Due to the fundamental role that sand flies (Diptera Psychodidae) have in Leishmania spp. (Kinetoplastida Trypanosomatidae) transmission, the objective of this study was to evaluate the ecology and spatial distribution of species circulating in the municipality of Seropédica, Rio de Janeiro, Brazil. A total of 17 371 specimens were collected between August 2016 and July 2017, and five species were identified Nyssomyia intermedia (Lutz & Neiva, 1912), Migonemyia migonei (França, 1920), Nyssomyia whitmani (Antunes & Coutinho, 1939), Pintomyia pessoai (Coutinho & Barretto, 1940), and Pintomyia fischeri (Pinto, 1926). In relation to the number of sand flies collected, we observed a significant difference between rural and periurban areas and between the neighbourhoods. We observed that N. intermedia has a relatively greater adaptation to the peridomiciliary environment. This is the first report of Pi. pessoai and Pi. fischeri (only females) in Seropédica as well as the first record of the frequency of phlebotomine species focusing on some ecological indices and the spatial distribution of sand flies. The results presented here aimed to provide an insight into the spatial population dynamics of phlebotomine species in low endemic areas of American Tegumentary Leishmaniasis (ATL) in Seropédica, highlighting important areas for entomological surveillance.Ocular rosacea is a chronic inflammatory condition that can occur in the absence of cutaneous features. The most common ocular features are chronic blepharoconjunctivitis with eyelid margin inflammation and meibomian gland dysfunction. Corneal complications include corneal vascularization, ulceration, scarring and, rarely, perforation. Diagnosis is largely based on clinical signs, although it is often delayed in the absence of cutaneous changes, particularly in children. It can also be associated with systemic disorders such as cardiovascular disease. Management ranges from local therapy to systemic treatment, depending on the severity of the disease. In this review, we describe the epidemiology, pathophysiology, clinical features and treatment of rosacea and ocular rosacea.
Thyroid ultrasound, thyroid scintigraphy and radioactive iodine uptake (RAIU) tests are helpful in the differential diagnosis of thyrotoxicosis and hyperthyroidism. In the setting where these techniques are unavailable or unusable more accessible, and cheaper techniques would be helpful.

We evaluated the capability of free triiodothyronine to free thyroxine ratio (FT3/FT4) to differentiate Graves' Disease (GD) and destructive thyroiditis (DT).

In total, 318 patients with GD and 140 patients with DT were included in the study. Patients were assigned to two groups GD and DT (subacute thyroiditis, painless thyroiditis, postpartum thyroiditis). Serum thyroid-stimulating hormone (TSH), FT4, FT3 levels and FT3/FT4 ratio were evaluated in each group. To obtain the optimal diagnostic cut-off value of FT3, FT4 and FT3/FT4 ratio, ROC curve analysis was performed of all untreated thyrotoxicosis patients.

The optimal FT3/FT4 ratio cut-off value was 2.96, with a sensitivity of 71.7%, the specificity of 88.6%. The area under the ROC curve of the FT3/FT4 ratio regarding the diagnosis of GD was 0.864 (95% CI 0.830-0.894). The cut-off level of FT3 for GD was determined as 6.6pg/mL which had a sensitivity of 72.3% and specificity of 68.6% (AUC=0.771 P<.001). The cut-off level of FT4 for GD was determined as 3.65ng/dl with a sensitivity of 35.5% and specificity of 83.6% (AUC=0.615 P<.001). When a high specificity is needed, FT3/FT4 cut-off value increases to 3.63 with 99.3% specificity and 36.5% sensitivity.

FT3/FT4 ratio helps distinguish GD and DT. In cases of situations where RAIU/scintigraphy and TRAb cannot be studied, the FT3/FT4 ratio is a viable diagnostic tool. Cut-off values with higher specificity can be more helpful in differential diagnosis of GD.

Here, we show that Candida albicans relies on an ER-resident protein, inositol-requiring enzyme 1 (Ire1) for sensing ER stress and activating the UPR. Compromised Ire1 function impacts cellular processes that are dependent on functional secretory homeostasis, as inferred from transcriptional profiling. Concordantly, an Ire1-mutant strain exhibits pleiotropic roles in ER stress response, antifungal tolerance, cell wall regulation and virulence-related traits. Hac1 is the downstream target of C. albicans Ire1 as it initiates the unconventional splicing of the 19 bp intron from HAC1 mRNA during tunicamycin-induced ER stress. Ire1 also activates the UPR in response to perturbations in cell wall integrity and cell membrane homeostasis in a manner that does not necessitate the splicing of HAC1 mRNA. Furthermore, the Ire1-mutant strain is severely defective in hyphal morphogenesis and biofilm formation as well as in establishing a successful infection in vivo. Together, these findings demonstrate that C. albicans Ire1 functions to regulate traits that are essential for virulence and suggest its importance in responding to multiple stresses, thus integrating various stress signals to maintain ER homeostasis.Subtle changes in sleep architecture can accompany and be symptomatic of many diseases or disorders. In order to probe and understand the complex interactions between sleep and health, the ability to model, track, and modulate sleep in preclinical animal models is vital. While various methods have been described for scoring experimental sleep recordings, few are designed to work in real time - a prerequisite for closed-loop sleep manipulation. In the present study, we have developed algorithms and software to classify sleep in real time and validated it on C57BL/6 mice (n = 8). SKL2001 concentration Hidden Markov models of baseline sleep dynamics were fitted using an unsupervised algorithm to electroencephalogram (EEG) and electromyogram (EMG) data for each mouse, and were able to classify sleep in a manner highly concordant with manual scoring (Cohen's Kappa >75%) up to 3 weeks after model construction. This approach produced reasonably accurate estimates of common sleep metrics (proportion, mean duration, and number of bouts). After construction, the models were used to track sleep in real time and accomplish selective rapid eye movement (REM) sleep restriction by triggering non-invasive somatosensory stimulation.
