Introduction Autism spectrum disorder (ASD) comprises difficulties in social communication and restrictive and repetitive behaviors. Despite an increased global prevalence, little remains known about early detection and diagnosis of autism in Mainland China. Our aim was to conduct a pilot investigation of the implementation of an Australian tool, Social Attention and Communication Surveillance (SACS), in Tianjin, China (SACS-C) by trained professionals to identify autism early compared to the Checklist for Autism in Toddlers-23 (CHAT-23) completed by parents and professionals. Materials and Methods A total of 10,514 children were monitored across 61 Community Health Service Centres in six Tianjin districts on the SACS-C at 12, 18, and 24 months of age following a half-day training of 225 child health practitioners. Children deemed at "high likelihood" for autism on either the SACS, CHAT-23, or both, were referred for developmental assessments at the Tianjin Women and Children's Health Centre (TWCHC). Results rs to monitor infants and toddlers for the early signs of autism, and determined the effectiveness of their use of SACS-C which had a better balance between accuracy and sensitivity in detecting autism in contrast to the CHAT-23 which missed the majority of children with autism (72.7%) vs. the SACS-C (46.7%). Given the emphasis on identifying as many children with autism as possible in Mainland China, SACS-C was identified as the tool of choice by the TWCHC. However, more work is needed to improve the psychometric properties in using the SACS-C in Mainland China so that it is comparable to its use in Australia.Introduction Patients with atrial fibrillation (AF) can experience ischemic stroke despite adequate anticoagulant therapy. The secondary prevention strategy of these so-called "resistant strokes" is empirical. Since about 90% of patients with ischemic stroke due to atrial fibrillation have thrombus in left atrial appendage (LAA) we sought to explore the possibility that resistant stroke could have a LAA morphology resistant to anticoagulants. Case Report A 77 years old man affected by AF experienced two cardioembolic ischemic stroke while on anticoagulants. The study of LAA showed a windsock-like morphology in the proximal part while distally the LAA presented a cauliflower morphology with a large amount of pectinate muscles and blood stagnation. The precise characteristics of LAA were properly understood integrating images obtained by cardiac CT, transesophageal echocardiography, and selective angiography. A high risky LAA for thrombus formation was diagnosed and its occlusion (LAAO) as an add-on therapy to anticoagulants was proposed and performed. Six month follow-up was uneventfully. Conclusion The systematic study of LAA in patients with resistant-stroke could help to identify LAA malignant morphology. The efficacy on stroke recurrence of the combined therapy (anticoagulants plus LAAO) is worthy to be tested in randomized trials.To maintain the balance, the postural system needs to integrate the three main sensorial systems visual, vestibular, and somatosensory to keep postural control within the limits of stabilization. Damage of one of these systems, in this case, the vision, will have a great disturbance on the postural control influencing the behavior of the balance, resulting in falls. The aim of this study protocol for a randomized, controlled clinical trial is to analyze the effects of transcranial direct current stimulation (tDCS) combined with proprioceptive exercises on postural control in individuals with congenital and acquired blindness. In this randomized, controlled, double-blind, clinical trial, male, and female individuals with blindness between 18 and 55 years of age will participate in this study divided into three phases 1-Determine differences in postural control and gait between individuals with congenital and acquired blindness with and without the use of a guide stick when wearing shoes and when barefoot; 2-A pilot study to analyze the effects a bilateral cerebellar anodal tDCS on postural on postural control and gait; and 3-A treatment protocol will be conducted in which the participants will be allocated to four groups G1-active tDCS + dynamic proprioceptive exercises; G2-sham tDCS + dynamic proprioceptive exercises; G3-active tDCS + static proprioceptive exercises; and G4-sham tDCS + static proprioceptive exercises. Evaluations will involve a camera system for three-dimensional gait analysis, a force plate, and electromyography. Dynamic stability will be determined using the Timed Up and Go test and static stability will be analyzed with the aid of the force plate. The viability of this study will allow the determination of differences in postural control between individuals with congenital and acquired blindness, the analysis of the effect of tDCS on postural control, and the establishment of a rehabilitation protocol.Introduction Novel coronavirus disease 2019 (COVID-19) infection caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is spreading worldwide. We hypothesized that patient flow in epilepsy care would change as a result of the COVID-19 pandemic. The purpose of this study was to compare the number of patients who visited our epilepsy center before and during the first peak of the pandemic. Methods We recorded the number of patients with epilepsy referred from general physicians (GPs) to our hospital (GP-H group), the number of patients who visited our hospital on a regular basis (R group), and the number of patients referred from our hospital to GPs (H-GP group) between July 2019 and June 2020. Results A total of 1,839 epilepsy patients made 4,197 visits to our hospital 979 males and 860 females (age range, 0-94 years; mean age, 37.6 years; median age, 34 years). There were 433 patients in the GP-H group (247 before the pandemic, 186 during the first peak of the pandemic; p = 0.008). In the R group, 1,406 patients made 3,764 visits (1,992 visits before the pandemic, 1,772 during the first peak of the pandemic). selleck chemical In the H-GP group, 135 patients were referred to GPs (47 patients before the pandemic, 88 patients during the first peak of the pandemic; p = 0.023). Conclusion Patient flow in the epilepsy care network changed as a result of the COVID-19 pandemic. These changes might present an opportunity to strengthen local interdisciplinary epilepsy care.
