Our study shows that varied testing, feeding, and isolation practices resulted in favorable short-term outcomes for SARS-CoV-2-positive mothers and their newborns. High-risk populations can be safely and effectively treated in resource-limited environments.Retinopathy of prematurity (ROP) is one of the leading yet preventable causes of childhood blindness worldwide. The purpose of this review is to provide a practical template for observational and treatment methods in order to reduce the overall incidence of any ROP and to improve both short-term and long-term outcomes once Type 1 ROP (treatable ROP) develops.
To assess the prevalence and outcomes of spontaneous intestinal perforation (SIP) in very low birth weight infants.
This cross-sectional study utilized the National Inpatient Sample dataset for the years 2002-2017. All premature infants with birth weight (BW) <1500 g and with gestational age (GA) ≤32 weeks were included. Analyses were repeated after stratifying the population into two BW sub-categories <1000 g and 1000-1499 g. Trend analysis was done using Cochran-Armitage test. Regression analysis was conducted to control for gestational age, race, and sex.
A total of 658,001 infants were included. SIP (n = 10,443, 1.6%) was mostly (81.9%) in the category <1000 g with 89.9% ≤28 weeks of gestation. There was a significant trend for increased SIP over the years (p < 0.001). SIP was associated with increased mortality (aOR = 2.24, CI 2.04-2.46, p < 0.001).
After controlling for gestational age and other confounders, SIP is associated with increased mortality in premature infants.
After controlling for gestational age and other confounders, SIP is associated with increased mortality in premature infants.Pivotal to the maintenance of cellular homeostasis, macroautophagy (hereafter autophagy) is an evolutionarily conserved degradation system that involves sequestration of cytoplasmic material into the double-membrane autophagosome and targeting of this transport vesicle to the lysosome/late endosome for degradation. EPG5 is a large-sized metazoan protein proposed to serve as a tethering factor to enforce autophagosome-lysosome/late endosome fusion specificity, and its deficiency causes a severe multisystem disorder known as Vici syndrome. Here, we show that human EPG5 (hEPG5) adopts an extended "shepherd's staff" architecture. We find that hEPG5 binds preferentially to members of the GABARAP subfamily of human ATG8 proteins critical to autophagosome-lysosome fusion. The hEPG5-GABARAPs interaction, which is mediated by tandem LIR motifs that exhibit differential affinities, is required for hEPG5 recruitment to mitochondria during PINK1/Parkin-dependent mitophagy. Lastly, we find that the Vici syndrome mutation Gln336Arg does not affect the hEPG5's overall stability nor its ability to engage in interaction with the GABARAPs. Collectively, results from our studies reveal new insights into how hEPG5 recognizes mature autophagosome and establish a platform for examining the molecular effects of Vici syndrome disease mutations on hEPG5.In this study, we evaluated the effectiveness of palliative breast radiation therapy (RT), with single fraction RT compared with fractionated RT. Our study showed that both RT fractionation schemas provide palliation. Single fraction RT allowed for treatment with minimal interference with systemic therapy, whereas fractionated RT provided a more durable palliative response. Due to equivalent palliative response, at our institution we have increasingly been providing single fraction RT palliation during the COVID-19 pandemic. The COVID-19 pandemic has devastated the world with health and economic wreckage. Precise estimates of adverse outcomes from COVID-19 could have led to better allocation of healthcare resources and more efficient targeted preventive measures, including insight into prioritizing how to best distribute a vaccination. We developed MLHO (pronounced as melo), an end-to-end Machine Learning framework that leverages iterative feature and algorithm selection to predict Health Outcomes. MLHO implements iterative sequential representation mining, and feature and model selection, for predicting patient-level risk of hospitalization, ICU admission, need for mechanical ventilation, and death. It bases this prediction on data from patients' past medical records (before their COVID-19 infection). MLHO's architecture enables a parallel and outcome-oriented model calibration, in which different statistical learning algorithms and vectors of features are simultaneously tested to improve prediction of health outcomes. this website Using clinical and demographic data from a large cohort of over 13,000 COVID-19-positive patients, we modeled the four adverse outcomes utilizing about 600 features representing patients' pre-COVID health records and demographics. The mean AUC ROC for mortality prediction was 0.91, while the prediction performance ranged between 0.80 and 0.81 for the ICU, hospitalization, and ventilation. We broadly describe the clusters of features that were utilized in modeling and their relative influence for predicting each outcome. Our results demonstrated that while demographic variables (namely age) are important predictors of adverse outcomes after a COVID-19 infection, the incorporation of the past clinical records are vital for a reliable prediction model. As the COVID-19 pandemic unfolds around the world, adaptable and interpretable machine learning frameworks (like MLHO) are crucial to improve our readiness for confronting the potential future waves of COVID-19, as well as other novel infectious diseases that may emerge.Alveograph analysis is an established method for flour characterisation, and several alveograph parameters have been introduced over the years. Typically, ten parameters are found for every analysis from the air pressure curve in the modern versions of the alveograph, but the relationships between the parameters and their potential redundancy are not well described in the literature. In this work, an overview of the parameters is provided, including how they are found and what they may represent, and the integral relationship between the parameters was investigated using Pearson correlation analysis of the parameters from 532 pressure curves. The parameters G (swelling index), Dmax (maximum of first derivative), SH (strain hardening index) and K (strength coefficient) exhibited very strong correlations with other alveograph parameters (r > 0.97), and these parameters do therefore not provide additional information. The parameters P (maximum overpressure), L (abscissa at rupture), W (deformation energy), P/L (configuration ratio), Ie (elasticity index) and Dmin (minimum of first derivative) on the other hand, represent a relatively basic set of parameters that uniquely characterises various parts of the pressure curves and thus the dough rheology/physics during dough inflation.
