Protein carbamylation is a post-translational protein modification caused, in part, by exposure to urea's dissociation product cyanate. Carbamylation is linked to cardiovascular outcomes and mortality in dialysis-dependent end-stage kidney disease (ESKD), but its effects in earlier pre-dialysis stages of chronic kidney disease (CKD) are not established.
We conducted two nested case-control studies within the Chronic Renal Insufficiency Cohort Study. First, we matched 75 cases demonstrating CKD progression [50% estimated glomerular filtration rate (eGFR) reduction or reaching ESKD] to 75 controls (matched on baseline eGFR, 24-h proteinuria, age, sex and race). In the second study, we similarly matched 75 subjects who died during follow-up (cases) to 75 surviving controls. Baseline carbamylated albumin levels (C-Alb, a validated carbamylation assay) were compared between cases and controls in each study.
At baseline, in the CKD progression study, other than blood urea nitrogen (BUN) and smoking status, there were no significant differences in any matched or other parameter. In the mortality group, the only baseline difference was smoking status. Adjusting for baseline differences, the top tertile of C-Alb was associated with an increased risk of CKD progression [odds ratio (OR) = 7.9; 95% confidence interval (CI) 1.9-32.8; P = 0.004] and mortality (OR = 3.4; 95% CI 1.0-11.4; P = 0.05) when compared with the bottom tertile. C-Alb correlated with eGFR but was more strongly correlated with BUN.

Understanding organizational readiness has been used to assess and address such barriers but the employment of readiness assessments has largely been done in high-income countries. We conducted a qualitative study to describe the institutional needs and barriers in KT specific to academic institutions in low- and middle-income countries. We conducted a review of the grey and published literature to identify country health priorities and established barriers and facilitators for KT. Key-informant interviews (KII) were conducted to elicit perceptions of institutional readiness to conduct KT, including experiences with KT, and views on motivation and capacity building. Participants included representatives from academic institutions and Ministries of Health in six countries (Bangladesh, Democratic ators for both individuals and institutions can inform the development of capacity building strategies that meet readiness needs.
The goal of this study was to examine the behavioral effects and to suggest possible underlying mechanisms of binocularity on audiovisual temporal perception in normally-sighted individuals.
Participants performed two audiovisual simultaneity judgment tasks-one using simple flashes and beeps and the other using audiovisual speech stimuli-with the left eye, right eye, and both eyes. Two measures, the point of subjective simultaneity (PSS) and the temporal binding window (TBW), an index for audiovisual temporal acuity, were derived for each viewing condition, stimulus type, and participant. The data were then modeled using causal inference, allowing us to determine whether binocularity affected low-level unisensory mechanisms (i.e., sensory noise level) or high-level multisensory mechanisms (i.e., prior probability of interring a common cause, pC=1).
Whereas for the PSS there was no significant effect of viewing condition, for the TBW, a significant interaction between stimulus type and viewing condition ensory noise affecting the measurement of physical asynchrony during audiovisual temporal perception.
The purpose of this study was to quantify hyper-reflective lesions on en face optical coherence tomography (OCT) and study its functional relevance in macular telangiectasia type 2 (MacTel).
This was a retrospective, cross-sectional cohort study.
Baseline image and functional data from participants of a phase II clinical trial (NCT01949324) that studied the effect of Ciliary Neurotrophic Factor in patients with MacTel were analyzed. The projection of hyper-reflectivity within different layers on OCT was used to generate an en face view and measure the en face size of hyper-reflectivity. Ellipsoid zone (EZ)-loss was additionally evaluated, and en face images were superimposed onto microperimetry sensitivity maps, allowing to estimate mean retinal sensitivity within areas displaying hyper-reflectivity and EZ-loss, respectively. Best-corrected visual acuity (BCVA) and reading speed were also analyzed.
Fifty-two eyes from 52 patients were analyzed. Hyper-reflectivity was present in 32 eyes (62%), and EZ-l impairment, leading to an almost complete loss of retinal sensitivity as observed on microperimetry.The physical inputs to our visual system are dictated by the interplay between lights and surfaces; thus, for surface color to be stably perceived, the influence of the illuminant must be discounted. To reveal our strategy to infer the illuminant color, we conducted three psychophysical experiments designed to test our optimal color hypothesis that we internalize the physical color gamut under various illuminants and apply the prior to estimate the illuminant color. In each experiment, we presented 61 hexagons arranged without spatial gaps, where the surrounding 60 hexagons were set to have a specific shape in their color distribution. We asked participants to adjust the color of a center test field so that it appeared to be a full-white surface placed under a test illuminant. Results and computational modeling suggested that, although our proposed model is limited in accounting for estimation of illuminant intensity by human observers, it agrees fairly well with the estimates of illuminant chromaticity in most tested conditions.
