Health professionals should expect to find less excursion of lateral trunk bending in step-down tasks compared to level walking and consider that GMed has different roles in these two tasks.Gait analysis and physical clinical measures are usually performed in children with cerebral palsy to help the surgeons make therapeutic decision. However, the level of physical activity in daily life is not systematically assessed. The aim of this cross sectional study was to examine the correlations between three-dimensional gait analysis kinematic and spatiotemporal parameters, clinical measures and physical activity. find more Participants were 30 children with cerebral palsy (10-18 y), with GMFCS I-III. Daily physical activity was measured with an Actigraph GT3X accelerometer in free living environment during seven consecutive days. The percent of time spent in sedentary, in moderate to vigorous physical activity and the number of steps per day were computed from the accelerometer data. Kinematics parameters did not correlate with physical activity. Moderate correlations were found between spatio-temporal parameters and physical activity, for instance timing of toe-off (r = -0.40, p = 0.03). Few physical examination parameters were correlated with physical activity, such as the hip flexors selective motor control (r = 0.69 with moderate to vigorous activity and r = 0.70 with steps per day, p less then 0.05). The physical activity profile cannot be sufficiently determined by a combination of clinical measures.A practice session is common prior to strength testing. However, the benefits of practice have not been previously reported. The purpose of this study was to determine the effect of a practice session on peak torque, mean torque and between trial variability across three test days. We hypothesized that peak and mean torque would be higher and less variable the second and third test days than the first. Twenty-five healthy, young participants completed 3 maximal voluntary isometric and isokinetic knee extensions on three separate days. No difference in isometric torque was found between days 1 and 2, but there was a significant decrease in isokinetic torque (8.45 Nm). There was a significant decrease in both mean isometric and isokinetic torque from day 1 to day 3 (12.67 and 13.59 Nm). Contrary to our hypothesis, no benefit from a practice session was found. Healthy, young adults are able to produce peak knee extensor torques on the first day of testing and do not demonstrate any benefit from additional testing. Thus, a practice day preceding isometric and isokinetic knee extensor strength testing may not be necessary when testing healthy, young participants, and may, in fact, negatively impact subsequent strength measurements.
To compare the clinical characteristics of pregnant women and perinatal outcomes with or without recurrent severe intrahepatic cholestasis of pregnancy (sICP), and identify possible factors associated with disease recurrence.

A retrospective study of 164,603 deliveries was performed to identify pregnant women diagnosed with sICP in the previous pregnancy from January 2012 to December 2020. Eligible patients were divided into two subgroups according to the status of disease recurrence in the second pregnancy recurrent severe ICP (r-sICP) and non-recurrent severe ICP (nr-sICP). Demographics, clinical characteristics, maternal and perinatal outcomes, and potential factors linked to disease recurrence were analyzed.

Totally 118 patients were enrolled and respectively classified into the r-sICP group (
 = 63) and the nr-sICP group (
= 55). The proportion of hepatitis B virus (HBV) infection (HBsAg+, HBeAg+, HBcAb+) and early-onset ICP (<28 weeks) in the r-sICP group in the previous pregnancy were higher than those in the nr-sICP group. In the second delivery, neonatal outcomes in the r-sICP group were worse than those in the nr-sICP group. Logistic regression analysis of predictive factors for disease recurrence in the second delivery revealed that the combination of HBV infection and early-onset ICP in the previous delivery had the steepest receiver-operating characteristic (ROC) curve value 0.720 (95%CI 0.629-0.812).

Patients with sICP displayed a higher recurrence rate in the second pregnancy. Being <28 weeks at the time of ICP diagnosis and having HBV infection in the previous delivery appear to be independent predictive factors for disease recurrence of sICP.
Patients with sICP displayed a higher recurrence rate in the second pregnancy. Being less then 28 weeks at the time of ICP diagnosis and having HBV infection in the previous delivery appear to be independent predictive factors for disease recurrence of sICP.Purpose To describe the spectral-domain optical coherence tomography (SD-OCT) features of typical cystoid degeneration. Methods This was a retrospective study of 11 eyes with typical cystoid degeneration (TCD). All patients had a complete ocular examination, ultra-widefield (UWF) pseudocolor fundus photography and SD-OCT with a 55° wide-field lens. We analyzed the cross-sectional structural information of SD-OCT imaging with TCD.Results On SD-OCT, the TCD regions exhibited rolling hills patterns with irregularly elevated retinal surface, and multiple intraretinal hyporeflective cavities separated by irregular septums were seen in the neurosensory retina. Destructive changes were seen in the ellipsoid zone and the pigment epithelium. Consolidated vitreous with moderate to high reflectivity was seen over the lesion and there might be vitreoretinal adhesions and tractions. Results SD-OCT shows exquisite structural features of the anatomy in vivo detail of the TCD. This imaging technique may deepen our structural understanding of TCD and may influence decision-making in clinical practice.
To identify whether COVID-19 vaccines should be administered in pregnant and breastfeeding women by reviewing the guidance and other evidence.

We reviewed the COVID-19 vaccination guidance for pregnant and breastfeeding women published to date and evidence from preclinical experimental and observational clinical studies, and discuss their implications.

Pregnant women were excluded from the initial phase 3 clinical trials of COVID-19 vaccines resulting in limited data on their efficacy and safety during pregnancy and postpartum. As a result, since December 2020, there has been conflicting advice from public health, governmental, and professional authorities on this matter. From the end of 2020 up to now, some consensus guidance has been published with a prevalent precautionary approach on the administration of vaccines in pregnant women, in breastfeeding ones, or for those who are planning a pregnancy (either spontaneously or with assisted technologies). After the first few months of vaccine administration in some countries, more permissiveness seems to prevail, although with inconsistencies.
