Primary spontaneous bacterial peritonitis (SBP) is a rare cause of acute abdomen in previously healthy patients, even more unusually caused by a group A Streptococcus (GAS) (also known as Streptococcus pyogenes) infection. We report a young, otherwise healthy female who presented with generalized abdominal pain that was initially managed conservatively as gastroenteritis, with a computed tomography (CT) scan showing a ruptured corpus luteal cyst. Upon subsequent readmission with worsened pain and symptoms, a repeat CT scan showed worsened free fluid with signs of peritonitis. A diagnostic laparoscopy confirmed primary peritonitis with an unknown infection source and causative pathology, as the appendix, ovaries and bowels were healthy-looking. Fluid cultures returned positive for GAS Pyogenes, while blood and urine cultures were negative. The discussion reviews the challenges in diagnosis and treatment of GAS primary peritonitis, highlighting the need for clinical suspicion, early diagnosis via laparoscopy or laparotomy and prompt antibiotic therapy as the current standard for treatment.1,32,4-bis(3,4-dimethylbenzylidene)sorbitol (DMDBS) is highly effective in nucleation of the α- form of isotactic polypropylene (iPP). However, its role in high-pressure crystallization of iPP, facilitating the formation of the γ- polymorph, has not been explored. The present paper focuses on the influence of DMDBS on nucleation of high-pressure crystallization of iPP. iPP with 0.2-1.0 wt.% of the DMDBS was crystallized under elevated pressure, up to 300 MPa, in various thermal conditions, and then analyzed by PLM, WAXD, SEM, and DSC. During cooling, crystallization temperatures (Tc) were determined. It was found that under high-pressure DMDBS nucleated crystallization of iPP in the orthorhombic γ- form. As a consequence, Tc and the γ- form content increased for the nucleated iPP, while the size of polycrystalline aggregates decreased, although the effects depended on DMDBS content. The significant increase of Tc and the decrease of grain size under high pressure of 200-300 MPa required higher content of DMDBS than the nucleation of the α-form under lower pressure, possibly due to the effect of pressure on crystallization of DMDBS itself, which is a prerequisite for its nucleating activity.Metabolic syndrome is a group of metabolic risk factors whose combination significantly contributes to the development of the risk of cardiovascular disease, diabetes, stroke, some cancers and is a clear indicator of morbidity rate. The aim of this study was to identify physical activity programs that can successfully influence the reduction of risk factors in metabolic syndrome of the elderly. Subjects were aged between 60 and 80 years, had three of five signs of metabolic syndrome, and were randomly divided into three groups of 20 subjects. The first group conducted a continuous cycling ergometer (55% VO2max), the second group a physical activity strength program and the third was a control group. Before and after the experimental treatment body composition, biochemical parameters, functional parameters, cardiovascular functions, metabolic and hematological system were determined. Significant differences between control and experimental groups were determined using MANOVA. The training effects of the experimental and control groups were determined using the ANOVA for repeated measurements with Bonfferoni correction. The results showed that a physical activity program of strength has a better effect on disease regulation in the elderly with metabolic syndrome than a moderate-intensity physical activity program which also has a significant change but in less variables.The use of non-selective tumor necrosis factor (TNF) inhibitors is well known in the treatment of inflammatory diseases such as rheumatoid arthritis, Crohn's disease, and psoriasis. Its use in neurological disorders is limited however, due to rare adverse events of demyelination, even in patients without preceding demyelinating disease. MTPs vary across institutions, and ratios of blood components can change during clinical management. As a result, significant amounts of components can be wasted. We completed a review of all MTP activations from 2015 to 2018, providing an in-depth analysis of waste in our single Level 1 trauma center. An interdepartmental group analyzed patterns of blood component wastage to guide three quality improvement initiatives. Specifically, we (1) completed a digital timeline for each MTP activation and termination, (2) improved communications between departments, and (3) provided yearly training for all personnel about MTP deployment. The analysis identified an association between delayed MTP deactivations and waste (RR = 1.48, CI 1.19-1.85, p = 0.0005). An overall improvement in waste was seen over the years, but this could not be attributed to increased closed-loop communication as determined by the proportion of non-stop activations (F(124,3) = 0.98, not significant). Delayed MTP deactivations are the primary determinant of blood component waste. Our proactive intervention on communications between groups was not sufficient in reducing the number of delayed deactivations. However, implementing a digital timeline and regular repetitive training yielded a significant reduction in wasted blood components.The purpose of this paper is to summarize the research themes and hotspots of safety signs research between 1990 and 2019 through the scientometric analysis method. In total, 3102 articles of literature from the Web of Science core database were analyzed by the CiteSpace visualization tool and the results were displayed in mapping knowledge domains. The overall characteristics analysis showed that safety sign is an emerging research field in a rapid development stage-81.4% of the literature works were published in the past ten years, and the United States was in the leading position, followed by China and Canada. The keyword co-occurrence analysis indicated that traffic signs and driving safety were the most popular research topics and have been combined with simulation technology in recent years, whereby individual mental health has been added as an influential factor. The journals and category co-citation analysis showed that the safety signs research involved many subjects, mainly engineering, transportation and public safety.
