In most more developed countries, the total fertility rate (TFR) is below 2.1 and net immigration is positive. This paper proposes and calculates for 22 populations for 2011-15 a 'Current Migration Replacement TFR' which in combination with the mortality and absolute net migration for that period generates a stationary population equal in size to the mid-period population. The results show the Current Migration Replacement TFR ranges widely from 0.60 for Singapore to 2.05 for Slovakia. That the Current Migration Replacement TFR is below the 2011-15 TFR in 14 of the 22 countries shows that, when considered in combination with current migration and mortality, in most of the countries the current 'below 2.1' TFR is coherent with population increase, not population decline, over the long run. For New Zealand, Australia, Norway, Sweden and the UK continued current fertility in combination with constant mortality and constant absolute net migration is coherent with more than doubling of the current population size. The value of this measure for illustrating the interconnected population size implications of sub-replacement fertility and immigration, for sub-categorisation of 'post-transitional' populations by population growth prospects, and for guiding population policy is discussed.Do timing attitudes-subjective evaluations of particular ages as good ages to marry-influence entrance into marriage? We theorized that both parents' and children's timing attitudes influence expectations of when children will marry. We tested the model using intergenerational panel data from Nepal collected in 2008-2014. Timing attitudes of young people and their parents did influence expectations, as well as entrance into marriage. The Lee-Carter method and its later variants are widely accepted probabilistic approaches to mortality forecasting, due to their simplicity and the straightforward interpretation of the model parameters. This model assumes an invariant age component and linear time component for forecasting. We apply the Lee-Carter method on smoothed mortality rates obtained by LASSO-type regularization and hence adjust the time component with the observed lifespan disparity. Smoothing with LASSO produces less error during the fitting period than do spline-based smoothing techniques. As a more informative indicator of longevity, matching with lifespan disparity makes the time component more reflective of mortality improvements. The forecasts produced by the new method were more accurate during out-of-sample evaluation and provided optimistic forecasts for many low-mortality countries.Existing static and kinematic models of concentric tube robots are based on the ordinary differential equations of a static Cosserat rod. In this paper, we provide the first dynamic model for concentric tube continuum robots by adapting the partial differential equations of a dynamic Cosserat rod to describe the coupled inertial dynamics of precurved concentric tubes. This generates an initial-boundary-value problem that can capture robot vibrations over time. We solve this model numerically at high time resolutions using implicit finite differences in time and arc length. This approach is capable of resolving the high-frequency torsional dynamics that occur during unstable "snapping" motions and provides a simulation tool that can track the true robot configuration through such transitions. Further, it can track slower oscillations associated with bending and torsion as a robot interacts with tissue at real-time speeds. Experimental verification of the model shows that this wide range of effects is captured efficiently and accurately.Clinical manifestations of valproic acid (VPA) toxicity can range from just mild confusion and drowsiness to serious encephalopathy, leading to depressed sensorium and even coma and death. The exact cause(s) of how VPA influences the integrity of brain function remains unknown. Nevertheless, several mechanisms have been postulated including a surge in the blood ammonia concentration. Valproic acid-induced hyperammonemic encephalopathy is a rare yet serious sequalae and that can lead to grave outcomes. We report a case of hyperammonemic encephalopathy with preserved liver function following a moderate VPA intoxication in a toddler, who was successfully managed conservatively. Additionally, we briefly discuss mechanistic basis of VPA toxicity and highlight some of the available potential therapies.Kaposiform hemangioendothelioma (KHE) is a rare, vascular malignancy that is often associated with coagulopathies and thrombocytopenia secondary to platelet trapping. Typically, a person diagnosed with KHE with Kasabach-Merritt phenomenon (KMP) presents with a reddish-purplish lesion, thrombocytopenia, and elevated D-dimer, which can lead to high morbidity and mortality. Sirolimus has been identified as a treatment option for KHE with or without KMP for reduction in lesion size and hematologic parameters. In this case report, a female born at 26.5 weeks was noted at birth to have a purpuric lesion on her right upper back and flank area. She was diagnosed with biopsy-confirmed KHE with KMP. She was started on sirolimus 0.01 mg (0.02 mg/kg; 0.14 mg/m2) once a day, and because of high trough concentrations treatment was held until concentrations decreased. Sirolimus was then microdiluted to a 0.01 mg/mL concentration in medium-chain triglyceride oil for administration. Prior to discharge from the hospital the commercially available product was dispensed for home use. After 6 months of treatment, she achieved a reduction in lesion size and improvement in hematologic parameters, and treatment was stopped at 9 months.
