It has been shown that at 3,200 m of altitude there is an increase in obstructive events in healthy children aged 7 to 16 years; on the other hand, it is known that SpO2 dispersion between individuals becomes wider as altitude increases, a phenomenon that is more marked during sleep. About 17 million Colombians live in regions between 2,500 m and 2,700 m, as do significant populations in other Latin American countries. This research aimed to characterize respiratory polygraphy sleep parameters in healthy, non-snoring children aged 4-9 years living at 2,560 m. We carried out home respiratory polygraphy in 32 children with a mean age of 6.2 years (range 4-9 years). The average recorded sleep time was 7.8 h, the median apnea-hypopnea index was 9.2/h, the obstructive apnea-hypopnea index had a median of 8.8/h (p5 4.2 to p95 17.9) and central apnea a median of 0.4/h. The median SpO2 was 93% (p5 90.5 to p95 94) and transcutaneous CO2 had a median of 39.4 mmHg (p531.7 to p95 42.3). The median oxygen desaturation index ≥ 3% was 11.2 and median oxygen desaturation index ≥ 4% was 3.9. Normal measurements for respiratory polygraphy obtained at sea level do not apply to children at altitude. If such guidelines are used, obstructive sleep apnea will be over-diagnosed, resulting in unnecessary adenotonsillectomies, among other interventions.
Diffusion-weighted imaging (DWI) is sensitive to head movements, which may cause signal losses because of motion-induced gradient imbalances. Prospective motion correction using fast optical tracking can attenuate these artifacts. Approaches include quasicontinuous updates of gradients and radiofrequency (RF) pulses or dynamically applying a rebalancing gradient to restore the gradient balance, but these prior methods used bipolar diffusion gradients. The goal of this project was to develop and evaluate a motion-insensitive implementation for the more common monopolar diffusion sequence.

A monopolar diffusion sequence was developed with motion updates before each RF pulse and each diffusion-weighting gradient. The sequence was tested in a phantom and human brain at b=1000s/mm
and rotational velocities up to 20°/s. Motion sensitivity, signal losses, and in vivo image profiles were compared between scans with and without intrasequence motion updates.

With typical motion parameters, intrasequence motion updates with optimal parameters reduced the motion sensitivity of DWI (motion-induced gradient moment imbalance) sevenfold. Optimal results were achieved by matching the echo time of the pulse sequence to an even multiple of the tracking system frame-to-frame period. Average signal losses and the frequency of signal dropouts in phantom and in vivo measurements were reduced when intrasequence updates were enabled, and quality measures of DTI analyses were improved.

Chemical exchange saturation transfer (CEST) MRI is versatile for measuring the dilute labile protons and microenvironment properties. However, the use of insufficiently long RF saturation duration (Ts) and relaxation delay (Td) may underestimate the CEST measurement. This study proposed a quasi-steady-state (QUASS) CEST analysis for robust CEST quantification.

The CEST signal evolution was modeled as a function of the longitudinal relaxation rate during Td and spin-lock relaxation rate during Ts, from which the QUASS-CEST effect is derived. Numerical simulation and in vivo rat glioma MRI experiments were conducted at 11.7 T to compare the apparent and QUASS-CEST results obtained under different Ts/Td of 2 seconds/2 seconds and 4 seconds/4 seconds. Magnetization transfer and amide proton transfer effects were resolved using a multipool Lorentzian fitting and evaluated in contralateral normal tissue and tumor regions.

The simulation showed the dependence of the apparent CEST effect on Ts and Td, and such reliance was mitigated with the QUASS algorithm. Animal experiment results showed that the apparent magnetization transfer and amide proton transfer effects and their contrast between contralateral normal tissue and tumor regions increased substantially with Ts and Td. In comparison, the QUASS magnetization transfer and amide proton transfer effects and their difference between contralateral normal tissue and tumor exhibited little dependence on Ts and Td. In addition, the apparent magnetization transfer and amide proton transfer were significantly smaller than the corresponding QUASS indices (P < .05).
