Balloon pulmonary angioplasty (BPA) offers a new treatment for inoperable patients. The national BPA service for the UK opened in October 2015 and we now describe the treatment of our initial patient cohort. Methods Thirty consecutive, inoperable, anatomically suitable, symptomatic patients on stable medical therapy for CTEPH were identified and offered BPA. They initially underwent baseline investigations including Cambridge Pulmonary Hypertension Outcome Review (CAMPHOR) quality of life (QoL) questionnaire, cardiopulmonary exercise test, 6 min walk distance (6MWD), transthoracic echocardiography, N-terminal probrain natriuretic peptide (NT pro-BNP) and right heart catheterisation. Serial BPA sessions were then performed and after completion, the treatment effect was gauged by comparing the same investigations at 3 months follow-up. We report a case of IgG4-related disease mimicking lung cancer with pleural dissemination. A 76-year-old male non-smoker was admitted to our hospital because of chest X-ray abnormality. Chest computed tomography scan showed a lung nodule measuring 26 × 14 mm with tiny nodules on the adjacent pleural surface. Wedge resection by video-assisted thoracoscopic surgery (VATS) was performed to aid diagnosis. Pathological findings of the nodule consisted of lymphoid follicular hyperplasia with lymphoplasmacytic infiltrate, fibrosis, and obstructive vasculitis. Focal and scattered thickening of the pleura with lymphoplasmacytic infiltrate was also observed. The IgG4/IgG ratio in the most prominent area exceeded 80%. Thus, we made a diagnosis of IgG4-related lung and pleural disease. To our knowledge, there has been no report of IgG4-related lung disease mimicking lung cancer with pleural dissemination. © 2020 The Authors. Alveolar soft part sarcoma are uncommon in the female genital tract. We here report a case of alveolar soft part sarcoma in a 20-year-old nullipara, presenting with vaginal bleeding and profound anemia requiring blood transfusions. Ultrasonographic examination revealed a polyp in the lower uterine segment. Surgical resection of the polyp was performed, and pathological evaluation showed typical histological, immunohistochemical, and molecular features consistent with alveolar soft part sarcoma. Patient underwent for total hysterectomy. Currently, she follows up with her surgeon and has no new complains. Given the infrequency of alveolar soft part sarcoma, this case report raises the awareness of alveolar soft part sarcoma as one of the entities to consider when confronted with a uterine polyp in a young patient. © The Author(s) 2020.Facial multiple miliary osteoma cutis is a variant of osteoma cutis usually occurring in women with a previous history of acne vulgaris. Successful ablative laser treatment has been reported using both CO2 and ErYAG lasers among other invasive treatment modalities, like surgical removal. We report a patient with biopsy-proven facial multiple miliary osteoma cutis responding to non-ablative Q-switched NdYAG laser therapy. The patient had excellent cosmesis without textural changes or hypopigmentation despite her Asian background after three sessions over 6 months. Multiple miliary osteoma cutis is therefore now amenable to non-surgical non-ablative therapy by using Q-switched NdYAG laser therapy reducing the risk of textural changes and hypopigmentation, especially in dark complexion and high-risk individuals. To our knowledge, this treatment approach has not been previously reported. © The Author(s) 2020.Objectives To compare the accuracy of averaged scores from the original Norbeck Social Support Questionnaire (NSSQ)and averaged scores from each of three new NSSQ versions (NSSQ-R.aid, NSSQ-R.n/a, and NSSQ-R.format). These three new versions of the widely used NSSQ were developed to address three previously identified concerns regarding score accuracy the Aid subscale's examples of aid, lack of an n/a response option, and the network nomination/rating procedure. Missing data rates were also assessed. Methods A convenience sample (N = 223) completed one of the four NSSQ versions. Score accuracy (restriction) was assessed by size of correlation between averaged scores (averaged score/network size) and network size, with low correlations indicating less score restriction and higher score accuracy. Fisher's r-to-z transformations assessed the significance of the difference between all correlations from the three versions. Missing data rates were assessed using chi-square tests of independence. Results The cumulative effects of removing the aid examples and use of the n/a response option improved score accuracy; averaged Aid scores from the NSSQ-R.n/a were statistically significantly less restricted than corresponding scores on the original NSSQ. read more The final version (NSSQ-R.format) actually resulted in statistically significant decreased score accuracy for averaged Affect scores. There were no statistically significant differences in missing data rates among versions. Conclusion Averaged scores from the NSSQ-R.n/a should be used.
