Whereas insufficient exposure can jeopardize the operation itself, over-doing the exposure might lead to complications. We developed a new technique, harnessing the strength of Virtual Reality (VR) technology in planning, rehearsal and navigation, to achieve the optimal skull base exposure for resection of tumors. VR models of patient-specific anatomy were used to rehearse the surgical exposure. From the altered models, the one with the ideal exposure was chosen, integrated with the navigation system in the operating suite, and used as a template to achieve the optimal exposure in surgery. selleck chemical The use of these VR templates is demonstrated in two cases involving skull base tumors. In both cases, over-zealous bone removal could have increased the risk of complications, and inadequate exposure would jeopardize the tumor resection. Navigation guided by the VR templates aided the creation of the "ideal" surgical exposure to reach the surgical goals. Complete resections were achieved and neither patient suffered any approach-related complications. In conclusion, virtual reality is a powerful tool to improve the safety and efficacy of neurosurgical procedures. With preoperatively-altered VR templates, the surgeon is no longer navigating just to find bearings, but to duplicate an opening designed to simultaneously provide sufficient exposure while limiting postoperative complications. Intuitively useful and successful in early application, there has been no identifiable disadvantages to date.While the majority of brain metastases arise from lung cancer, breast cancer, or melanoma, new treatments and improved prognoses have altered the profile of primary cancers that metastasize to the brain. We sought to determine the proportion of brain metastases from less common primary sites and conduct trend analyses. We reviewed the charts of 3585 patients with brain metastases seen at our institution from 2008 to 2018. We determined the primary site for each of these patients, and the Mann-Kendall test was used to evaluate temporal trends in the yearly proportion of brain metastases originating from each primary cancer. The five most common primary sites were lung (43.0%), breast (19.9%), melanoma (8.2%), renal (5.0%), and colorectal (3.8%). The proportion of yearly brain metastases originating from breast cancer (p = 0.029) and melanoma (p = 0.013) decreased by 23.8% and 46.7%, respectively, from 2008 (0.21 breast, 0.15 melanoma) to 2018 (0.16 breast, 0.08 melanoma), while no change was found in the proportion of brain metastases from lung, renal, and colorectal cancers. Brain metastases arising from rare primary sites, defined as those comprising at most 2% of all brain metastases, increased by 34.4% (p = 0.005). Limited sample size prohibited trend analysis of other individual primary sites. We report a decrease over 11 years in the proportion of brain metastases originating from breast cancer and melanoma at our institution, and an increase in brain metastases from rare primary sites. Further work with larger, multi-center databases will enable additional evaluation of brain metastases from rare primary sites.
Although tirofiban therapy is considered a potentially effective treatment to reduce the incidence of thrombotic complications in patients receiving endovascular treatment (EVT), the safety and efficacy of tirofiban remain controversial. Our objective was to investigate the efficacy and safety of EVT plus tirofiban therapy in patients with emergent large artery occlusion.
Relevant articles from randomized controlled trials (RCTs) or observational studies that compared treatment with tirofiban to treatment without tirofiban in patients undergoing EVT were retrieved from the PubMed and Embase databases. We calculated odds ratios (ORs) with corresponding 95% confidence intervals (CIs) for the safety and efficacy outcomes based on a random effects model.
Twelve studies including 2533 patients were identified for the analysis. Overall, the risk of fatal intracranial haemorrhage (ICH) was higher for the treatment with tirofiban group than for the treatment without tirofiban group in patients with large artery occlusion who underwent EVT (p=0.002), whereas the risk of any ICH, symptomatic ICH, parenchymal haematoma type 2, in-hospital mortality and 3-month mortality did not differ significantly (p>0.05). No significant differences in reocclusion rate, recanalization rate or excellent functional outcome were found between the patients treated with or without tirofiban, but significantly favourable functional outcome at 3months occurred in the tirofiban group (p=0.017).
Tirofiban administration in patients receiving EVT significantly improved 3-month favourable functional outcomes, whereas an increased risk of fatal ICH was also observed. Further rigorous trials are needed to verify the safety of tirofiban.

Subacute combined degeneration (SCD) is a demyelinating disease commonly caused by vitamin B12 deficiency. Several studies have been reported SCD could be accompanied by anemia. However, the correlation between anemia and clinical severity of SCD patients is unclear. In this study, we aim to analyze the clinical characteristics of SCD concomitant with anemia, and investigate the effect of anemia in predicting the severity of SCD.
A total 42 patients were included in the study. Clinical, laboratory, radiological findings, and outcomes from the patients were analyzed. All patients were treated with vitamin B12 for no less than 6months and a functional disability rating scale was used to evaluate severity of neurological impairment at the time of admission and 3 and 6months after admission in our study.
85.7% patients had macrocytosis. Decreased serum vitamin B12 levels were found in 27 patients (64.3%). MRI showed long-segment abnormality on the spinal cord in 22 patients. No differences in rating score were found in patients grouped by sex, age, clinical course, serum vitamin B12, or MRI manifestations at the time of admission or at the follow-up visits.
