To gauge real-time respiratory motion management (RRMM) methodologies, treatment target areas, and implementation preferences/obstacles within particle therapy centers globally, an institutional questionnaire was distributed from July 2020 through June 2021. In order to establish recommendations regarding necessary actions and a future vision, a three-round DELPHI consensus analysis (October 2022) was undertaken. Across 17 countries, 70 responses were received. The data reveals a 100% response rate in Europe (23 out of 23 centers), 96% in Japan (22 out of 23 centers), and 53% in the USA (20 out of 38 centers).
From the 68 clinically operational centers, 85% leveraged RRMM, with 41% concurrently performing both rescanning and active methods. Sixty-four percent of the patients employed active-RRMM in at least one treatment area, frequently utilizing a system where the gating process was governed by an external marker. A desire to modify or augment their RRMM approach was expressed by 48% of active RRMM users. Limited resources and technical limitations served as the principal barriers. The DELPHI analysis, with unanimous agreement, identifies optimising rescanning parameters, improving motion models and pre-treatment 4D evaluation as important clinical priorities for the future. Analysis of clinopyroxene major element variations across crystals from five distinct lava fountains provides insights into the relative proportions of deep and shallow magma reservoirs fueling these eruptions. Hierarchical clustering (HC), an unsupervised machine learning approach, is used to objectively identify compositional clusters and their variations within clinopyroxene during this intense eruptive period. As our research demonstrates, variations in monitoring parameters and eruption intensity are reflected in the mineral record as both shifting cluster compositions and variations in the chemical complexities of individual crystals. We additionally incorporate random forest thermobarometry to establish the relationship between each cluster and its formation P-T conditions. The February to April 2021 eruptive series is attributed to the injection of a hotter, deeper magma, introduced into a storage zone under 1-3 kbar pressure, and subsequently mixing with slightly more evolved magma. The February 28th episode's production of mafic magma was the most extensive, combined with the highest average height of lava fountains and the greatest average discharge rate over time, thus marking it as the most intense phase of the observed eruptive cycle. Our study demonstrates that the episode resulted in a decrease in the deep magma source and a subsequent increase in the chemical complexity of the erupted magma, along with a reduced average discharge rate and a lower fountain height. We suggest this technique to rapidly, objectively, and effectively link petrological and geophysical/geochemical observations during ongoing volcanic activity. We project that the structured application of this method will expose the magmatic processes dictating the development of ongoing eruptions.
