This study aimed to investigate the clinical and pathological features of leprosy bacillus-related neuropathy in different clinical stages.
Four patients confirmed to have leprosy between 2012 and 2020 were chosen as the primary study subjects. The patients' clinical data were retrospectively analyzed to investigate the clinical and pathological features of the different clinical types of leprosy.
Each patient had a different degree of deformity. For instance, in Case 1, upon dermatological examination, the eyebrows were not falling off, and the face was slightly invaded. The great auricular nerve was thickened, and there were plaques on the back of the right hand with ill-defined edges but no itching. No atrophy in the thenar or hypothenar was observed, though pain and a warm sensation were noted. The swelling of the right leg was accompanied by sensory abnormalities. Moreover, the bilateral ulnar nerves were swollen, and demyelination changes were observed upon nerve biopsy. A small number of granulomas were noted in the nerve interstitium, and the acid-fast staining was positive. Additionally, in Case 2, on dermatological examination, scars were observed on the front of the tibia, and large, reddish, symmetrically distributed patches were seen on the back. Satellite foci were visible as well. Contracture was observed in the little finger, and the ulnar nerve was damaged. Skin and nerve biopsy revealed leprosy bacillus via acid-fast staining.

The aim of this study was to explore the effects of the expressions of micro ribonucleic acid (miR)-132 and sirtuin1 (Sirt1) on lung injury in sepsis rats, and to elucidate the regulatory relation between miR-132 and Sirt1.
The model of sepsis-induced lung injury was successfully established in rats via injection of lipopolysaccharide (LPS) into the caudal vein (model group). Before modeling, the rats were infused with miR-132 antagomir via the trachea (miR-132 antagomir group) or intraperitoneally injected with the Sirt1 activator (SRT1720) (SRT1720 group). Meanwhile, the rats injected with an equal volume of normal saline via the caudal vein were enrolled in the control group. The expressions of the inflammatory factors interleukin-6 (IL-6), IL-1β, and tumor necrosis factor-α (TNF-α) were determined using enzyme-linked immunosorbent assay (ELISA). Quantitative Reverse Transcription-Polymerase Chain Reaction (qRT-PCR) and Western blotting were applied to detect gene and protein expressions in lung tissueating the expression of Sirt1.
The aging of the population and chronic pain represents topical issues in developed countries. These often translate into polypharmacy, inappropriate medications, and adverse drug events, with the risk of misinterpreting these latter with new medical conditions, generating what is referred to prescribing cascade. Prescribing cascades may lead to the prescription of new drugs, which could cause new potential side effects and unnecessary costs for individuals and healthcare systems. Therefore, the purpose of our review was to collect a good deal of prescribing cascades examples involving pain therapy medicines, to help clinicians minimize drug-related clinical outcomes.
We search in MEDLINE database through PubMed, including 31 studies and 80 different examples of prescribing cascades.
The medications most commonly resulting in the initial drug therapy prescribed were represented by psychoanaleptics (27/80, 33.7%). Among adverse drug events, the most common one, misinterpreted as a new medical condition, was represented by tremor and extrapyramidal symptoms (20/80, 25%). This study provides a list of several examples of prescribing cascades in pain medicine and is essential to raise awareness of the potential dangers they could involve in all patient populations. Collaboration between clinicians and clinical pharmacologists may lead to more appropriate polypharmacy schemes.

This review discusses the impact of the neuro-hormone melatonin on skeletal muscle disorders based on recent literature data with the aim to clarify the utility of the melatonin therapy in patients affected by muscle diseases.
It has been pointed out the possible role of melatonin as a food supplement to cure muscular disorders characterized by muscle wasting. Oxidative damage has been proposed as one of the major contributors of the skeletal muscle decline occurring both in physiological and pathological conditions. It is known that excessive oxidant levels lead to mitochondrial damage, and in turn, contribute to apoptotic signaling activation and autophagic impairment. This condition is common in a variety of skeletal muscle disorders.
The scientific evidence enhances the antioxidant effect of melatonin, that has been demonstrated by several studies both in vitro and in vivo. This effect counteracts mitochondrial impairments and reduces oxidative stress and autophagic alterations in muscle fibers. Its beneficial role in restoring muscle decline, takes place mainly in atrophic conditions correlated to muscle aging.
The findings of the research suggest that melatonin may be considered as a valid dietary supplement, useful to prevent muscle wasting, in particular, in sarcopenia-associated diseases.
