To evaluate Medicaid insurance access disparities for urologic care at urgent care centers (UCCs) in the United States.

We conducted a cross-sectional study using a "secret shopper" methodology. We sampled 240 UCCs across 8 states. Using a standardized script, researchers posed as a patient with either Medicaid or commercial insurance in the clinical setting of obstructing nephrolithiasis. The primary study endpoint was whether a patient's insurance (Medicaid vs commercial) was accepted. We assessed factors associated with Medicaid acceptance using logistic regression models adjusted for state-level and facility-level characteristics. Additionally, we calculated triage rates, emergency department referral rates, and the ability of a UCC to refer the patient to a specialist.

Of 240 UCCs contacted, 239 (99.6%) accepted commercial insurance and 159 (66.2%) accepted Medicaid. UCCs in Medicaid expansion states more frequently accepted patients with Medicaid insurance (74.2% vs 58.3%, respectively, P < .01). On multivariable logistic regression analysis, state Medicaid expansion (OR 1.84, 95% CI 1.04-3.26, P=.04) and affiliation with an institution (OR 2.97, 95% CI 1.59-5.57, P < .01) were independently associated with greater odds of accepting Medicaid. Medicaid-insured patients were significantly less likely to be triaged or referred to the emergency department compared to commercial patients.

One major complication of SCD is priapism, defined as an erection of the penis lasting over four hours beyond sexual stimulation or orgasm. SCD priapism is caused by sickled erythrocytes obstructing venous outflow and can lead to permanent erectile dysfunction. This article reviews the pathology, physiology, and management of SCD priapism, including potential novel therapeutic agents.Acute kidney injury (AKI) carries high morbidity and mortality, and effective treatments are lacking. Preclinical models support involvement of micro-RNAs (miRs) in AKI pathogenesis, although effects on the kidney transcriptome are unclear. We previously showed that injection of cord blood endothelial colony forming cell-derived exosomes, enriched in miR-486-5p, prevented ischemic AKI in mice. To further define this, we studied direct effects of miR-486-5p in mice with kidney ischemia-reperfusion injury. RNA-Seq was used to compare the impact of miR-486-5p and exosomes on the transcriptome of proximal tubules and kidney endothelial cells 24 hours after ischemia-reperfusion. In mice with AKI, injection of miR-486-5p mimic increased its levels in proximal tubules and endothelial cells, and improved plasma creatinine, histological injury, neutrophil infiltration, and apoptosis. Additionally, miR-486-5p inhibited expression of its target phosphatase and tensin homolog, and activated protein kinase B. In proximal tubules, miR-486-5p or exosomes reduced expression of genes associated with ischemic injury and the tumor necrosis factor (TNF) pathway, and altered distinct apoptotic genes. In endothelial cells, genes associated with metabolic processes were altered by miR-486-5p or exosomes, although TNF pathway genes were not affected. Thus, our results suggest that miR-486-5p may have therapeutic potential in AKI.In a population of interacting individuals, the environment for interactions often changes due to individuals' behaviors, which in turn drive the evolution of individuals' behaviors. The interplay between the environment and individuals' behaviors has been demonstrated to remarkably influence the evolutionary outcomes. In reality, in highly cognitive species such as social primates and human beings, individuals are often capable of perceiving the environment change and then differentiate their strategies across different environment states. We propose a model of environmental feedback with state-dependent strategies individuals have perceptions of distinct environment states and therefore take distinct sub-strategies under each of them; based on the sub-strategy, individuals then decide their behaviors; their behaviors subsequently modify the environment state. We use the theory of stochastic games and evolutionary dynamics to analyze this idea. We find that when environment changes slower than behaviors, state-dependent strategies (i.e. taking different sub-strategies under different environment states) can outperform state-independent strategies (i.e. taking an identical sub-strategy under all environment states), such as Win-Stay, Lose-Shift, the most leading strategy among state-independent strategies. The intuition is that delayed environmental feedback provides chances for individuals with state-dependent strategies to exploit those with state-independent strategies. Our results hold (1) in both well-mixed and structured populations; (2) when the environment switches between two or more states.
