MRI-based screening in women with a ≥ 25% lifetime risk of breast cancer , but no identifiable genetic mutations may be associated with false positives. This study examined the psychological impact of abnormal screens and biopsies in non-mutation carriers participating in high-risk screening with no personal history of breast cancer.
Non-mutation carriers participating in the High-Risk Ontario Breast Screening Program at two sites were mailed demographic surveys, psychological scales, and chart review consent. Scales included the Consequences of Screening in Breast Cancer questionnaire, Lerman Breast Cancer Worry Scale, and Worry Interference Scale. Missing data were managed with multiple imputation. Multivariable regression was used to assess whether abnormal screens or biopsies were associated with adverse psychological effects.
After contacting 465 participants, 169 non-mutation carriers were included. Median age was 46years (range 30-65). Over a median 3years of screening, 63.9% of women experiencedd not appear to have adverse psychological effects.While checkpoint inhibitors have been approved in patients with newly metastatic PDL1-positive triple negative breast cancer, similar clinical benefit with immunotherapy alone or in combination with chemotherapy has not been observed in patients with hormone receptor-positive, HER2- negative breast cancer in the metastatic setting. However, in the ISPY2 trial, an increase in pathologic response has been observed with the addition of immunotherapy (± PARP inhibition) to chemotherapy compared to chemotherapy alone in patients with high-risk hormone receptor-positive, HER2- breast cancer. We review strategies to enhance the immunotherapeutic activity in this subtype of breast cancer, including combinations of checkpoint inhibition with chemotherapy, endocrine therapy, PARP inhibitors, HDAC inhibitors, CDK4/6 inhibitors, and radiotherapy. Combinations with agents targeting novel immunotherapeutic targets are also discussed. Though there remains an unmet need for immunotherapy approaches in patients with hormone-receptor positive breast cancer, there are a number of approaches that may lead to increased anti-tumor activity with immunotherapy in this tumor subtype.
Diabetes Mellitus (DM) has been one of the well known risk factors of breast cancer (BC) development and also associated with adverse clinical outcomes of BC patients. Glucagon-like peptide-1 (GLP-1) receptor agonists have been used as antidiabetic therapeutic agents and recent epidemiological studies have reported their use to be correlated with increased BC risks. However, biological or pathological details have remained unknown. Therefore, in this study, we examined the status of GLP-1 receptor (GLP-1R) in BC with and without DM and correlated the findings with the clinicopathological factors of the patients to explore the possible involvement of GLP-1 in BC pathology.
We immunolocalized GLP-1R in cancer and adjacent non-pathological breast tissues in BC patients with DM (125 cases) and without DM (58 cases). We then compared the status of GLP-1R with that of fibroblast growth factor 7 (FGF7) and fibroblast growth factor receptor 2 (FGFR2), Ki-67 labeling index (Ki-67 LI) and disease free survival (DFS) of the patients and also between cancerous and non-pathological breast tissues.
GLP-1R immunoreactivity was significantly higher (p = 0.044) in the patients with DM than without in carcinoma tissues. However, this was detected only in invasive carcinoma (p < 0.01) and not in non-invasive carcinoma nor non-pathological mammary glands. SW-100 concentration FGF7 was significantly correlated with the status of GLP-1R in BC (p = 0.045). In addition, in ER positive BC cases, those with GLP-1R positive status tended to have higher Ki-67 LI of more than 14% (p = 0.070).
These findings all demonstrated the possible association between GLP-1R status and biological features of BC, especially of invasive BC in DM patients.

In a new eco-efficiency model proposed by this paper, the input indexes take into account the consumption of capital, human, resources and energy, and the environmental load caused by them. The output indexes take into account GDP, income, and tax revenue. An optimal weighted cross-evaluation efficiency (OWCE) model based on data standardization is proposed, by improving the traditional data envelopment models of CCR and BCC. The OWCE model not only objectively weights but also unifies the comparison scale, and facilitates the establishment of the super-efficiency decomposition model, which is conducive to further exploring the reasons for the difference of eco-efficiency in various regions. Empirically, the eco-efficiencies of 11 provinces (municipalities) along the Yangtze River Economic Belt (YREB) were analyzed based on the data from 2008 to 2019. The results show that there has been a serious imbalance in the 11 provinces, showing a trend of high in the east and low in the west, although the eco-efficiency has been improving continuously in the past 10 years. Shanghai, Zhejiang, and Jiangsu, which are located in the traditional Yangtze River lower delta region, are the top in terms of eco-efficiency, among which Shanghai ranks the first place with absolute advantage, and also is far ahead in sub-efficiencies of basic input, energy consumption, capital and human input, and environmental cost. Geographical location, especially whether it is close to the ocean, and the length of river flow have a certain positive impact on eco-efficiency. Through in-depth analysis, high-energy consumption, high pollution, and low economic output are the main reasons for low eco-efficiency in the middle and upper reaches of the Yangtze River.
