This is the first report of wild boars clinically infected with FMDV in the Middle East. © 2020 Blackwell Verlag GmbH.AIMS/INTRODUCTION Tofogliflozin is a potent and highly selective sodium glucose cotransporter 2 inhibitor and is currently used to treat patients with type 2 diabetes mellitus (T2DM). We designed a 3-year study of tofogliflozin in patients with T2DM to evaluate the safety and effectiveness in routine clinical practice. The 3-month and 12-month interim analysis demonstrated tofogliflozin was well-tolerated, safe, and clinically effective. Here we report the results of the 24-month interim analysis. MATERIALS AND METHODS This is a 3-year prospective, observational, and multicenter post-marketing study (Japanese Study of tofogliflozin with T2DM Patients/Long Term, (J-STEP/LT). RESULTS Of the 6897 patients enrolled, 6712 and 6461 patients were analyzed for safety and effectiveness of tofogliflozin, respectively. During the 24 months observation period, the incidence rates of adverse drug reactions (ADRs) and serious ADRs were 11.25% and 1.21%, respectively. As to ADRs of special interest, the incidence rates of hypoglycaemia, polyuria/pollakiuria, volume depletion-related events, urinary tract infections, genital infection were 0.83%, 1.28%, 1.46%, 1.18%, 1.62%, respectively. Renal disorders, and cardiovascular and cerebrovascular disorders occurred in 0.63% and 0.76% of the patients, respectively. Glycated hemoglobin A1c (HbA1c) and body weight decreased significantly by -0.70% (p less then 0.0001) and -2.95 kg (p less then 0.0001), respectively from baseline to week 104 (last observation carried forward). CONCLUSIONS Significant safety concerns have not been observed and clinical benefit including a long-term reduction in HbA1c over 104 weeks (24 months) observation period with weight loss was suggested in this 24-month interim analysis of the 3-year J-STEP/LT study in routine clinical practice. Abexinostat EBL causes great economic losses, so a fast and reliable diagnostic method is critical for understanding the status of BLV. This will allow us to control BLV infections efficiently and mitigate economic losses. In this study, we established a direct diagnostic test for BLV using dried blood-spotted filter papers without sample pre-treatment. The study was based on 159 clinical blood specimens collected in EDTA from one farm in Kyushu, Japan. The blood-spotted filter papers were used as the template for direct filter PCR. When an ELISA was used as the diagnostic gold standard, the sensitivity and specificity of the direct filter PCR were 90.1% and 97.5%, respectively. The kappa value for the direct filter PCR and real-time PCR methods was 0.97. The dried blood samples spotted onto filter papers were stable for at least 10 days at room temperature, even when the samples were from cattle with a low BLV proviral load. Direct filter PCR is a rapid, easy, reliable and cost-effective diagnostic test that directly detects the BLV proviral genome in clinical blood specimens without DNA extraction. Moreover, it simplifies the collection, transportation and storage procedures for clinical blood specimens. © 2020 Blackwell Verlag GmbH.BACKGROUND Ultrasound elastography, is a pioneer sonographic modality that is conducted during endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in order to increase the accuracy of sampling location. The current study aims to evaluate the usefulness of elastography during EBUS-TBNA in a population with a high prevalence of anthracosis. METHODS This prospective single-blinded study was performed on 69 lymph nodes (LNs) of patients with mediastinal lymphadenopathy undergoing EBUS-TBNA and EBUS-elastography from October 2017 to July 2018. The stiffness level of the tissue was translated into a color to demonstrate the hardness of tissue. Blue and total areas of each section were measured to calculate the hardness of each LN. RESULTS Sixty-nine LNs were evaluated by elastography. Twenty percent of LNs were malignant. There was a statistical difference between malignant and non-malignant nodes based on color dominancy (P = 0.032). However, with the exclusion of anthracosis nodes from the analysis, the difference was more significant (P less then 0.001). Moreover, when the blue dominancy was used as the predictor of malignancy or anthracosis, the results showed a significant correlation (P less then 001). CONCLUSION The usefulness of elastography in selecting the hardest area of tissue that is appropriate for diagnosing diseases has been proven previously. Since in countries with a high prevalence of anthracosis, blue color achieved using elastography predicts either malignancy or anthracosis so, cases with blue dominancy of LNs in elastography and the white color in the EBUS-TBNA indicate anthracosis-caused calcification should be reconsidered. © 2020 John Wiley & Sons Ltd.A three-component Pd-catalyzed coupling of ynamides, aryl diazonium salts, and aryl boronic acids for the synthesis of novel triaryl-substituted enamides is described. This transformation represents the first example of an umpolung regioselective unsymmetrical syn-1,2-diarylation/aryl-olefination of ynamides. The aryl moieties of the diazonium salt (electrophile) and boronic acid (nucleophile) are explicitly incorporated in the electrophilic α- and nucleophilic β-position, respectively, of the ynamide, resulting in a single isomer of the N-bearing tetrasubstituted olefin. The scope is broad (68 examples), showing excellent functional-group tolerance. DFT calculations substantiate the rationale of the mechanistic cycle and the regioselectivity.
