Estimating the prevalence of unintended pregnancies and associated socio-demographic and health factors is the goal of this study conducted on a national cohort of young Australian women.
Further examination of the 2013-2015 three-wave data set from the Australian Longitudinal Study on Women's Health focused on the new young cohort. By employing a three-pronged approach of internet engagement, traditional media exposure, and peer-to-peer networking, women born between 1989 and 1995 were recruited. A web-based health and healthcare use survey was undertaken by 17,010 respondents in 2013; this acted as a baseline, and annual follow-ups were conducted. The analysis utilizes data from women who have had vaginal intercourse, specifically from waves 2 (n=9726/11344) and 3 (n=6848/8961). We investigated factors associated with past and recent unplanned pregnancies through the use of multivariable regression analyses.
The second wave of data revealed a lifetime prevalence of unintended pregnancies of 126% among women aged 19 to 24, increasing to an alarming 810% among women who had previously given birth. A woman's place of residence affected the results of pregnancies for those who had an unintended pregnancy in the past. Comparing women with polycystic ovary syndrome (PCOS) and healthy women, what are the unique aspects of their vaginal microbiome (VMB)?
At a solitary academic center, affiliated with a university, a cross-sectional study was executed. The study's cohort consisted of 1446 individuals, specifically 713 with polycystic ovary syndrome (PCOS) and 733 healthy control participants. 16S rRNA gene sequencing was utilized in the examination of the vaginal swabs. The study investigated the difference in microbiome diversity and composition between PCOS and control groups. Microbial interaction networks were examined, and functional prediction was also investigated.
The PCOS group displayed a greater alpha diversity compared to the control group, a finding supported by Shannon (P=0.003) and Simpson (P=0.002) diversity analyses. The PCOS group further showed a substantially higher degree of intra-group variability (P < 2.2E-16). Within the PCOS group, there was a notable decrease in the genus Lactobacillus (851% vs. 893%, false discovery rate [FDR]=0.002), juxtaposed with increases in Gardnerella vaginalis and Ureaplasma (51% vs. 33%, FDR=0.0006; 12% vs. 6%, FDR=0.0002, respectively). Lactobacillus acidophilus, Prevotella buccalis, and G. vaginalis were determined to be the primary distinguishing species. Anti-Mullerian hormone (AMH) serum levels and triglyceride levels were positively correlated with L. acidophilus, as evidenced by a statistically significant association (P=2.01E-05 and P=0.0004, respectively). The presence of P. buccalis was negatively associated with serum levels of AMH and testosterone, as indicated by P-values of 0.0002 and 0.0003 respectively. There was a positive correlation between G. vaginalis and serum levels of AMH, oestradiol, and progesterone, as indicated by the following p-values: 0.0004, 0.0005, and 0.003, respectively. The PCOS group's VMB interaction network data suggested that Lactobacillus crispus, Prevotella timonensis, and P. buccalis could be crucial drivers. The analysis uncovered 55 predicted genes with differing abundances in PCOS compared to control samples, according to FDRs less than 0.025.
A higher diversity of vaginal microbiome and enhanced heterogeneity were observed in the PCOS cohort. The PCOS group displayed a decrease in the percentage of Lactobacillus, accompanied by an increase in the proportions of both Gardnerella and Ureaplasma. Further investigation into the correlation between PCOS and VMB is justified by these results.
The PCOS group demonstrated a significantly greater diversity within their vaginal microbiome, characterized by a marked increase in heterogeneity. For the PCOS group, Lactobacillus levels decreased, whereas the proportions of Gardnerella and Ureaplasma displayed a significant rise. These findings necessitate further investigation to confirm the link between PCOS and VMB.
What is the association between self-reported psychological stress and ovarian reserve parameters in the subfertile female population?
An observational study of the Environment and Reproductive Health study included 520 women from Massachusetts General Hospital, seeking fertility care and enrolled between 2005 and 2019. Women, aiming to measure psychological stress, finalized the short, validated version of the PSS4. Markers of ovarian reserve included antral follicle counts (AFC), circulating serum levels of follicle-stimulating hormone (FSH) on day 3, and anti-Müllerian hormone (AMH), measured in a subset of the participants (n=185).
Increased PSS4 scores showed an inverse association with levels of AFC and serum AMH. Infertility diagnoses were analyzed, factoring in age, BMI, race, smoking habits, education level, physical activity, and the specific type of infertility. Women categorized in the second and third stress tertiles demonstrated lower AFC (133, 95% confidence interval [CI] 127-138; and 135, 95% CI 130-141) in comparison to those in the lowest stress tertile (143, 95% CI 138-149), with statistically significant differences observed (both p < 0.005). Women in the middle PSS4 score groups (second and third tertiles) had lower average AMH serum levels compared to those in the lowest PSS4 score group (first tertile), specifically 299 (95% confidence interval 224 to 374) and 299 (95% confidence interval 222 to 376) for the second and third groups respectively, while the lowest group had a mean of 394 (95% confidence interval 323 to 464). The associations observed varied depending on socioeconomic factors, particularly amongst women who were younger, belonged to minority races, held a college degree, or had annual household incomes below $100,000.
