Conventional treatment strategies for catecholaminergic polymorphic ventricular tachycardia (CPVT) include avoidance of strenuous exercise and competitive sports, drugs such as ß-blockers and flecainide and, cervical sympathectomy. An implantable cardioverter-defibrillator (ICD) has been utilized if the response to these strategies is inadequate; however, ICD use in CPVT patients, in addition to usual complications, is associated with an increased risk of life-threatening electrical storm. Ivabradine is a selective inhibitor of hyperpolarization-activated cyclic nucleotide-gated potassium channel 4 (HCN4) generated funny current (If ) which has been shown to be efficacious in suppression of inappropriate sinus tachycardia, junctional tachycardia, atrial tachycardia and ventricular ectopy in humans. We report an 18 year-boy with a severe CPVT phenotype refractory to flecainide, nadolol and sympathectomy who exhibited suppression of ventricular arrhythmias after initiation of ivabradine. These findings are of importance as ivabradine could be an important add-on therapy in CPVT patients who are drug-refractory or are unable to continue conventional therapies at the recommended doses. All rights reserved.in English, Spanish ANTECEDENTES Los estudios han sugerido que la pancreatectomía distal laparoscópica (laparoscopic dital pancreatectomy, LDP) resulta ventajosa en comparación con la pancreatectomía distal por vía abierta (open distal pancreatectomy, ODP) respecto a la estancia hospitalaria, pérdida sanguínea y recuperación. Solamente existe un estudio aleatorizado que muestra una mejor recuperación funcional después de la LDP en comparación con la ODP. MÉTODOS En un ensayo de superioridad unicéntrico, abierto y de grupos paralelos, los pacientes consecutivos evaluados por el comité multidisciplinario de tumores y a los que se indicó una pancreatectomía distal estándar fueron asignados al azar de forma prospectiva a LDP o ODP. El resultado primario fue la estancia hospitalaria postoperatoria. RESULTADOS De 105 pacientes evaluados, 60 fueron aleatorizados, de los cuales 58 pacientes (24 mujeres; 41%) fueron incluidos y asignados a LDP (n = 29; edad media 68 años) o ODP (n = 29; edad media 63 años) e incluidosrta en comparación con la cirugía abierta, con un menor tiempo para la recuperación funcional y menos hemorragia.Defensive responses to transgressions can have a negative impact on decision-making within government and organizations, on relationships, and even an individual's well-being. Transgressors who are defensive are less likely to acknowledge or appreciate the extent of harm caused, and their responsibility in having contributed to it or in helping to repair it. It is therefore important to understand what situational factors increase or reduce defensiveness and, thus, offer solutions for those trying to foster responsibility-taking by individuals in relationships, organizations, and society. This paper presents two studies exploring what underpins defensive responses in the context of transgressions. In Study 1 (N = 202), participants recalled an interpersonal transgression, and in Study 2 (N = 143), omnivorous participants watched a guilt-eliciting documentary about meat production practices. Both studies demonstrate that defensiveness increases in response to social/moral threat. Further, Study 2 demonstrates that this defensiveness can be reduced by addressing the underlying threat to social/moral identity via value affirmation, encouraging moral engagement, and repair. © 2020 The British Psychological Society.in English, Spanish ANTECEDENTES Actualmente se desconoce si la quimioterapia adyuvante ofrece un beneficio en la supervivencia de los pacientes que se someten a resección de un adenocarcinoma ampular. El objetivo de este estudio fue comparar la supervivencia mediante la concordancia estimada por emparejamiento por puntaje de propensión, entre pacientes con y sin quimioterapia adyuvante después de la resección de un adenocarcinoma ampular. MÉTODOS Se realizó un estudio internacional de cohortes multicéntrico, que incluyó a los pacientes que se sometieron a una duodenopancreatectomía por adenocarcinoma ampular (2006-2017) en 13 centros de seis países. Los puntajes de propensión se usaron para emparejar a los pacientes que recibieron quimioterapia adyuvante con los que no; tanto en la cohorte completa como en dos subgrupos (subtipo pancreaticobiliar / mixto e intestinal).
