In the 50 years since the expansion of the legal definition of charity for tax-exempt hospitals, there have been periodic regulatory actions at the municipal, state, and federal level to quantify charitable contributions and justify the deferral of tax revenues. The movement toward risk-based reimbursement in the last decade creates an incentive for a shift in hospital leadership understanding and approach to community benefit programs and services. The historical interpretation of community benefit as an issue of compliance with legal obligations is being questioned by forward-thinking hospital leaders, in recognition that more strategic resource allocation offers the potential to reduce financial risk associated with preventable emergency department and inpatient utilization. Recent actions in the policy arena to strengthen community benefit practices, as well as policies in related areas such as homelessness and behavioral health, challenge hospitals to strengthen their focus on prevention. Therefore, it is necessary to develop the parental preparatory professional support provided by midwives for expectant parents within Sweden. This study will evaluate the effects on expectant parents of receiving a combination of an "inspirational lecture" and "ordinary antenatal parental classes" compared with only "ordinary antenatal parental classes." Methods/Design This block randomized controlled trial included an intervention as a pilot study, in which expectant parents were randomized for (1) the inspirational lecture and ordinary antenatal parental classes (intervention group [IG]) (n = 66) or (2) ordinary antenatal parental classes (control group [CG]) (n = 60). Data collection with repeated questionnaires was conducted in the first week and 6 months after birth. Statistical analyses were conducted for participant characteristics, differences between parents within IG and CG, effects of the intervention, intention to treat, and internal consistency of the included measurements. Results The intervention showed a tendency to be gainful for one out of four outcomes related to birth experience, and parents' perceived quality of parental couple relationship consensus and sexuality and manageability. These results were more prominent for the partners. Parents within both the intervention and control groups reported decreased social support in the first 6 months after birth. Conclusion and Clinical Implications Overall, the concept of the inspirational lecture in combination with ordinary antenatal parental classes as parental preparatory professional support seems to be a valuable care intervention. However, this study was a pilot study and the results should therefore be interpreted with caution. More research is needed since childbirth and transition to parenthood are complex processes in need of comprehension.We examined age differences in global self-esteem in Japan from adolescents aged 16 to the elderly aged 88. Previous research has shown that levels of self-liking (one component of self-esteem) are high for elementary school students, low among middle and high school students, but then continues to become higher among adults by the 60s. However, it did not measure both aspects of self-esteem (self-competence and self-liking) or examine the elderly over the age of 70. To fully understand the developmental trajectory of self-esteem in Japan, we analyzed six independent cross-sectional surveys. These surveys administered the Rosenberg Self-Esteem Scale, which measured both self-competence and self-liking, on a large and diverse sample (N = 6,113) that included the elderly in the 70s and 80s. Results indicated that, consistent with previous research, for both self-competence and self-liking, the average level of self-esteem was low in adolescence, but continued to become higher from adulthood to old age. However, a drop of self-esteem was not found over the age of 50, which was inconsistent with prior research in European American cultures. Our research demonstrated that the developmental trajectory of self-esteem may differ across cultures.Since COVID-19 in the pediatric population is infrequently severe, the indirect costs of the pandemic, related to the measures implemented to deal with the spread of the virus, can be worse than the infection itself. To assess this issue, we evaluated the number of children vaccinated or evaluated for the most common diseases in a poor village in Sierra Leone, showing a worrisome drop in vaccinations performed and children evaluated for acute diseases. Our preliminary findings highlight that support is needed to guarantee basic services to children during the COVID-19 pandemic, particularly in poor settings where preventive measures can be lifesaving in the long term.The coronavirus disease 2019 (COVID-19), caused by SARS-CoV-2, is highly infectious and its ongoing outbreak has been declared a global pandemic by the WHO. Pregnant women are susceptible to respiratory pathogens and the development of severe pneumonia, suggesting the urgent need to assess the potential maternal and infant outcome of pregnancy with COVID-19. The intrauterine vertical transmission potential of SARS-CoV-2 also remains controversial. Herein, we discuss the potential effect of COVID-19 on maternal and infant outcomes based on current studies, including those published in Chinese, in a total of 80 mothers with COVID-19 and 80 infants. We also comprehensively explored the mother-to-child transmission routes of SARS-CoV-2, in particular the route of intrauterine vertical transmission. Given SARS-CoV-2 is a sister to SARS-CoV, of the SARS-related coronavirus species, we made a comprehensive comparison between them to learn from experiences with SARS. Although there is no evidence supporting the intrauterine vertical transmission of SARS-CoV-2, our comprehensive analysis suggests that the adverse maternal and infant outcomes caused by COVID-19 cannot be underestimated.
