Copyright © 2020, Hobson et al.With increased global travel, unique diseases from different parts of the world pose a new category of differential diagnosis, and an incubation period of different infectious diseases helps narrow differential diagnosis to point to one specific etiology. The normal incubation period of Plasmodium falciparum malariais 7-40 days. We report a case of symptomatic P. falciparum malaria that manifested four years after a visit to a malaria-endemic area in a 51-year-old female patient. Our case illustrates the importance of a broad differential with regard to infectious disease including remote travel history and raises questions of the survival of P. falciparum for four years despite the administration of traditional chemoprophylactic agents. Copyright © 2020, Sleiman et al.Linear immunoglobulin A (IgA) bullous dermatosis (LABD) presents as a rare autoimmune disease that can either occur spontaneously or induced by certain drugs, the most common of which is vancomycin. LABD is a subepidermal blistering disease that is diagnosed by detecting linear IgA bands along the basement membrane. We present a case of a 59-year-old man with worsening blistering skin rash who was treated with vancomycin and piperacillin-tazobactam for pneumonia. Copyright © 2020, Dessie et al.Introduction The benefits of atropine in the treatment of acute organophosphate (OP) poisoning has been well established, while that of oximes is still uncertain. Tolebrutinib concentration Pralidoxime is the most often used oxime worldwide. In vitro experiments have consistently shown that oximes are effective reactivators of human acetylcholinesterase enzyme, inhibited by OP compounds. However, the clinical benefit of pralidoxime is still unclear. A recent meta-analysis has found that pralidoxime provides no significant improvement in outcome and rather may cause harm while increasing the economic burden in low-income communities where its use is the most prevalent. Objectives This study aimed to provide an updated evaluation of the efficacy of pralidoxime in addition to atropine alone in the treatment of patients with acute OP poisoning in terms of mortality, need for ventilator support, and the incidence of intermediate syndrome. The intermediate syndrome is a clinical syndrome that occurs 24 to 96 hours after the ingestion of an Oh Eddleston 2009 being of the lowest risk and Cherian 2005 being of high risk. The risk of mortality (RR = 1.53, 95% confidence interval (CI) 0.97 to 2.41, P = 0.07) and the need for ventilator support (RR = 1.29, 95% CI 0.97 to 1.71, P = 0.08) were not significantly different between the pralidoxime and the control group. There was a significant increase in the incidence of intermediate syndrome in the pralidoxime group (RR = 1.63; 95% CI 1.01 to 2.62, P = 0.04). Conclusions Based on our meta-analysis of the available RCTs, pralidoxime was not shown to be beneficial in patients with acute OP poisoning. Our findings are consistent with the other literature. Copyright © 2020, Kharel et al.Objective The purpose of this study was to analyze the incidence of infections in patients following placement of External Ventricular Drain (EVD) in either the Emergency Room (ER) or the Intensive Care Unit (ICU)/ Operating Room (OR) at a single Comprehensive Stroke Center. Methods Retrospective analysis of post-procedure infection rates in 710 patients with EVDs placed on site between 2010 and 2018 was performed. We analyzed cases between sex, age, stroke and non-stroke related and further requirement of conversion of the EVD to a ventriculoperitoneal (VP) shunt. Results Significant decrease in EVD related infection (ERIs) rates following the shift in EVD placement from ER to ICU/OR (from 13% to 7.7%, p=.03) among all ages, sex and type of brain injury was observed. Furthermore, our data also shows that the rate of conversion of EVDs to VP shunts is independent of the setting where EVD was placed, but increases in patients who develop ERIs. 23.1% of stroke patients that developed an ERI required a conversion to VP shunt while 67.3% of non-stroke patients that developed an ERI required further VP shunt (p less then .001) showing that non-stroke EVD patients with infections are more likely to require VP shunt. Conclusion This is one of the larger retrospective studies conducted on EVD related infections. ERIs were significantly higher when EVDs were placed in the ER. Moreover, our results highlight the relation between ERIs and further requirement of conversion EVD to VP shunt. These figures highlight the importance of focusing on infection rates, and the implications CSF infection has on the long-term care of patients. Copyright © 2020, Altschul et al.Ectopic thyroid tissue (ETT), though an uncommon finding, is prone to be clustered along the midline in the neck and rarely it shows up as a lateral neck mass. Whenever the ETT is discovered in unusual places, the possibility of malignancy is higher, and rarely a benign variant. We present a 71-year-old female with a past history of hypertension, hypercholesteremia, and thyroid nodules presented to the physician's office complaining of an unusual swelling in the right side of a neck. The physical examination revealed a rubbery, non-tender, mobile, dominant mass in the right upper neck at the jugulodiagastric region in the upper anterior cervical triangle. Ultrasonography (USG) and computed tomography (CT) of the neck strongly suggested the benign characteristics of the mass. The postoperative histological examination of the specimen was indicative of benign thyroid tissue with no metastatic potential and no lymphoid tissue confirming the diagnosis of ETT. To better understand the clinical, pathological, and radiological nature of this rare disease, we present a rare case of ETT in the lateral cervical area which was resected.
