While reported cases are scarce in the literature, early recognition of sepsis and prompt withdrawal of vasoconstrictors in an intensive care setting, combined with timely heparinisation, appear to be the most common management strategy thus far.The principle of negative pressure technique dates back to the earliest civilizations; during the Roman era, the technique of using dome-shaped cupping glasses was used to create the suction needed to promote healing. This technique was used throughout the 19th century. In 1821, a British physician named Dr. Francis Fox invented the "glass leech" technique. Thereafter in 1952, an innovative approach was introduced to the treatment of serious, complex wounds through the use of sub-atmospheric or negative pressure known as "negative pressure wound therapy" (NPWT). Later, the "vacuum-assisted closure", or VAC therapy system founded by Dr. Louis Argenta in 1990 revolutionized the advanced wound care market, and still remains the most clinically proven alternative for the treatment of complex, hard-to-heal wounds. These therapies utilize a foam dressing that is conformed to the wound bed. When sealed and placed under negative (vacuum) pressure, the system creates a unique wound-healing environment that has been shling promotion and skin function restoration without any obvious side effects. In this review, we have hypothesized a novel modality, focusing on the treatment of wound complications secondary to comorbidity by a combination of negative pressure therapy followed by a positive pressure infusion with growth factor concentrates.Leech therapy, which can lead to complications such as anemia and bleeding, has been used to treat many diseases since ancient times. Furthermore, some substances in leech saliva are known to have anticoagulant effects. Acute mesenteric ischemia, which develops due to mesenteric vascular obstruction, can be treated medically or surgically. Non-occlusive mesenteric ischemia (NOMI) occurs as a result of decreased blood flow in mesenteric vessels due to hypovolemia, hypotension, etc. In this report, we mentioned a 57-year-old male patient who was admitted to the emergency department with syncope and weakness. In his medical history, the patient was stated to have used leech therapy to treat diabetic wounds on his feet, and prolonged and unstoppable bleeding was seen after leech bites. On his physical examination, there was tenderness in all quadrants of the abdomen. Abdominal computed tomography without contrast agent showed hepatic portal venous gas and pneumatosis cystoides intestinalis (PSI). The patient underwent laparotomy owing to the development of acute abdomen during the follow-up. Necrosis was seen in the terminal ileum and entire colon. Low flow in mesenteric vascular vessels of these necrotic segments was indicated with intraoperative Doppler ultrasonography. All necrotic segments were resected and open end-ileostomy was performed. The patient was discharged on the 17th day of follow-up. In conclusion, excessive bleeding caused by leech therapy can cause NOMI.Ovarian tumors occurring secondarily to metastatic colorectal carcinoma remain a rare occurrence. Since ovarian tumors remain predominantly asymptomatic in the initial stages, they are often diagnosed incidentally. The vague, non-specific symptoms elicited by a secondary ovarian carcinoma, coupled with a histopathology remarkably similar to that evoked by primary ovarian tumors, render its ascertainment a diagnostic challenge. We hereby delineate an interesting case of a metachronous ovarian adenocarcinoma in a 19-year-old patient with a prior medical history significant for colorectal carcinoma treated with hemicolectomy. Subsequent diagnostic workup divulged a mass in her left adnexal region, which was ultimately diagnosed as a metastatic colon adenocarcinoma to the ovary. Unfortunately, the patient succumbed to the aggressive malignancy and did not survive. We therefore aim to accentuate the diagnostic and therapeutic dilemmas fomented by ovarian adenocarcinomas that arise secondarily to primary colorectal cancers.Introduction Laparoscopic colonic resection is increasingly becoming popular worldwide and aims to provide curative resection in addition to the inherent benefits of laparoscopic surgery. The aim of this study was to evaluate the long-term outcomes of laparoscopic right hemicolectomy in a Pakistani cohort of patients. Methods and procedures We retrospectively analyzed the medical records of all patients who presented to our hospital with the diagnosis of right-sided colon carcinoma from January 2010 to December 2018 and underwent laparoscopic right or extended right hemicolectomy. Demographics, operative findings, histopathology report, and follow-up of patients were recorded and the analysis was performed on Statistical Packages for the Social Sciences (SPSS) Version 20 (IBM Corp, Armonk, NY). Results Seventy-five patients were included, 56 (74.7%) of whom were males and 19 (25.3%) were females. The median age was 52 years (range 25-82 years). The median hospital stay was five days (Range 3-13 days). The median blood loss was 70 milliliters and the mean operative time was 195.5±77.6 minutes. c-Kit inhibitor Laparoscopic extended right hemicolectomy was performed in 23 (16.67%) patients and standard right hemicolectomy in 52 (83.33%) patients. Most (72%) of the patients had a pathological T3 tumor, and the majority (61.3%) of the patients had no nodal involvement (pN0). The mean number of lymph nodes removed was 20+8. The median numbers of involved lymph nodes were 1.14+2.19. All the patients had R0 resection. Postoperatively, two patients had pelvic collection, and there was no 30-day mortality. Local recurrence occurred in four patients and distant metastases were observed in nine patients. The median follow-up in our study was 40.5±18.35 months. The median disease-free survival was 42±2.17 months and the median overall survival was 44±2.16 months. Conclusion Our experience with laparoscopic right colon resections has confirmed the safety and feasibility of the procedure.
