Moreover, we find that Caper interacts with the RNA-binding protein Fmrp to regulate adult locomotor behavior. Finally, we show that caper dysfunction leads to various phenotypes that have both a sex and age bias, both of which are commonly seen in neurodegenerative disorders in humans.The pathological spectrum of intraductal papillary neoplasm of bile duct (IPNB) remains to be clarified. A total of 186 IPNBs were pathologically examined using the type 1 and 2 subclassifications proposed by Japanese and Korean biliary pathologists incorporating a two-tiered grading system (low-grade and high-grade dysplasia), with reference to four subtypes (intestinal [i], gastric [g], pancreatobiliary [pb], and concocytic [o] subtype). IPNBs were classifiable into type 1 composed of low-grade dysplasia and 'high-grade dysplasia with regular structures' (69 IPNBs), and type 2 of 'high grade dysplasia with irregular structures and complicated lesions' (117 IPNBs). Type 1 was more common in the intrahepatic bile duct (78%), whereas type 2 was frequently located in the extrahepatic bile duct (58%). Mucin hypersecretion was more common in type 1 (61%) than in type 2 (37%). IPNBs were classifiable into the four subtypes 86 iPNBs, 40 gIPNBs, 31 pbIPNBs, and 29 oIPNBs. The four subtypes were histologically evaluable with reference to the type 1 and 2 subclassifications. iIPNB and pbIPNBs were frequently classified as type 2, whereas types 1 and 2 were observed at similar rates in gIPNB and oIPNB. Stromal invasion was almost absent in type 1, irrespective of subtype, but was found in 66 of 117 type 2 IPNBs (P less then .01), and postoperative outcome was favorable in IPNBs without invasion compared with IPNBs with invasion (P less then .05). The type 1 and 2 subclassifications with reference to the four subtypes may provide useful information for understanding IPNB.
The mechanism of action (MOA) of emergency contraceptive pills (ECPs) is frequently mischaracterized. Our objective was to identify how members of the general public understand the mechanisms of ECPs.

We recruited a convenience sample from social media for a survey about reproductive health attitudes and analyzed spontaneous descriptions of how ECPs work. Among 1443 respondents, 533 mentioned an MOA in their description of ECPs. While nearly half of these responses (49.5%) stated that ECPs prevent pregnancy before fertilization occurs (in accordance with most biomedical ECP research), over 60% described a mechanism related to preventing implantation of a fertilized egg. Nine percent of responses described a postimplantation mechanism that would be considered abortion by mainstream medical standards. Person-centeredness is a critical component of quality in family planning. We previously validated an 11-item Interpersonal Quality of Family Planning (IQFP) scale. We sought to create a parsimonious version of the scale in preparation for testing its appropriateness as a patient-reported outcome performance measure.

To explore clarity and importance of each of the 11 items, we conducted English and Spanish cognitive interviews with patients who received contraceptive counseling (n=33) at 3 publicly funded California clinics. We triangulated these results with psychometric analysis of previously collected IQFP data (n=1097) to assess validity and reliability of selected item combinations.

The 11-item IQFP scale was reduced to a 4-item scale (the Person-Centered Contraceptive Counseling scale, or PCCC) that includes items evaluating provider performance regarding respect for patients, information provision, and eliciting and honoring patient preferences for birth control. Interview participants deemed thre work will investigate the validity and reliability of the 4-item PCCC as a performance measure to determine the appropriateness of its use in the quality improvement context.
Apelin-13 can be expressed in brain tissue and exert neuroprotective effects. We attempted to determine whether serum apelin-13 is a prognostic biomarker for severe traumatic brain injury (sTBI).

Of 126 sTBI patients and 126 healthy controls, serum apelin-13 concentrations were quantified using ELISA. The trauma severity was assessed by Glasgow coma scale scores and Rotterdam computerized tomography scores. The relationship between serum apelin-13 concentrations and posttraumatic 30-day mortality was assessed using multivariate analysis.

Serum apelin-13 concentrations were significantly lower in patients than in controls. Serum apelin-13 concentrations of non-surviving and surviving patients within posttraumatic 30days were strongly correlated with Glasgow coma scale scores and Rotterdam computerized tomography scores. Serum apelin-13 emerged as an independent predictor for 30-day mortality and overall survival. There was a significant discriminatory capability with respect to serum apelin-13 concentrations for the risk of 30-day death.
