Employing a prospective cohort study with 190 pregnant women in central Taiwan, we examined the patterns of sleep quality and depressive symptoms, along with their predictors, in perinatal women throughout pregnancy and the postpartum phase. The Edinburgh Postnatal Depression Scale and the Pittsburgh Sleep Quality Index were used, respectively, to assess depressive and sleep-related symptoms from mid-pregnancy to the end of the first three postpartum months. Analysis of the data collected from the structured questionnaires involved the application of group-based trajectory modeling and logistic regression modeling. For the purpose of the study, pregnant women (predominantly primiparous) with an average age of 32.3 years, give or take 4.1 years, were enrolled. Sleep quality during the perinatal period displayed three recognizable patterns: 'consistent good' (184%), a pattern of 'worsening sleep quality' (489%), and 'unvarying poor quality sleep' (326%). Our analysis further revealed three consistent patterns of depressive symptoms: 'stable low' (363%), 'stable mild' (421%), and 'stable high' (216%). The development of depression was substantially influenced by sleep quality, a finding supported by a highly significant statistical result (p < 0.0001). High fatigue symptoms and a lack of social support were found to be associated with increasing patterns of poor sleep and depressive symptoms. Distinctive sleep quality dynamics and stable depression trajectories were noted. Our investigation uncovered a strong correlation between sleep patterns and depressive tendencies, with shared contributing factors including fatigue and social support networks. Promptly evaluating a mother's sleep quality and depressive symptoms is key for identifying vulnerable women in the perinatal period. This proactive approach allows for targeted interventions to enhance their sleep and mental health.
