In 2019, investigators analyzed 2018 National HIV Behavioral Surveillance data from people who inject drugs in Boston, MA, who met eligibility criteria. Proportions of people who inject drugs with U.S. Preventive Services Task Force-based pre-exposure prophylaxis indication were estimated by types of HIV acquisition risk in the past year injection exposure only, sexual exposure only, and overlapping injection and sexual exposures. Investigators then evaluated pre-exposure prophylaxis awareness, conversations with healthcare providers about pre-exposure prophylaxis, and self-reported pre-exposure prophylaxis use among those with and without pre-exposure prophylaxis indications.
The prevalence of pre-exposure prophylaxis indication was 92% overall pre-exposure prophylaxis uptake among and delivery to people who inject drugs.
Indication for pre-exposure prophylaxis was high, but awareness was low, conversations about pre-exposure prophylaxis with healthcare providers were uncommon, and pre-exposure prophylaxis use was extremely low. These findings highlight important areas for clinical and community-based interventions to improve pre-exposure prophylaxis uptake among and delivery to people who inject drugs.
This study is a systematic review of interventions to improve adherence to guideline recommendations for prescribing opioids for chronic noncancer pain.
Investigators searched CINAHL, Embase, MEDLINE, PsycINFO, the Cochrane Library, and Joanna Briggs Institute Evid Based Pract database from inception until June 3, 2019. Interventional studies to improve adherence to recommendations made by opioid guidelines for chronic noncancer pain in North America were eligible if outcomes included adherence to guideline recommendations or change in quantity of opioids prescribed. Data were extracted independently and in duplicate. Quantitative synthesis was performed using random effects meta-analysis. Confidence in evidence was determined using the Grades of Recommendation, Assessment, Development, and Evaluation.
A total of 20 studies (8 controlled and 12 prospective cohort) involving 1,491 providers and 72 clinics met inclusion. Interventions included education, audit and feedback, interprofessional support, sharies when prescribing opioids for chronic noncancer pain; however, the effect on patient important outcomes (e.g., overdose, addiction, death) is uncertain.Computational modeling of cardiovascular biomechanics should generally start from a homeostatic state. This is particularly relevant for image-based modeling, where the reference configuration is the loaded in vivo state obtained from imaging. This state includes residual stress of the vascular constituents, as well as anisotropy from the spatially varying orientation of collagen and smooth muscle fibers. Estimation of the residual stress and fiber orientation fields is a formidable challenge in realistic applications. To help address this challenge, we herein develop a growth based Algorithm to recover a residual stress distribution in vascular domains such that the stress state in the loaded configuration is equal to a prescribed homeostatic stress distribution at physiologic pressure. A stress-driven fiber deposition process is included in the framework, which defines the distribution of the fiber alignments in the vascular homeostatic state based on a minimization procedure. Numerical simulations are conducted to test this two-stage homeostasis generation algorithm in both idealized and non-idealized geometries, yielding results that agree favorably with prior numerical and experimental data.
