This paper examines caregiving for sick older family members in the context of socio-economic transformations in rural China, combining empirical investigation with normative inquiry. The empirical part of this paper is based on a case study, taken from fieldwork in a rural Chinese hospital, of a son who took care of his hospitalized mother. This empirical study highlighted family members' weiqu (sense of unfairness)-a mental status from experiencing mistreatment and oppression in family care, yet with constrained power to explicitly protest or make care-related choices. Underpinning people's weiqu and constrained choice, as informed by the conception of structural injustice, is the impact of unjust social structures, organized by unfavourable norms, discriminatory social policies and institutions targeting rural populations. By restraining individual choices and capacities in supporting health care for aging populations, these unjust structures create additional difficulties for and discriminations against rural families and their older members. Some policy recommendations are proposed to mitigate structural injustice so as to empower families and promote care for older people in rural settings.Collagen-based sauce braised meat products with a unique flavor, chewiness, and springiness are very popular in China. In order to understand how the structure, thermal properties, and chemical components of collagen-based tissues influence the texture characteristics of collagen-based meat products during stewing, three kinds of tendon tissues (chicken feet tendon [CFT], sheep tendon [ST], and bovine tendon [BT]) and three kinds of skin tissues (chicken feet skin [CFS], sheep skin [SS], and porcine skin [PS]) were selected to heat for 45 min at 95 °C, and texture, thermal properties, micromorphology, ultrastructure, glycosaminoglycan (GAG) content, and collagen content were evaluated among the six kinds of collagen-based tissues. Results showed that BT had the highest hardness, chewiness values, total collagen content, and the thickest fibers diameters, followed by ST, CFT, PS, and SS, whereas CFS had lowest values among six kinds of tissues. The thermal stability and GAG levels in tendon samples were higher than those in skin samples. Thicker diameters, higher thermal stability, GAG, and total collagen content could be responsible for the higher hardness and chewiness of tendon samples compared with skin tissues. These findings provide the understanding of relationship between meat quality of collagen-based meat products and their morphological and chemical properties. PRACTICAL APPLICATION This research confirms that the difference in texture of different kinds of collagen-based tissues was related to the difference in structure and chemical properties. The result can be used to optimize the processing conditions of collagen-based sauce braised meat products in food industry.Background To evaluate the outcomes of patients with severe bronchiolitis, who received preemptive high flow nasal cannula (HFNC) treatment according to the authors' protocol and to identify potential baseline characteristics that might predict patients who will not benefit from HFNC. Methods This was a retrospective chart review of patients with severe bronchiolitis, who received preemptive HFNC treatment according to the authors' protocol and who were admitted to the pediatric emergency department (PED) between January 1, 2015, and December 31, 2016. Results 84 total patients were enrolled over the two-year period. 23 patients (27.3%) failed HFNC. Of these, four responded to non-invasive mechanical ventilation (NIV) and 19 required subsequent invasive ventilation. According to logistic regression analysis, existence of a chronic condition, significant tachycardia, existence of dehydration, and a venous pH less then 7.30 at admission were found to be predictors of HFNC failure. There were no cases of pneumothorax or any other reported adverse effects related to HFNC therapy. Conclusions Preemptive HFNC treatment, complying with a preestablished protocol, might be a safe way to support patients with severe bronchiolitis in high-volume, resource limited PEDs. Mass Spec Rev.Objective We aimed to describe the extent of neurodevelopmental impairments and identify the genetic etiologies in a large cohort of patients with epilepsy with myoclonic atonic seizures (MAE). Methods We deeply phenotyped MAE patients for epilepsy features, intellectual disability, autism spectrum disorder, and attention-deficit/hyperactivity disorder using standardized neuropsychological instruments. We performed exome analysis (whole exome sequencing) filtered on epilepsy and neuropsychiatric gene sets to identify genetic etiologies. Results We analyzed 101 patients with MAE (70% male). The median age of seizure onset was 34 months (range = 6-72 months). The main seizure types were myoclonic atonic or atonic in 100%, generalized tonic-clonic in 72%, myoclonic in 69%, absence in 60%, and tonic seizures in 19% of patients. We observed intellectual disability in 62% of patients, with extremely low adaptive behavioral scores in 69%. In addition, 24% exhibited symptoms of autism and 37% exhibited attention-deficit/hyperactivity symptoms.
