Although duodenal cancer is rare, no epidemiological research on this disease has been conducted in Asian countries. We aimed to elucidate the incidence and clinical features of duodenal cancer in Japan using a large-scale national database.

Data of patients with primary duodenal cancer diagnosed from January 1, 2016, to December 31, 2016, were extracted from the Japanese national cancer registry. Excluding malignant neoplasm of the Vater's ampulla, we calculated the incidence among the population as a crude number of patients with duodenal cancer divided by the total Japanese population in 2016. We performed multivariate analyses using logistic regression models to identify risk factors for advanced cancer, defined as metastatic cancer or local invasion to adjacent organs.

Data on 3005 patients were included. The incidence of duodenal cancer was 23.7 per 1000000 person-years. In total, 56.4% of cases were detected at the localized stage. In the localized cancer group, endoscopic resection was more frequently performed (48.0%), whereas in the advanced cancer group, surgery and chemotherapy were the major treatment options (39.3% and 41.5%, respectively). Multivariate analyses identified age ≥80years (odds ratio [OR], 1.489; 95% confidence interval [CI], 1.113-1.992; P=0.007), incidental detection (OR, 2.325; CI, 1.623-3.331; P<0.0001), and precise examination for symptomatic patients (OR, 10.561; CI, 7.416-15.042; P<0.0001) as independent risk factors for advanced cancer.

This review provided strong support for central nervous system (CNS) infection with SARS-CoV-2 and shed light on the neurological mechanism underlying the lethality of SARS-CoV-2 infection. Among the published data, only 1.28% COVID-19 patients who underwent cerebrospinal fluid (CSF) tests were positive for SARS-CoV-2 in CSF. However, this does not mean the absence of CNS infection in most COVID-19 patients because postmortem studies revealed that some patients with CNS infection showed negative results in CSF tests for SARS-CoV-2. Among 20 neuropathological studies reported so far, SARS-CoV-2 was detected in the brain of 58 cases in nine studies, and three studies have provided sufficient details on the CNS infection in COVID-19 patients. Almost all in vitro and in vivo experiments support the neuroinvasive potential of SARS-CoV-2. In infected animals, SARS-CoV-2 was found within neurons in different brain areas with a wide spectrum of neuropathology, consistent with the reported clinical symptoms in COVID-19 patients. Several lines of evidence indicate that SARS-CoV-2 used the hematopoietic route to enter the CNS. But more evidence supports the trans-neuronal hypothesis. SARS-CoV-2 has been found to invade the brain via the olfactory, gustatory, and trigeminal pathways, especially at the early stage of infection. Severe COVID-19 patients with neurological deficits are at a higher risk of mortality, and only the infected animals showing neurological symptoms became dead, suggesting that neurological involvement may be one cause of death.

This was a retrospective cohort study including all fetuses with a diagnosis of gastroschisis at our institution from November 2012 through October 2017. We excluded multiple gestations, pregnancies with major structural or chromosomal abnormalities, and those for which prenatal and postnatal follow-up were unavailable. Performance characteristics of ultrasound to predict being small for gestational age (SGA) were calculated for the first and last ultrasound estimations of fetal weight.

Our cohort included 75 cases of gastroschisis. At the initial ultrasound estimation, 15 of 58 (25.9%) fetuses met criteria for FGR; 48 of 70 (68.6%) met criteria at the time of the last ultrasound estimation (median, 34.7 weeks). Cesarean delivery was performed for 37 of 75 (49.3%), with FGR and concern for fetal distress as the indicatconcerns.The role of light in partitioning ecological niche space remains a frontier in understanding the assembly of terrestrial vertebrate communities and their response to global change. Leveraging recent advances in biologging technology and intensive field surveys of cloud forest bird communities across an agricultural land use gradient in the Peruvian Andes, we demonstrate that eye size predicts (1) the ambient light microenvironment used by free-ranging birds, (2) their foraging niche, and (3) species-specific sensitivity to agricultural land use change. For 15 species carrying light sensors (N = 71 individuals), light intensity levels were best explained by eye size and foraging behavior, with larger-eyed species using darker microenvironments. Across the cloud forest bird community (N = 240 species), hyperopic ("far-sighted") foragers, (e.g., flycatchers), had larger eyes compared to myopic ("near-sighted") species (e.g., gleaners and frugivores); eye size was also larger for myopic insectivores that foraged in the forest understory. 2,3-Butanedione-2-monoxime price Eye size strongly predicted sensitivity to brightly lit habitats across an agricultural land use gradient.
