Lipid-rich necrotic core (LRNC), a high-risk coronary plaque feature assessed by coronary computed tomography angiography, is associated with increased risk of future cardiovascular events in patients with subclinical, nonobstructive coronary artery disease. Psoriasis is a chronic inflammatory condition that is associated with increased prevalence of high-risk coronary plaque and risk of cardiovascular events. This study characterized LRNC in psoriasis and how LRNC modulates in response to biologic therapy.

Consecutive biologic naïve psoriasis patients (n=209) underwent coronary computed tomography angiography at baseline and 1-year to assess changes in LRNC using a novel histopathologically validated software (vascuCAP Elucid Bioimaging, Boston, MA) before and after biologic therapy over 1 year.

Study participants were middle-aged, predominantly male with similar cardiometabolic and psoriasis status between treatment groups. This study examined the behavioral consequences of first- and second-level agenda setting by testing the effects of exposure to flu-related news coverage and different frames on flu vaccine uptake, while controlling for confounding sociodemographic factors, utilizing computational methods. Findings suggested that the media effects on children and adult influenza vaccine uptake differed. The frame of influenza caused deaths/illnesses stories was the only media frame that positively predicted flu vaccine uptake among children. Four frames, including influenza caused deaths/illnesses stories, flu prevention tips, things to know about flu vaccine, and public accountability positively predicted flu vaccine uptake among adults. Exposure rate negatively predicted flu vaccine uptake among adults. A few sociodemographic factors impacted flu vaccine uptake. Results also revealed the most and least exposed frames and state exposure rates to flu-related news stories.
Helicopter transport (HT) is an efficient, but costly, means for injured patients to receive life-saving, definitive trauma care. Identifying the characteristics of inappropriate HT presents an opportunity to improve the utilization of this finite medical resource.

Trauma registry records of all HT for a 3-year period (2016-2018) to an urban Level I trauma center were reviewed. HT was defined as inappropriate for patients who were discharged home from the emergency department or had a hospital length of stay <1 day, and who were discharged alive. Chi-square analysis and Student's

value of less than .15 were subject to binary logistic regression analysis. A
value ≤.05 was considered significant.

There were 713 patients who received HT during the study period. One-hundred and forty-eight (20.8%) patients met the criteria as an inappropriate HT. In univariate analysis, Glasgow Coma Scale >8, Shock Index <0.9, and fall mechanism were found to be significantly associated with inappropriate HT. Age >55 was found to be associated with an appropriate HT. The average Injury Severity Score of the inappropriate HT group was 3.86 (±3.85) compared with 16.80 (±11.23) (
= .0001, Student's
-test).

Our findings suggest that there are evidence-based predictors of patients receiving inappropriate HT. Triage of HT using these predictors has the potential to decrease unnecessary deployments and reduce health care costs.

As of August 2020, i.e. 6 months after the first COVID-19 publication was released, more than 40000 articles have been published in Medline, the 1552 being indexed as case reports (COVID Group). During the same period, a Medline search for case reports about infectious diseases excluding COVID-19 (ID Group), identified 496 publications. The two groups were compared in terms of authorship. COVID-19 case reports had a mean of 6.1 authors per manuscript (median 5, SD 3.8), compared to 6.2 (median 5, SD 3.3) in the other group (p = .43). The COVID-19 group had 51 cases (3.3%) authored from 15 or more authors, compared to 1.4% (7 cases) in the ID group (p = .03). Fourteen percent of all COVID-19 case reports have 10 or more authors and 13 manuscripts (1%) have been authored by 20 or more persons. We conclude that COVID-19 case reports reflect the modern trend of authorship proliferation with two-page case reports being authored from >15 scientists.
Hip fractures are common and disabling injuries, usually managed surgically. The most common type outside the joint capsule are trochanteric fractures, usually fixed with either sliding hip screw or intramedullary nail. Data are available in the National Hip Fracture Database (NHFD) on early failure and other major complications, but late or subtler complications may escape recording. This study sought to quantify such problems after fixation performed at 3different sites and identify their predictors.

Patients with a trochanteric fracture treated at 1 of 3 sites were identified from the NHFD over a 3-year period. Any with further, related episodes of care were identified, and reasons recorded, then age- and sex-matched with those with no such episodes. Data was collected on Arbeitsgemeinschaft für Osteosynthesefragen classification, tip-apex distance, American Society of Anesthesiologists (ASA) grade, Abbreviated Mental Test Score and pre-injury mobility. The cohorts were compared, and a binomial logistic regression model used to identify predictors of problems.
