynx approximation distance and degree were significantly reduced in dysphagic patients. CONCLUSIONS Our findings support the use of ultrasonography in aid of swallowing clinical (non-instrumental) evaluation for the bedside screening of dysphagia in acute stroke patients.in English, Hungarian Bevezetés és célkitűzés A perifériás artériás betegségek legnagyobb alcsoportját az alsó végtagi verőérbetegségek képezik, melyek a népesség mintegy 4–6%-ában fordulnak elő. Magyarországon a vizsgált időszakban összesen 6798 major amputációra került sor. Az időben és megfelelő módszerrel elvégzett kezeléssel az alsó végtagi amputációk (melyek 30 napos mortalitása 20%, 2 éves mortalitása pedig 40–60% közötti) száma hatékonyan csökkenthető. Tanulmányunk célja feltárni az ellátásban rejlő területi különbségeket. Módszer A perifériás erek kezelése finanszírozási adatainak bemutatása során retrospektív adatelemzést végeztünk a Nemzeti Egészségbiztosítási Alapkezelő 2015–2017. évi finanszírozási adatainak felhasználásával. A halálozási arány területi különbségeit leíró statisztikával vizsgáltuk, elemezve a halálozási arányszám és egyéb faktorok közötti esetleges összefüggéseket. Eredmények A kistérségi – járási szintű – elemzés (n = 174) nagy különbségeket tárt fel az országon belül az amputációk 10 ezer lakosra jutó arányát tekintve.
