The objective of this short narrative literature review is to highlight the different difficulties encountered by medical doctor in the daily use of EMR. We show that these are not simple transitional phenomena related to a "resistance to change", but rather the fact of a deeper and unfinished transformation. Beyond the "perception of misfit with work processes" or the threat of a loss of autonomy, we propose to analyze this so-called "resistance" in relation to the formalization of medical work induced by EMR. Our question concerns the compatibility of the multiple objectives of EMR, the potential influence of computerization on the steps of entering and consulting medical information, the impact on the clinical reasoning, the reality of assistance to medical "performance". The question is not so much what EMRs do less well than the paper record, but to provide insights into how tomorrow's EMRs will do better than today's.Objective To identify health assets and describe quality of life for people diagnosed with severe mental disorder through their own testimonies; to understand the necessities and barriers that affect their quality of life; and offer improvement recommendations. Method A qualitative, descriptive analysis based on the information obtained through semi-structured interviews and asset mapping. Results Assets are similar to those found in undiagnosed people, while needs and barriers differ in both groups. The main assets include social relationships, socioeconomic assets, personal development, leisure and free time assets, places, art, daily activities, which usually take place within the mental health network, sense of belonging, and autonomy. To improve their health and life quality, they need access to employment and studies, more and better social relationships, and more control over their lives. Side effects of medication and socio-economic exclusion, and the lack of autonomy derived from it are the main existing barriers, which have a negative effect in self-esteem and social interactions. find more An important measure to improve health and quality of life is the increase of economic and social assets for a more autonomous personal development. Conclusions This community action for health care has served to reveal the similarities between health and quality of life assets for severe mental disorder diagnosed and undiagnosed people, as well as the differences in the needs and difficulties to achieve them.Objective To assess the diagnostic capability of PET/CT with [18F]F-Fluoromethylcholine in prostate cancer (PC) with biochemical recurrence and its therapeutic impact. Material and methods We included 108 patients, diagnosed with PC with biochemical criteria for recurrence. A PET/CT Choline scan was performed by dynamic pelvic and whole body study at 60min post-tracer injection. The relationship between the positive studies and the PSA value was analysed by classifying patients into three groups (2ng/ml showed a higher percentage of disease detection than patients with a lower PSA level, with significant differences (p less then 0.0001). PET/CT [18F]F-Choline was able to detect local disease, not previously known from MRI, in 29.41% of patients. PET/CT Choline had an impact on therapeutic management in 67 of 108 patients (62%). Conclusions PET/CT with [18F]F-Fluoromethylcholine is a useful tool in the detection of locoregional and disseminated disease of PC treated with suspicion of recurrence, providing a change in therapeutic management in 62% of patients.Background The changing demographics of our society will lead to an increasing number of patients presenting for orthopedic surgery with increasing comorbidity. We investigated the association between comorbidity and both the risks (complications) and benefits (improved function) of total hip arthroplasty (THA) for primary hip osteoarthritis, whilst controlling for potential confounders including age. Methods One thousand five hundred and eighty-four patients (67.1 ± 10.6 years; 54% men) in our tertiary care orthopedic hospital completed the Oxford Hip Score before and 12 months after THA. Comorbidity was assessed using the American Society of Anesthesiologists (ASA) grade and Charlson Comorbidity Index (CCI). Details regarding perioperative complications (hospital stay plus 18 days after discharge; mean 27 ± 3 days) were extracted from the clinic information system and graded for severity. Results For ASA1, 2, and ≥3, respectively, there were 3.1%, 3.0%, and 6.6% surgical/orthopedic complications; 3.7%, 12.5%, and 27.4% general medical complications; and 6.7%, 14.5%, and 29.8% complications of either type. ASA was associated with complication severity (P less then .001). In multiple regression, increasing ASA grade (OR 1.74; 95% CI, 1.33-2.29) and age (OR 1.06; 95% CI, 1.05-1.08), both showed an independent association with increased risk of a complication; CCI explained no further significant variance. CCI, but not age, was associated with the 12-month Oxford Hip Score (beta coefficient, -0.742; 95% CI, -1.130 to -0.355; P = .002) while ASA grade explained no further variance. Conclusion Greater comorbidity was associated with increased odds of a complication and (independently) slightly worse patient-rated outcome 12 months after THA. The aim of this study is to examine the effect of preoperative tramadol use on postoperative outcomes in patients undergoing elective THA. Methods A total of 5304 patients who underwent primary THA for degenerative hip pathology from 2008 to 2014 were identified using the Humana Claims Database.
