With the diversity of prosthetic components patients with a transtibial amputation can now expect to return to high function. Subsequently, the ipsilateral knee is at risk of developing osteoarthritis with a need for a provision of total knee arthroplasty (TKA).

We describe a novel technique for TKA in a transtibial amputee utilizing navigation with a tibial jig. Post-operative radiographs revealed varus tibial alignment with neutral limb alignment. Resolution of stump swelling allowing accurate prosthesis fitting was seen at 8 weeks. A new prosthesis optimized functional alignment and a return to moderate labor was achieved at 10 weeks. At 1-year postoperatively, patient satisfaction was excellent.

We herein report a case of idiopathic unilateral external torsion of the tibia treated with Taylor spatial frame (TSF) fixation combined with performance of the wake-up test under anesthesia. The wake-up test is performed toward the end of a surgical procedure after all corrections have been made and hardware has been placed. The patient is slowly awakened and asked to move their feet. After recovery from anesthesia, patients do not remember performing this test.

A 17-year-old Japanese male patient complained of a gait disturbance because of external torsion of his foot. The patient was determined to have lateral tibial torsion with a pre-operative foot progression angle of 40.8°. Deformity correction using the TSF was considered to be safe. The tibia was derotated by -34.8°, and the patient had a post-operative foot progression angle of 7.0°. His symptoms and signs improved after derotation osteotomy at the low tibial portion. Two years postoperatively, the patient exhibited no leg length discrepancy or rotation malalignment of the lower extremities as measured both clinically and radiographically.

To the best of our knowledge, there are no reports on the combined use of the TSF and wake-up test for acute correction of idiopathic external torsion of the tibia. Intraoperative use of the wake-up test with subsequent TSF fixation is effective and safe for acute correction of limb deformities.

Atypical femoral fractures (AFFs) have an unpredictable healing process leading to higher complication rates. The literature describes various treatment modalities of non-unions in AFF. We aimed to report a case of non-union with AFF and successful union with exchange K-nailing and augmented plating techniques.

A 75-year-old lady with 5 years of bisphosphonate intake sustained an AFF at the mid-diaphyseal level after a trivial injury which was fixed with closed intramedullary nailing. Selleck Molidustat Nine months after fixation, she developed refracture at the fracture site with segmental breakage of the nail suggesting non-union. This was managed by the removal of fibrous structures at the fracture site, removal of a broken nail, exchange K-nailing, augmented plating, and iliac crest bone grafting. The fracture healed at 15 months from the injury and achieved good functional outcome at a short-term follow-up of 1 year.

Subacute osteomyelitis (OM) is a difficult condition to diagnose and treat, further complicated with an atypical presentation of a Brodie's abscess (BA). BA is typically seen in pediatric, male populations, with minimal incidence in adult populations. Concern for malignancy and cold abscess can preempt oncological work-up. Duration of symptoms and radiological findings are often helpful but may not always match known classical findings.

Here we present a case of subacute OM in a 30-year-old Japanese male with a distant medical history of OM as a child with a subsequent review of Brodie's versus cold abscesses in the context of an atypical, asymptomatic presentation.

Clinical history and suspicion can be crucial to determine the presence of a BA in comparison to a cold abscess in the context of subacute OM. Crossing multiple disciplines from primary care and emergency treatment to orthopedic and oncological surgery, providers must be aware of atypical presentations of OM. Here, we investigated the effects of Sophora alopecuroides L.-derived alkaloids on the gut microbiota of mice with depression-like behaviors. We first established a mouse model of depression via chronic unpredictable mild stress (CUMS) and detected changes in depression-like behaviors and depression-related indicators. Simultaneously, 16S rRNA sequencing was performed to investigate gut microbiota changes. Sophora alopecuroides L.-derived alkaloids improved depression-like behaviors and depression-related indicators in mice. The alkaloids decreased the gut microbiota diversity of CUMS mice and depleted intestinal differentially abundant "harmful" microbiota genera. Spearman analysis showed that there is a certain correlation between the differential microbiota (Lactobacillus, Helicobacter, Oscillospira, Odoribacter, Mucispirillum, Ruminococcus), depression-like behaviors, and depression-related indicators.
