Congenital idiopathic bilateral vocal fold dysfunction (BVFD) is an uncommon cause of neonatal stridor and respiratory distress postnatally. Approximately 50% of affected neonates or infants will historically require tracheostomy for this condition. Timing and candidacy for tracheostomy in BVFD patients is often subjective and poorly understood. Polysomnography (PSG) and video swallow studies (VSS) may be helpful in the management of patients with BVFD prior to tracheostomy by quantifying their degree of upper airway obstruction during sleep and feeding dysfunction while awake.

We performed a single-institution retrospective case series of BVFD patients from 2000 to 2018 who had postnatal PSGs performed prior to tracheostomy. Demographics, gestational age, and VSS results prior to PSG were recorded for all patients. Findings from PSGs included non-REM AHI, REM AHI, oxygen nadir, % total sleep time (TST) O2<90%, peak end-tidal (ET) CO2, % TST ETCO2 >52 torr. Rates of post-PSG tracheostomy, gastrostomng dysfunction was found in approximately 50% of patients. The presence of feeding dysfunction, severe OSA, or any PSG abnormality was not individually associated with the subsequent performance of a tracheostomy in our patients. PSG is likely useful in supporting but not supplanting one's clinical decision-making in the management of patients with congenital idiopathic BVFD.

Increased overcrowding in the emergency department is a potential threat to the quality and safety of patient care. Innovative ways are needed to explore overcrowding, the variables affecting patient flow and interventions necessary for future flow improvement.

The aim of this review is to explore nurses' role(s) and their contribution to maintaining patient flow in acute hospitals through emergency departments.

A systematic review ofmixed studies (qualitative, quantitative and mixed-method) using narrative synthesis was undertaken. Five major databases-PubMed, CINHAL, BNI, ASSIA and SCOPUS-were searched to identify appropriate primary and secondary studies. Selected studies were critically appraised with a modified CASP tool. Data extraction and analysis was undertaken using narrative synthesis.

In total, 34 articles (31 primary studies and three systematic reviews) met the inclusion criteria. Field studies linking MPs and PAEs in seawater have not yet been reported. We investigate for the first time the correlation between MPs contamination and the presence of PAEs in the surface seawater of Jiaozhou Bay (JZB), a semi-enclosed metropolitan bay in northern China heavily impacted by human activity. selleck The abundance of MPs, dominated by polyethylene and polyethylene terephthalate mostly smaller than 2 mm, ranged between 24.44 items/m3 and 180.23 items/m3, with the majority being black and transparent fibers and fragments. Concentrations of PAEs varied from 129.96 ng/L to 921.22 ng/L. Relatively higher abundances of MPs and higher concentrations of PAEs were generally found in areas near riverine inputs and sewage treatment plants. There was a strong correlation between PAEs concentration and MPs abundance, suggesting that they are closely linked. In a risk assessment combining PAEs and MPs, the risk quotients (RQs) indicated that the ecological risk of di-n-butyl phthalate in JZB was relatively high (0.046 less then RQ less then 0.516); the risk of the other PAEs were low. The overall ecological hazard index (HI) of PAEs was low to medium (0.098 less then HI less then 0.897). The risk of MPs pollution in JZB, as indicated by Pollution Load Index (PLI), was moderate (PLIJZB = 11.76), and mainly due to polyvinyl chloride.Cardamine violifolia (Brassicaceae) is a novel selenium(Se) hyperaccumulation plant with rich nutrients, and serves as a good source of special vegetables in Enshi, China. The present study aimed to investigate the effects of the application of selenate, selenite, and Se yeast (50-800 mg/L) on the growth, Se accumulation, nutrient uptake, and antioxidant response of C. violifolia. The results showed that the Se accumulation efficiency was selenate > selenite > Se yeast, the maximum Se concentration could achieve over 7000 mg/kg, and about 90% was organic Se. The major Se speciation found was mainly SeCys2 and the proportion of various Se species were affected by the Se forms and concentrations. Besides, the plant growth, nutrition quality indexes, element uptakes, and antioxidant responses indicated that 200 mg/L selenate was optimum for C. violifolia to accumulate Se without much impacts, while to obtain more proportion of organic Se, 200 mg/L selenite might be a better choice.
