In the present study, we conducted dosimetric analysis of incidental axillary irradiation between volumetric modulated arc therapy (VMAT) and three-dimensional conformal radiotherapy (3D-CRT). A total of 20 patients with early stage left breast cancer who underwent breast-conserving surgery followed by postoperative radiotherapy were analyzed. For VMAT plans, dose-volume constraints were not imposed on the axilla, as with 3D-CRT. We compared the dosimetric parameters of the planning target volumes, organs at risk and axillary level I-III of the two plans. VMAT showed better target coverage and a normal organ-sparing effect compared with 3D-CRT. The incidental axillary irradiation of VMAT was lower; the mean dose and the V40Gy were significantly reduced at all axillary levels, with the exception of no difference in the maximum dose to axillary level I. In conclusion, VMAT decreased incidental axillary irradiation, even in the absence of a dose-volume constraint on the axilla, and can, therefore, decrease the risk of radiotherapy-related lymphedema. However, caution is also required because it is unclear whether this incidental axillary irradiation is beneficial for reducing recurrence on the axilla. Copyright © 2020, Spandidos Publications.The aim of the present study was to retrospectively investigate the risk factors of local failure for T1 glottic carcinoma irradiated with a prescription dose of 66 Gy. Between July 2006 and December 2017, 64 patients with T1 glottic squamous cell carcinoma treated with 66 Gy/33 fractions were analyzed for risk factors of local failure. The sex, age, performance status, T stage, overall treatment time, anterior commissure involvement, smoking status during/after treatment, histological tumor grade and pretreatment hemoglobin level were investigated. The maximum, mean and minimum doses, and the homogeneity index for the glottic larynx were calculated for dosimetric risk factors of local failure. The median follow-up duration was 51 months. Local failure was observed in 6 patients (9.5%). Among all risk factors, only the minimum dose to the glottic larynx was found to be significantly associated with local failure (P=0.025). The 5-year local control rates for a minimum dose to the glottic larynx of less then 65 and ≥65 Gy were 79 and 95%, respectively, with a statistically significant difference (P=0.015). No patients exhibited grade ≥3 late adverse effects. The minimum dose to the glottic larynx was the only factor significantly associated with local failure. Thus, local control of T1 glottic carcinoma may improve with a minimum dose of ≥65 Gy to the glottic larynx. In conclusion, radiotherapy with a minimum prescription dose of ≥65 Gy to the glottic larynx appears to be safe and achieves a high local control rate for T1 glottic carcinoma. Copyright © 2020, Spandidos Publications.Flor·Essence (FE), a natural food grade herbal formula product manufactured by Flora Manufacturing & Distributing Ltd., has been used by patients with cancer in North America to stimulate immune cells in order to attenuate or reverse immune damage. To elucidate the mechanisms underlying the effects of FE on the immune system, spleen lymphocyte proliferation was analyzed by an MTT assay, and the phagocytic capacity of macrophages was measured via the neutral red phagocytosis method. The cytotoxicity of natural killer (NK) cells towards K562 cells was assessed via a CytoTox 96 assay. The production of the cytokines interleukin (IL)-12 and interferon (IFN)-γ in the peripheral blood was determined via ELISA and PCR analysis. The expression levels of caveolin-1 and NF-κB were measured via western blotting. In addition, cyclophosphamide was used to establish a mouse model of immunosuppression. It was found that the proliferation of splenocytes, the phagocytic capacity of macrophages and the cytotoxicity of NK cells against K562 cells were increased after oral administration of FE to mice. FE augmented the production of IL-12 and IFN-γ in the peripheral blood of mice. FE significantly increased the expression of proliferating cell nuclear antigen and caveolin-1, and decreased NF-κB expression. Finally, FE enhanced the viability of immune cells from cyclophosphamide-treated immunosuppressed mice. The results indicated that FE could activate immune responses and enhance natural immunity, suggesting that oral administration of FE can activate the body's immune response and resist damage caused by cyclophosphamide chemotherapy. Here, a case of anal canal carcinoma with pagetoid spread and inguinal lymph node metastasis, which exhibited a significant response to preoperative chemoradiotherapy (CRT), is presented. A 58-year-old woman was admitted to The University of Tokyo Hospital with a complaint of discomfort around the anus. Physical examination revealed an erythematous inflamed skin lesion in the perianal region and a tumor of 15 mm in diameter detected on palpation in the left inguinal region, which was diagnosed as metastatic adenocarcinoma by excisional biopsy. Colonoscopy revealed moderately differentiated adenocarcinoma of 15 mm in diameter in the anal canal. Skin biopsy of the perianal region revealed an infiltration of pagetoid cells, which were positive for cytokeratin 7, and negative for cytokeratin 20 and gross cystic disease fluid protein 15. Based on these results, the patient was diagnosed as having anal canal adenocarcinoma with pagetoid spread.
