Lack of knowledge and understanding of dementia was evident, but the stigma about dementia was non-existent. This study fills a research gap by providing qualitative research examining knowledge about and attitudes towards dementia among a Bangladeshi community in England. Karger AG, Basel.The heart is a central organ in the fetal adaptation to an adverse environment. Fetal cardiac changes may persist postnatally and increase the risk of cardiovascular disease in adulthood. Knowledge about fetal cardiac structural as well as functional remodeling has radically improved over the last few years. As it occurs in postnatal life, the fetal heart remodels - changing its structure and shape - to adapt to an insult. Several conditions have been reported to be associated with fetal cardiac remodeling including intrauterine growth restriction, diabetes, exposure to antiretroviral drugs, conception by assisted reproductive technologies, pulmonary stenosis, and other congenital heart diseases. Here we summarized the main observable patterns of cardiac remodeling, i.e., globular shape, hypertrophy without dilation, and hypertrophy with cardiomegaly. We discuss the potential pathophysiology behind different types of remodeling. Defining precisely the distinct patterns of fetal cardiac remodeling is critical for advancing in the understanding of fetal cardiovascular programming and its consequences on adult health, and potentially for the design of preventive strategies that might have an impact on long-term cardiovascular health. © 2020 S. Karger AG, Basel.PURPOSE The aim of our study was the external validation of an extended variant of the four-tiered diagnosis-specific graded prognostic assessment (DS-GPA) that includes more information about extracranial disease burden and blood test results, and predicts survival of patients with brain metastases. The extracranial DS-GPA (EC-GPA) includes serum albumin, lactate dehydrogenase, and number of extracranial organs involved. Originally, the score was developed in Germany. PATIENTS AND METHODS A retrospective analysis of 236 patients with brain metastases treated with primary whole-brain radiotherapy in North-Norway was performed (independent external validation cohort). RESULTS The four-tiered EC-GPA score showed good discrimination between all prognostic groups (log-rank test p less then 0.05 for all pairwise comparisons). One-year survival was 0, 11, 30, and 100%, respectively. Median survival was 0.7 months (95% CI, 0.5-0.9) in the worst prognostic group, with a hazard ratio for death of 44.31 (95% CI, 5.78-339.50) compared to the best group. In the German database, the corresponding HR was 31.64 (median survival 0.4 months). The remaining hazard ratios in this validation study were 7.13 and 12.10, compared with 4.84 and 9.26 in the score development study. CONCLUSIONS This study provides an independent validation of the EC-GPA, which was the best prognostic model for defining patients who did not benefit from radiation therapy of brain metastases in terms of overall survival in the original German study. Karger AG, Basel.in English, German Hintergrund Es wurde die Multicomponent-Multitarget-Therapie SUC (Solidago compositum ad us. vet., Ubichinon compositum und Coenzym compositum, Heel GmbH, Baden-Baden, Deutschland) mit dem bekannten Angiotensin-Converting-Enzym-Inhibitor (ACEI) Benazepril in einer prospektiven, nichtrandomisierten, zweiarmigen Kohortenstudie an Katzen mit chronischer Nierenerkrankung (CNE) untersucht. Ziel war es, die Verträglichkeit und Wirksamkeit von SUC bei Katzen mit CNE zu beurteilen. Material und Methoden Einhundertsechsunddreißig Katzen mit Verdacht auf CNE wurden untersucht, bei 70 Katzen wurde eine CNE diagnostiziert und diese wurden in die Studie aufgenommen. Dreiunddreißig Katzen wurden mit SUC therapiert und 35 Katzen erhielten Benazepril. Der Beobachtungszeitraum betrug 168 Tage. Das Ansprechen auf die Therapie wurde definiert als ein verbessertes bzw. ein stabiles Serumkreatinin am Ende der Studie im Vergleich zum Ausgangswert. Zusätzlich wurde zur Bewertung der Lebensqualität der Katzen ein klinischer Summenscore erfasst. Ergebnisse Das Kreatinin blieb in beiden Studiengruppen nahezu unverändert, mit geringgradig niedrigeren Werten in der SUC-Gruppe. Der klinische Summenscore verbesserte sich in der SUC-Gruppe im Vergleich zur Benazepril-Gruppe signifikant an den Tagen 3, 28, 56 und 112, nicht aber am Tag 168. Schlussfolgerungen Die Ergebnisse zeigen, dass SUC eine neuartige und gut verträgliche Behandlungsalternative zu ACEIs bei Katzen mit leichter bis mittelschwerer CNE darstellt. Die Ergebnisse deuten darauf hin, dass eine zweimal wöchentliche Therapie mit SUC effektiver sein könnte als eine Dosierung einmal pro Woche.The relationship between the concentration of chemically soluble fluoride found in toothpaste and that present in saliva, during and after brushing, was evaluated as an indicator of potentially bioavailable fluoride in toothpaste. Ten adult participants brushed their teeth with the assigned toothpastes group I fresh sample of a fluoride toothpaste Na2FPO3/CaCO3, 1,378 µg F/g of total soluble fluoride (TSF); groups II-IV aged samples of toothpaste presenting TSF concentrations of 1,160, 900, and 597 µg F/g, respectively; group V non-F placebo toothpaste. The volunteers brushed their teeth for 1 min with 0.7 g of the toothpaste, all toothbrushing residues (TR) produced were collected, the mouth was rinsed with water, and saliva samples were collected up to 120 min. Total fluoride (TF) and TSF concentrations were determined in TR and in saliva samples using a fluoride ion-selective electrode. TSF concentration (µg F/mL) in TR was determined as an indicator of fluoride bioavailability during toothbrushing and the areas under curves of saliva fluoride concentration versus time (area under the curve, AUC = µg F/mL × min) were calculated as an indicator of fluoride bioavailability after toothbrushing.
