To gain insights into the recovery process, we use this dataset to build models that predict a patient's next day physical activity duration and next night sleep duration. Using our data-driven approach and the combined dataset, we obtained a normalized root mean square error prediction of 9.11% for daytime physical activity and 11.18% for nighttime sleep duration. Our sleep result is comparable to the accuracy we achieved using the manufacturer's sleep labels (12.26%). Our device-independent predictions are suitable for both point-of-care and remote monitoring applications to provide information to clinicians for customizing therapy services and potentially decreasing recovery time.
One of the main reasons for risky sexual behavior observed in HIV serodiscordant couples despite the knowledge of the partner's status and counselling is childbearing. In Cameroon, there are few reports on HIV serodiscordant couples. This paper describes the influence of HIV on sexual relationships and decision to procreate.
This cross-sectional study was conducted in five health centers. Self-administered questionnaire was used to collect social and demographic information, while semi-structured in-depth individual and couple interviews were used to explore sexual relationships and decisions about fatherhood/motherhood. Blood samples were collected from the couples and tested for HIV to confirm serodiscordance. The data were analyzed using the GraphPad Prism Version 6 software.
A total of 53/192 (27.6%) HIV serodiscordant couples participated in the study, and 18/74 (24.32%) HIV positive seroconcordant couples and 32/80 HIV negative seroconcordant couples were used as controls. The majority of HIV-posiaby.
Sexuality of serodiscordant couples as well as of HIV positive seroconcordant couples was affected by the presence of HIV/AIDS. The desire to procreate may lead couples to adopt risky sexual behaviors. It is important to define specific guidelines for serodiscordant couples in order to improve their sexual life and consequently enable them to procreate with minimal risk of infecting their partner and or to transmit the virus to their baby. This ongoing column is dedicated to providing information to our readers on managing legal risks associated with medical practice. We invite questions from our readers. The answers are provided by PRMS, Inc. (www.prms.com), a manager of medical professional liability insurance programs with services that include risk management consultation, education and on-site risk management audits, and other resources offered to health care providers to help improve patient outcomes and reduce professional liability risk. The answers published in this column represent those of only one risk management consulting company. Other risk management consulting companies or insurance carriers might provide different advice, and readers should take this into consideration. The information in this column does not constitute legal advice. For legal advice, contact your personal attorney. Note The information and recommendations in this article are applicable to physicians and other health care professionals so "clinician" is used to indicate all treatment team members. Objective This pilot study explored the effects of therapeutic immersive virtual reality (VR) on pain in upper limb complex regional pain syndrome (CRPS). click here While acute pain relief with VR has been studied in multiple populations, there is little data on the use of this modality in treating chronic pain, especially CRPS. Participants Volunteer participants were recruited from outpatient rehabilitation services. Inclusion criteria required the diagnosis of CRPS in at least one upper limb and the ability to communicate in English to receive instructions from study personnel. A total of eight participants were recruited, with six fully completing the study. Interventions An immersive virtual three-dimensional interactive kitchen environment was designed that allowed visualization of and object manipulation with virtual hands. Participants performed tasks representative of daily activities, as well as guided visualization exercises for a total of 10 sessions. Main Outcome Measure Preand post-session pain scale measurements (Short Form McGill Pain Questionnaire, Visual Analog Scale, and Wong-Baker FACES) and subjective feedback were collected with each session. Results Four of the six participants that completed the study reported subjective improvement of their pain and daily function. However, objective pain scales had limited correlation to reported subjective relief. Conclusions Immersive virtual reality might provide subjective analgesia and functional improvement in select patients with upper limb complex regional pain syndrome, but objective data is lacking.Lithium intoxication might be lethal particularly in elderly persons who have multiple drugs. We present an aged bipolar patient with lithium intoxication probably caused by the interaction between fluvoxamine and azilsartan. Fluvoxamine inhibits many cytochrome P450 enzyme (CYP) including CYP2C9, which metabolizes azilsartan. Thus, azilsartan could cause more negative effects for sodium reabsorption at renal tubules when co-administered with fluvoxamine, resulted in a decrease in lithium excretion. We present here the case of a 27-year-old man with schizoaffective disorder for roughly eight years who, seven years prior to presentation at our institution, suffered a severe traumatic brain injury (TBI) in a car accident. His course since that time has been marked by paranoid and guilty delusions, Cotard delusion (CD), which is the belief he is dead or does not exist, and scam susceptibility leading to the loss of nearly $200,000. He was hospitalized at our institution after his uncle called emergency medical services, concerned about the patient's increasing disorganization and worsening delusions in the setting of medication nonadherence. In our inpatient unit, clozapine was titrated to its highest tolerable dose and, while reality testing appeared and his CD resolved, he remained acutely vulnerable to scams even while hospitalized. We review the existing literature on scam vulnerability, which heretofore has focused primarily on the elderly, and also TBI rehabilitation strategies in an attempt to better understand the underpinnings of this patient's scam susceptibility and to construct a multidisciplinary approach to lessening his susceptibility to financial exploitation in the future.
